EDWARDS EDITORIAL: Rapid Deployment with the EDWARDS INTUITY Elite Bioprosthesis: MIS-AVR Reoperations Made Simpler?Tommaso Danesi Hinna, Loris Salvador, San Bortolo Hospital, Vincenza, Italy

Optimal application of new devices typically evolves naturallywith time and experience. Yet despite the advantages of rapid deploy-ment (or sutureless) valves, including ease of implantation, fast learn-ing curves and the associated reduction of procedural times, clear-cutindications remain broadly undefined.

As Minimally-Invasive Surgical Aortic Valve Replacement(MISAVR) requires easily implantable devices, rapid deployment de-vices are especially suitable for UHS and RAT videothoracoscopy; inour view, potentially one of the more interesting applications is forredo-MISAVR patients.

Indeed patients undergoing redo-MISAVR due to a failed con-ventional stented, stentless or full root prosthesis, often require lengthyprocedures, with a high risk of injury during debridment especially insmall calcified annuli or during the removal of degenerated aortic rootstentless bioprostheses. In these challenging cases RDAVR could provethe difference.

We report herein our first two implants of an EDWARDS INTUITYElite valve in complex redo-MISAVR.

The first patient was a 55-year-old male with systemic and pul-monary hypertension, HCV chronic infection and persistent AFib. Thepatient underwent AVR in 2006 for severe AR and a 29 mm TorontoSPV (St. Jude Medical, Minneapolis, MN) bioprosthesis was implanted.In 2015, the patient underwent a minimally invasive upper J-resternotomy due to the recurrence of severe AR due to post-endocardi-tis with perforation of the non-coronary leaflet and a peak pressuregradient (PPG) of 37 mmHg.

Externally the aortic root was heavily calcified with massive me-diastinal adhesions; the valve hada fully calcified root, and severe fi-

brosis with a perforation of the non-coronary cusp.After leaflet excision, a 23 mm EDWARDSINTUITY Elite valve was easily implanted. Aor-tic cross clamp and CPB time were 57 and 117minutes respectively. The patient was weaned fromCPB with a minimal inotropic support. Postopera-tive TEE showed no PVLs and low gradients. Thepatient was extubated after 5 hours of AMV. Inotropic agents weresuspended after 6 hours. ICU stay and hospital stay were 2 and 9 daysrespectively. No transfusions were needed.

The second patient was a 57-year-old male affected by systemichypertension. In 2009 the patient underwent AVR for severe AR dueto aortic root enlargement; at that time a 25 mm homograft was im-planted. In 2015, the patient presented with a severe calcific degenera-tion of the homograft and underwent a minimally invasive upper J-resternotomy with a 21 mm EDWARDS INTUITY Elite. Aortic crossclamp and CPB time were 62 and 113 minutes respectively. No inortopicagents were needed. AMV time, ICU stay and hospital stay were 3hours, 1 and 6 days respectively.

These two cases demonstrate how the prosthetic choice at the timeof the first and second operation is crucial. The classical surgical ap-proach would have required a redo-Bentall-de Bono procedure with ahigher grade of surgical complexity and risk.

Rapid deployment bioprostheses demonstrated to be safe and suit-able for redo-MISAVR patients, allowing performing fast and effi-cient reoperation in a safe minimally invasive fashion. In our experi-ence redo-MISAVR has finally become “minimal” for patients and“simpler” for surgeons.

Industry embraced those founding pioneers and acknowlegedtheir spirit of innovation. “As one former CEO of one of themedical device companies phrased it, cardiac surgeons “were thebutton salesmen in the age of zippers.”

Dr. Falk also discussed the advances in today’s environment,and noted that there has been tremendous growth in percutaneousvalve therapies. In addition, meetings are rapidly evolving based oninterventional therapies. He has observed that industry is also

paying moreattention tointerventionaltherapy, as demon-strated by the declinein research anddevelopmentexpenditure within

companies, and also by venturecapitalists.”

“The acceptance of minimallyinvasive concepts into the main-stream of cardiac surgery was aclear validation of the early workperformed by ISMICS membersand is underscored by the fact thattwo of ISMICS’ former Presidents,Michael Mack and RandolphChitwood, later in their careers, alsoserved as Presidents of STS.”

In looking at today’s overallhealthcare landscape, Dr. Falkopined that “Creating value meansproviding new ways of health caredelivery that have value for life.”
