Dear ISACHD Members,
As the updates in this newsletter fromour various working group chairs andrepresentatives attest, ISACHDcontinues to make strides inadvancing its mission of promoting,pursuing, and maintaining excellencein the care of adults with congenitalheart disease (ACHD) on many fronts.
To highlight a few achievements, ourinternational APPROACHIS study,i.e., Assessment of Patterns ofPatientReported Outcomes in ACHDInternational Study, has enrolled over2800 subjects from 16 countries. Ithas become, no doubt, one of thelargest prospective studies ever
conducted in ACHD. Our Global Education Working Group, incollaboration with Cincinnati Children's Hospital Medical Center, hasassembled an impressive collection of online ACHD lectures within anACHD modular learning course. These may be accessed through ourwebsite (www.isachd.org) by clicking on the "ACHD Learning Center"tab. ISACHD is also intensifying its efforts to reach out to and betterserve the needs of all valued members of our ACHD teams, includingACHD fellows, nurses, advanced practice providers, and mental healthprofessionals. We very much look forward to discussing these and other initiatives withyou at our upcoming meeting during the American Heart AssociationScientific Sessions in Chicago. In addition to the updates and projectdiscussions with our new executive, representatives, and chairs, themeeting will feature the presentation of ISACHD's first annual LifetimeAchievement Award! News from AsiaPacificBy Clare O'Donnell This year in May, the ISACHD sponsored a session at the World HeartFederation Meeting in Melbourne, entitled "Adult Congenital HeartDisease an update for the General Cardiologist". Talks were given byTim Hornung, David Celermajer and Clare O'Donnell on topicsincluding Imaging in ACHD, intervention in ACHD, and Pulmonaryhypertension in ACHD, and also, Geoff Strange presented a paper basedon the date from the Australia/New Zealand ACHDPAH registry. Aformal plan was also made to encourage attendees to join ISACHD andparticipate in its global aims, which we hope had already bearing fruit!

During other sessions of the meeting, particularly inthe pediatric/congenital stream, there were reflections on the globalchallenges of ACHD care, including papers from Dr Craig Sable, onaddressing the needs of patients with unrepaired congenital heartdisease in subSaharan Africa, and a thought of provoking a paper fromDr Krishna Kumar from India, entitled 'Alternative models for takingcare of adults with congenital heart disease'.
News from SouthAmericaBy Amalia Elizari The 40th Argentinian Congress of Cardiology will be held in BuenosAires from October 15 to 18. During this Congress, it will be decided ofwhich CHD (Congenital Heart Disease) in South America will be incharge of gathering an information about the population of adult andpediatric patients bearing different congenital heart defects.
European NewsBy Els Pieper and Barbara Mulder From August 30th to September 3rd, the ESC congress was held inBarcelona, Spain, and attracted over 30,300 delegates. Besides manyACHD sessions during the entire conference, a joint ISACHD-ESCmeeting was organized on Sunday afternoon, with excellent speakers oninnovative interventions in ACHD. The session was cheerfully chairedby Gary Webb and Helmut Baumgartner. Our ISACHD president PaulKhairy himself gave an excellent talk on Electrophysiological devices inACHD: pacemakers, defibrillators, and beyond... Andrew Taylor gaveinsight in future MRI guided interventions, John Pepper showed theLondon results of external wraps for aortic root surgery, and AndreasEicken gave his thoughts about the future of transcatheter pulmonaryvalve therapy. Afterwards, we could enjoy a wonderful social event withdrinks and Spanish food at the Museum Nacional dÁrt de Catalunya inParc Montjuic. Next year, the ESC congress will be held in London.
Research Working Group
The ISACHD research working group has been tasked with the missionof identifying and helping implement multinational ACHD researchprojects. The research working group was formed in July of 2014. Weare pleased to announce the following membership: Sue Fernandes,Paul Khairy, Jolien RoosHesselink, Philip Moons, Clare O'Donnell,Francois Pierre Mongeon, Gruschen Veldtman, Barbara Mulder andJamil Aboulhosn.
The first such project to be endorsed by ISACHD research workinggroup is APPROACHIS, the acronym for Assessment of Patterns ofPatientReported Outcomes in Adults with Congenital Heart disease International Study. APPROACHIS is an international, multicenter research project onpatientreported outcomes in adult patients with congenital heart disease.Researchers and clinicians from 16 countries, across 4 continentscollaborate in one of the largest studies in the field of congenital heartdisease ever conducted.
The vision of the consortium is for APPROACHIS to be a project that
has a high impact on clinical practice and research in the field ofadult congenital heart disease;
allows us to provide a truly international perspective on patientreported outcomes.
This study is coordinated by the University of Leuven (KU Leuven), thePI is Professor Philip Moons PhD, RN. The steering committee includesProfessor Moons, Dr. Adrienne Kovacs and Prof. Koen Luyckx, PhD.The ISACHD research working group is in the process of reviewingmultiple multinational research projects and we are looking foradditional multinational studies to review and potentially endorse. Please contact the working group chairperson, Jamil Aboulhosn, MD([email protected]), if you would like to propose a multinational research protocol or get further information.
NOTEregistryNOACs in ACHD? The NOTE registry will evaluate its safety. Recognizing the Value of All Members of the ACHD TeamBy Sue Fernandes
A teambased approach is essential to ensure patientcentered care forcomplex patient populations. The type of team necessary to providehigh quality patientcentered care and the resources required for such ateam depend, in part, on the complexity of the underlying disease andadditional comorbidities.1
In few fields is the need for teambased care more evident than adultcongenital heart disease (ACHD). As this population continues to growsteadily, we are seeing a shift toward a greater percentage of patientswith moderate and complex forms of congenital heart disease (CHD).2 For the first time in history, large numbers of patients with moderate andcomplex CHD are reaching their 5th, 6th and even 7th decade of life. This improved survival allows these patients to live long enough to be

impacted by adult comorbidities such as acquired heart disease,diabetes, and cancer. Considering the many facets of care for ACHDpatients, it is clear that the type of team required to optimize outcomesmust be comprehensive, empowered by the resources necessary toensure a high quality patientcentered approach. The idea of the lone ACHD cardiologist is becoming obsolete. Theindispensable role that advanced practice providers (APPs) includingclinical nurse specialists, nurse practitioners, and physician assistants,along with nurses, social workers, and psychologists play in the daytoday care of ACHD patients is increasingly recognized. Adults with CHDconsume substantial healthcare resources, as reflected by the highnumber of outpatient visits, emergency room visits, and days ofhospitalization.3 Many ACHD physicians today partner with APPs tomeet the increasing need for health care access in the inpatient andoutpatient settings. While APPs are licensed and credentialed for ahigher level of direct patient care, the utilization of an office nursecoordinator in ACHD centers has not been fully examined, but is rapidlybecoming recognized as an integral part of a busy ACHD practice. AnACHD nurse coordinator can triage daily phone calls, maintain phonesurveillance of high risk patients, act as a clinical liaison and facilitatorfor insurance utilization if applicable, order and track testing, coordinatecare with sub specialists, and provide ongoing patient educationappropriate to changing clinical situations.4,5 In addition, ACHDpatients frequently require psychosocial or emotional support,6 much ofwhich can be provided by the ACHD nurse coordinator and APPs,although advanced psychosocial assessment should ideally be providedby individuals with expertise in mental health, such as social workers orpsychologists. The role that these mental health professionals play inACHD care is vital. To ensure access to this type of high quality,patientcentered ACHD care, it is essential that we embrace a teambased approach, recognize the value that each member brings to theteam and provide a venue for fostering multidisciplinary education,research and collaboration. At ISACHD, we value the ACHD team and all of its members. Westrive to be the organization that physicians, cardiology fellows, nurses,APPs, and mental health professionals turn to in the pursuit ofpromoting, maintaining and pursuing excellence in ACHD careworldwide. In the near future, we hope that you will see an increasingvisibility of the value we place on the ACHD teambased approach.Some of the initiatives under consideration include: adding workinggroups to meet the educational needs of nurses, APPs and mental healthprofessionals; expanding regional representation to include nonphysician members of the ACHD Team, including publications relevantto all members of the ACHD team in the ISACHD Journal Watch, andencouraging greater participation and leadership by nurses, APPs andmental health professionals within ISACHD.
