Late Applications

The Department will only accept your Application if it is late as a direct result of mishandling by the Department. In all other circumstances, in the interests of fairness, the Department reserves the right not to accept late Applications. In considering whether it would be fair to accept a late Application, the Department will take into account the degree of lateness, whether the cause of the lateness was beyond the Applicant's control and such other facts as it considers relevant. The Department may also ask the Applicant to provide evidence to support its claims regarding the reasons for late submittal. If the Applicant considers that their Application will be late they should notify the Contact officer prior to the Closing Time advising of the circumstances for the lateness. The chair of the assessment committee will take the reasons into consideration when deciding whether or not to accept the late Application.

PART A - GENERAL INFORMATION FOR APPLICANTS

1 Background and Policy Context

1.1 The Australian Government is building a better, fairer and more nationally consistent aged care system. The Living Longer Living Better aged care reform package provides $3.7 billion over five years. It represents the commencement of a 10 year reform program to create a flexible and seamless system that provides older Australians with more choice, control and easier access to a full range of services, where they want it and when they need it. It also positions the aged care system to meet the social and economic challenges of the nation’s ageing population.

1.2 The reforms give priority to providing more support and care in the home, better access to residential care, more support for those with dementia and strengthening of the aged care workforce. They have been progressively implemented from 1 July 2012 to give early benefits to consumers and providers but also to ensure there is a smooth transition for consumers and providers and sufficient time to adapt and plan ahead of further reform.

1.3 The Government has undertaken significant reform to the health system to bolster primary care and preventative care services to keep people out of hospital, to help people stay well in their community and importantly to ensure the health and wellbeing of Australians as they age. As part of Better Health Connections in the aged care reform package, the Government has a strong focus on building links between aged care and the health and hospitals systems.

1.5 Under the Better Health Care Connections: Models for Short Term, More Intensive Health Care for Aged Care Recipients Program (the Program), the Government is providing $25 million (including indexation) to support initiatives to encourage aged care providers to work with public and private health care providers and medical insurers to deliver short term, more intensive health care services. This will result in improved access to complex health care, including palliative and psycho-geriatric care. The Government is providing this funding over five years from 2012-13 to 2016-17 for seed grants to develop innovative models of health care services for aged care recipients.

1.6 The aim of the Program is to improve the quality of health care for aged care recipients.

1.7 You can find more information about the Program in the Grant Program Guidelines (the Guidelines) in Attachment A.

1.8 The Department has developed a formal assessment process, ranking system and an approach to underspends as outlined in the Guidelines. The Department will work with successful Applicants with the aim of having Standard Funding Agreements for those ranked “highly suitable” executed first. The remaining suitable Applicants will be engaged in subsequent stages. All organisations will need to be sufficiently staffed, networked and well established to undertake their projects as per their Applications. It

is anticipated that funding negotiations will be concluded prior to 1 March 2013 with projects commencing on 8 March 2013.

1.9 A template of the Standard Funding Agreement is at Attachment B.

2 Funding Objectives

2.1 The Program’s objectives are to:

2.1.1 develop innovative models to build better health care connections between existing health and aged care services, that are cost effective and improve the way the aged care system works within the broader health system;

2.1.2 improve access to complex health care services for aged care recipients;

2.1.3 increase awareness and information on successful models amongst aged care and health care service providers; and

2.1.4 diversify the aged care sector to include more complex community and residential health care services for aged care recipients.

3 What Will Be Funded?

3.1 Seed grants will be provided for activities including:

3.1.1 as a primary task, the development of models that build partnerships and innovative ways of working to join up the range of short term, more intensive health care services within a local area/region from which aged care recipients may need assistance;

3.1.2 establishment phase activities to enable operational aged care providers to be ready to start accepting aged care recipients for the short term, more intensive health care services; and

3.1.3 development of promotional material to encourage the uptake of practice within the aged care sector in the long term.

3.2 Seed grants will be provided in support of Applications which:

3.2.1 are led by a suitably placed and experienced operational aged care provider; and

3.2.2 enable realistic achievement of the Program’s aim and objectives within a local area/region.

3.3 Short term, more intensive health care services, such as palliative and psycho-geriatric care, may be provided to aged care recipients through:

3.3.1 in-reach subacute care services provided by accredited hospitals in residential and community settings; and

3.3.2 a broader range of more flexible and complex health care services provided in aged care homes and the community.

3.4 The health care services may range from hospital subacute services (those only provided by accredited hospitals) to a small number of simple interventions provided in aged care homes which potentially reduce unnecessary transfers to hospital.

3.5 As part of this arrangement operational aged care providers will continue to provide Specified Care and Services as required by the Aged Care Act 1997. Additional health care services will be provided by accredited hospitals, medical practitioners and other eligible health care providers using existing funding platforms and regulatory frameworks.

4 What Will Not Be Funded?

4.1 The Department will not fund activities that:

4.1.1 do not meet the criteria in Part A (5.1);

4.1.2 do not meet the mandatory criteria in Part D, Section 1;

4.1.3 do not meet the assessment criteria in Part D, Section 3;

4.1.4 duplicate existing projects or activities; or

4.1.5 only assist an individual without having any impact on the wider community.

4.2 Some specific examples of activities which will not be funded include direct health care service delivery such as palliative care and psychogeriatric care provided to aged care recipients.

5 Who Can Apply for the Funding?

5.1 For legal and accountability reasons, only incorporated bodies are eligible to apply for seed grants under this Program.

5.2 The Department is seeking Applications from interested operational aged care providers that have the capacity to best implement the Program. To be eligible for seed grants Applicants must be an approved provider of residential or community aged care with at least one operational service. Applications will not be accepted from non-operational aged care providers.

5.1 Favourable consideration will be given to operational aged care providers who form collaborations, consortia or partnerships with public and private health care providers and/or other organisations as required or as necessary to meet the selection criteria and health care needs of specific aged care recipients.

5.2 Joined-up or multi-regional approaches with an operational aged care provider as the lead organisation will also be considered if it can be demonstrated that such an approach is a more effective and efficient way to deliver short term, more intensive health care to aged care recipients.

5.3 Where the Application is a joint Application with one or more other organisation, an operational aged care provider with at least one operational service must be identified as the lead organisation and an authorised representative of the lead organisation must sign the Application.

6 How Much Funding is Available?

6.1 Funding of $7.59 million (exclusive of GST) from 1 January 2013 to 30 June 2014 is available under this Invitation To Apply for eligible organisations to develop innovative models for short-term, more intensive health care services for aged care recipients. The Department anticipates that the first payment of funding to the successful Applicants will occur in March 2013.

5.3 Program funding will be administered based on an open competitive selection process. Invitations To Apply will be released for each open competitive funding round conducted.

6.2 Funding provided under the Program is intended to be broad in scope and flexible to support a wide range of activities that enable aged care providers to build relationships with public and private health care providers and medical insurers for the delivery of short term, more intensive health care services.

7 Timeframes

7.1 The Department anticipates that it will select the successful seed grant Applicants in early 2013. The Department will contact the successful Applicants after they have been selected to discuss the preparation of the Standard Funding Agreement (see clause below).

7.2 The Department will not notify unsuccessful Applicants until it has executed Standard Funding Agreements with the successful Applicants.

8 Standard Funding Agreement

8.1 The successful Applicants for seed grants will be required to sign a Standard Funding Agreement with the Commonwealth, represented by the Department, before receiving any funding.

8.2 The Schedule attached to the Standard Funding Agreement will provide a detailed description of the project based on the Application. The funding will be paid in instalments throughout the project. The payment of funding is generally based on the funding recipient's achievement of outcomes and provision of deliverables (such as progress reports) and the demonstration of need for the instalment.

8.3 Applicants should note that, if they are successful, some details of their project (including a project outline, their name and the amount of funding awarded) will be posted on the Department's website as part of Department’s legislative reporting obligations.

9 Insurance

9.1 All Applicants should ensure that they have in place, or if successful are able to obtain, sufficient insurance to comply with the requirements for insurance specified below.

9.1.1 Workers Compensation to an amount required by law;

9.1.2 $10 million Public Liability; and

9.1.3 $1 million Professional Indemnity.

9.2 A Standard Funding Agreement with a short listed Applicant will not be executed until the required insurances have been obtained and copies of certificates of currency for the required insurances provided to the Department.

PART B - ASSESSMENT OF APPLICATIONS

1 How Will the Applications for Funding Be Assessed?

1.1 An assessment committee will be established by the Department to assess Applications against the assessment criteria and select the successful Applicants. The assessment committee will consist of officers from the Policy and Evaluation Branch and the Program Funding and Procurement Services of the Department.

1.2 The assessment committee will undertake an initial screening of Applications received against Part D, Section 1, 2 and 3 to ensure mandatory and assessment criteria are met and Applicants meet the minimum requirements such as:

1.2.1 Legal Entity;

1.2.2 required insurance levels or willingness to obtain;

1.2.3 completion of Sections 4 and 5;and

1.2.4 inclusion of letters of support for joint Applications.

1.3 Only those Applications which successfully meet all the Mandatory Criteria will be assessed by the Assessment Committee against the criteria in Part D, Section 3 ‘Funding Request’.

1.4 The First Assistant Secretary, Ageing and Aged Care Division, will be the Funding Approver.

2 Assessment Criteria

2.1 All Applications will be assessed against Part D, to ensure value with public money is achieved to meet the aim and objectives of the Program.

2.2 When assessing the Applications for seed grants, the assessment committee will use the assessment criteria in Part D, Section 3.

2.3 The assessment criteria are not listed in a particular order but do have equal importance.

2.4 In assessing an Application against the assessment criteria, the assessment committee may consider the Applicant's financial viability and the risk of the Application.

2.5 In assessing an Application the committee may use material included in response to one assessment criterion in the assessment of other criteria.

2.6 The assessment committee may also seek information about any Applicant from any other source, including from within the Commonwealth, whether or not the individuals or organisations contacted are nominated as referees by the Applicant.

PART C - HOW TO APPLY FOR FUNDING

1 Complete the Application

1.1 You must submit your Application for seed grants on the official Application in Part D.

1.2 The Declaration in the Application must be signed by a person authorised to act on behalf of the Applicant.

1.3 The Application must be completed in English.

2 Non-government or Private Sector Organisations.

2.1 If you are from a non-government or private sector organisation, you must include in your Application:

2.1.1 one (1) copy of the organisation's Certificate of Incorporation; and

2.1.2 one (1) copy of the organisation's previous financial year's audited financial statements. The Department may ask for further information about the Applicant's financial viability.

3 Joint Applications

3.1 If you submit a joint Application for funding, your Application must include a letter of support from each additional organisation (apart from the lead Applicant) which will be involved in the project.

4 Complete the Application Checklist

4.1 Before you submit your Application, complete the following checklist to ensure that your Application is complete.

5 Submit Your Application

5.1 You must submit your Application for seed grants in accordance with the submission details and methods on the cover page of the Invitation To Apply for Funding.

5.2 You are responsible for ensuring that your Application is completed accurately.

5.3 You may not alter the substance of your Application following its submission, except if you discover an error in your Application you should inform the Contact Officer immediately in writing. The Department may seek information from you to clarify any aspect of your Application, but it is not required to do so.

5.4 Prior to the closure of the Invitation To Apply for Funding, if the Applicant discovers an error in their submission, you should inform the contact officer immediately in writing.

5.5 The department reserves the right to reject incomplete, emailed or faxed Applications.

6 Retain a Copy of Your Application

6.1 It is suggested that you retain a copy of your Application for your records.

Proposed Model

The proposed model section should include the following information:

name of the project;

project aim, objective and outcomes;

identified geographical area and the target population for the project;

estimated number of aged care recipients to be assisted (if known);

a map of the health care services and other support systems in the local area/region;

how well the model meets the Program’s aim and objectives in the specified local area/region;

how the model will build partnerships and innovative ways of working to join up the range of short term, more intensive health care services within a local area/region from which aged care recipients may need assistance;

how the model will enable the provider to undertake all establishment phase activities and be ready to start accepting aged care recipients for the short term, more intensive health care services;

how the model’s promotional material will encourage the uptake of practice within the aged care sector in the long term;

how the model will address ebbs and flows in demand for the services and how referrals to the services that are not accepted will be appropriately managed;

how the model focuses on what older people can do rather than on what they cannot do, such as enablement, re-ablement and restorative care;

how the model links with other health and aged care reforms, for example Local Hospital Networks and Medicare Locals;

how the model will be viable and operational within the existing service system architecture operating in the local area/region;

what is the readiness of the local area/region to start effectively implementing the project; and

Project Plan and Budget

(maximum 800 words)

The Project plan and budget section should include the following information:

who will be responsible for the management of the project;

how the project will be managed;

how the finances for the project will be managed;

a project plan that sets out the key dates, timeline and key milestones for the implementation of the proposed model within the local area/region;

what monitoring and reporting will be undertaken to manage the project;

clearly state when short term, more intensive health care services will be established, staffed and operational, ready to commence delivery of services to aged care recipients;

a detailed recurrent budget for the proposed model, identifying all associated costs in Australian dollars and identified as GST exclusive (these costs are to be itemised, where possible, according to the key stages and milestones in the project plan and cover the funding period); and

justification for budget costs should be provided and be limited to the costs of implementing the project within the local area/region (Applicants should demonstrate that the budget is sufficient to meet the project’s aim, objectives and outcomes, and describe the intended benefits of the investment for the local area/region to demonstrate value for money).

Applications for collaborations, consortia or partnerships must include letters of support from any other key partners (for instance relevant state/territory government agencies and funded services, Local Hospital Networks or Medicare Locals); and

The capability and capacity requirements section should include the following information:

experience in project management;

financial management experience when undertaking projects;

experience and track record working with people who are aged care recipients;

expertise and knowledge to equip the lead organisation to understand and form partnerships or working relationships with Commonwealth, state and territory health care services and supports;

expertise and knowledge to equip the lead organisation to understand and form partnerships or working relationships with local health care services and supports;

capacity to engage with health care service providers, share experience and information, and analyse and formulate assessments/plans;

strong networking ability, and a high level of communications and negotiation skills;

an understanding of what services are in the local area/region and who needs to be connected with whom to successfully meet the aim and objectives of the project; and

an understanding of the local area’s/region’s demographics and the number of people within the project’s target group to be involved in the project.

Applications that do not completely satisfy all capability and capacity requirements listed should describe how they will acquire these to successfully implement the project.

Assessment Criteria:

The assessment criteria are the criteria against which all eligible, compliant Applications will be assessed in order to determine their merits against the Program’s aim and objectives and, for competitive programs, other competing Applications.

Applicants should address all of the following assessment criteria when completing Section 3 of the Invitation To Apply to be considered for seed grants. Seed Grants will be provided on a competitive basis.

how well the Applicant’s proposed model meets the Program’s aim and objectives;

PART E - FREQUENTLY ASKED QUESTIONSIn this Part we have included some sample answers to frequently asked questions.

Can my organisation apply for more than one grant under this funding round?

Yes, an operational aged care provider may submit more than one Application.

Will late Applications be accepted?

The Department will only accept your Application if it is late as a direct result of mishandling by the Department.

In all other circumstances, in the interests of fairness, the Department reserves the right not to accept late Applications. In considering whether it would be fair to accept a late Application, the Department will take into account the degree of lateness, whether the cause of the lateness was beyond the Applicant's control and such other facts as it considers relevant. The Department may also ask the Applicant to provide evidence to support its claims regarding the reasons for late submittal.

When will my organisation find out if my Application has been successful?

The selection process may take some time to complete, but it is anticipated that Applicants will be notified of the outcome by early 2013.

Can a number of organisations submit a joint Application? Yes. Favourable consideration will be given to operational aged care providers who form

collaborations, consortia or partnerships with public and private health care providers and/or other organisations as required or as necessary to meet the selection criteria and health care needs of specific aged care recipients.

Joined-up or multi-regional approaches with an operational aged care provider as the lead organisation will also be considered if it can be demonstrated such an approach is a more effective and efficient way to deliver short term, more intensive health care to aged care recipients.

However, in order to manage the funds appropriately, if a joint Application is successful, the Department will enter into a Standard Funding Agreement with the nominated lead organisation. Additionally, the joint Application must include a letter of support in line with Part C, Clause 3.1.

What is a lead organisation?

An operational aged care provider with at least one operational service must be identified as the lead organisation and an authorised representative of the lead organisation must sign the Application.

The lead organisation for the project will, if your Application is successful, sign the Standard Funding Agreement, receive the funding and assume legal responsibility for performing the Standard Funding Agreement.

A lead organisation must be an incorporated body which is able to enter into the Standard Funding Agreement. The other partner organisations do not have to be incorporated.

Are there any GST or income tax-related issues involved in receiving funding?

The Department recommends that you seek independent advice regarding the income tax implications of receiving funding.

Can my organisation be allocated less funding than it asked for?

Yes. The Funding Approver will consider whether the proposal will make an efficient, effective, ethical and economical use of Australian Government resources, as required by Australian Government (Commonwealth) legislation, and whether any specific requirements will need to be imposed as a condition of funding. Funding approval is at the discretion of the Approver. Allocated funding may be a lesser amount than requested by an Applicant, should there be a need to fund additional projects in excess of anticipated numbers.

When will my organisation receive the funding?

If your Application for funding is successful, an initial payment may be made after the Standard Funding Agreement has been signed by your organisation and by the Department representing the Commonwealth. All other payments of the funding will be based on deliverables (i.e. progress reports) as set out in the Standard Funding Agreement.
