Invitation ADDENDUM No 6 Invitation For Bid – Environmental Housekeeping Services
Bid Number HS-1101 Group 1 Questions and Answers
1. What does estimated annual service volume represent? For High Desert Health Center Group
(HDHCG) sites, annual service volume is clinic visits, which also may include some ancillary services at certain sites.
2. Is square footage gross or cleanable? For HDHCG sites, square footage is gross. 3. Antelope Valley Rehab is open until 10:30 PM, but suggested ES coverage ends at 6:30 PM. Do they
not want someone to clean for the last 4 hours? 8 hours of cleaning per day will be sufficient. 4. At some medical centers, for example,
a. the services require contractor to handle trash pickup – please confirm that trash collection is only for the areas assigned to contractor to clean. All HDHCG sites are full-service. The contractor is responsible for trash pick-up from all areas except the concession cafeteria at High Desert Regional Health Center (HDRHC).
b. Will contractor be responsible for collection of medical waste and bio-hazardous waste. Please confirm that this responsibility is only for the patient care areas /laboratory cleaning that contractor is assigned. Yes – at all HDHCG sites that have medical waste and biohazardous waste, contractor will collect medical waste and biohazardous waste from patient care areas/laboratory and place in a designated medical/biohazardous waste holding area for pick-up by a separate contractor.
5. Pest control in the facilities – is pest control services for only the assigned buildings being cleaned? Or is pest control for the entire campus? Please advise. All HDHCG facilities are full-service. Pest control includes all campus buildings and exterior areas along building perimeter. At the Antelope Valley Rehabilitation Center, contractor is only responsible for pest control to the two Infirmary buildings that will be cleaned by the contractor. At the High Desert – Offsite Warehouse, contractor will only be responsible for pest control for the Warehouse building and the building perimeter.
6. Please provide floor plans for all facilities. Floor plans have been provided to DHS Contracts and Grants for all HDHCG sites. Plans will be re-sent in separate e-mails.
7. Please provide a floorplan for each building (where available) where services are requested. If floor
plans are not available, please provide a list of buildings with square footage where services are expected (USC+LAC, MLK, High Desert Regional Health, etc.) and broken out where multiple buildings exist on each campus. Floor plans have been provided to DHS Contracts and Grants for all HDHCG sites. Plans will be re-sent in separate e-mails. For HDRHC, the square footage breakdown is: Building A: 120,735 sq. ft. Building B: 15,630 sq ft. Building C: 5,635 sq. ft.

8. Please include floor plans and square footages for buildings where county employees provide the housekeeping services, but where the contractor is providing the floor care. N/A for HDHCG sites. This is a full-service contract for all HDHCG facilities and no housekeeping services are provided by County employees.
9. Please update Appendix C2, 3 and 4 with any Floor care Requirements. None are listed in the
scope. As indicated in the Facility Specs for HDHCG facilities, contractor will be responsible for submitting a comprehensive floor care plan for each facility which must be approved by the Facility and which must cover each flooring type. Please refer to the Facility Specs. This requirement is not noted for the Off-Site Warehouse and the Acton Rehabilitation Center, but will also apply to those sites.
10. In the sites where the contract is a full service and there are no county employees, please provide
volume data on the following areas: a. Annual Number of surgical procedures including Cath Lab, Endoscopy, C-
sections and other sterile procedures requiring between case and terminal cleaning. Based on the first five months of 2019, the estimated number of surgical and GI procedures that will be performed in the HDRHC ASC in Calendar Year 2019 is 1,716.
b. Annual Number of room discharges and transfers in in-patient, observation patient areas. N/A for HDHCG facilities – none are hospitals.
c. Annual Number of births N/A for HDHCG facilities – none are hospitals. d. Is UVC disinfecting required UVC disinfecting is not currently required. e. Is routine terminal cleaning required in ED, urgent care areas? Urgent Care
areas may require terminal cleaning on an as-needed basis.
11. High Desert Regional Health Center Appendix C-1 pg. 2-4 a. Pest control—Are animal and bird issues considered an variable cost? The only animal
issues currently part of the pest control program are rodent control.
b. Is the Physical Therapy equipment part of the scope? Yes
c. Do the Specialty Clinics procedure rooms require terminal cleaning? Specialty Clinic procedure rooms may require terminal cleaning on an as-needed basis.
d. Are the Sky lights included in the window scope and if so, what is the frequency? Yes, the
sky-lights will require cleaning two times per year.
e. Is the parking log included in the scope—no reference is made in the scope. Yes –the contractor will be required to provide parking lot sweeping services at HDRHC and the South Valley Health Center. At Antelope Valley Health Center, Littlerock Community Clinic, Lake Los Angeles Community Clinic and Acton Rehabilitation Center, the contractor is responsible for the services described in the Facility Specifications.
12. We will need the size of the compactors as well as the size of the trash bins (dumpsters) at all
locations where trash is included for accurate quotes from vendors. HDRHC: One 30 cu. yard compactor, One 6 yard bin, five 5 cu. yard recycle bins Antelope Valley Health Center: 1 5 cu. yard bin

performed for accurate quotes from vendors. HDRHC: Lot 1: 78489 sq. ft. Lot 2: 50,160 sq. ft. Lot 3: 51,600 sq. ft. Lot 4: 46,000 sq. ft. Lot 5: 38,190 sq. ft. SVHC: Square footage of the parking area is not available.
14. Appendix C-1 pg. 19---Acton Health Clinic has an estimated annual service volume of 350 cases per
year. Operating days are 365 per year. That is less than 1 case per day. Is this number correct? This number refers to provider visit only and does not include nurse visits, nurse sick-call and daily medication dispensing which add to the clinic workload.
15. Please verify that the contractor is to pay for all; t-paper, seat covers, roll towels, multifold towels,
feminine products, can liners? Yes

17. Purell Sanitizing stations; are they provided by County or contractor? At the present time, they are
provided by the County’s soap vendor. In the future contract, they will be provided by the contractor.
If contractor, will need to know how many per location? Please refer to the list for question 15 – hand sanitizers. Hand sanitizer is also provided with hygiene stations. Annual spend history by location? N/A
18. All surgery rooms – due to not seeing them; Dr’s use very specific type of cleaning agents before surgery, who is to supply? All cleaning agents will be provided by the contractor, but subject to approval by the Facility Project Manager.
19. Pest Control – please provide information as to interior/exterior requirements? Pest control will be
provided for all interior areas and exterior building perimeter. Please refer to revised Facility Specifications Appendix C-1 – see attachment
20. Trash Solid Waste - please provide the list of all current trash haulers at each location. All HDHCG
facilities are serviced by Waste Management. 21. Window washing – please be specific as to what locations services are to receive; (i.e. Harbor UCLA
state 3 x year, is this for interior/exterior? MLK Outpatient Center notes interior only, but does not state how many times per year)? Refer to revised Facility Specifications Appendix C-1 – see attachment - please note that for HDRHC, the louvers are also included in window washing.

24. Maids carts – what type are we to have at each location? Bidder is to propose all supplies necessary to perform the required services. County reserves the right to approve or disapprove any of the proposed items. Please refer to note in Facility Budget – Supplies and Services.
25. Number of trash bins – please verify where and if counts are correct by each location?

perform strip/wax or carpet cleaning)? Bidder is to estimate all staffing positions and number of staffing required to perform the required services.
27. Floor care – strip/wax – we do not see where in the bid how frequently; stripping/waxing/buffing or
carpet extraction or spin bonnet cleaning. For HDHCG facilities, contractor will be responsible for submitting a comprehensive floor care plan for each facility which must be approved by the Facility Project Manager. Refer to revised Facility Specifications Appendix C-1 – see attachment
28. How many sq. ft. by location are floors vs. carpets We do not have a breakdown of square footage
by flooring types. One purpose of the walk-through was to allow vendors to observe areas.
At HDRHC, in general: lobby/atrium areas are Terrazzo, Clinic areas are vinyl plank flooring, offices/conference rooms/open office areas are carpet. In addition, there are some specialty areas that have sheet vinyl (Ambulatory Surgical Center, procedure rooms, laboratory, etc.). Other specialty flooring areas include Therapy Services. The Warehouse has concrete floors. At South Valley Health Center, in general, the waiting, hallways, and clinic areas have VCT tile and offices/conference rooms have carpet. At Littlerock Community Clinic and Lake Los Angeles Community Clinic, in general the waiting areas and offices have carpet and the clinic areas have VCT tile. At Antelope Valley Health Center, the lobby, hallways and clinic areas generally have vinyl plank flooring, office/conference areas have carpet. A few areas still have VCT tile (x-ray, storage, etc.). A few areas in the Public Health Clinic have sheet vinyl. The Off-site Warehouse has concrete floors, with some VCT tile.
29. Please provide floor plans by each location to be serviced – we don’t need blue prints of sites, just
layout of areas we are to services, which identifies; offices, hallways, restrooms, breakroom, showers, warehouse, exam rooms, surgery areas, etc. etc. these floor plans should not include areas that are done by County staff (in house). see attachment - all HDHCG facilities are full-service with all areas to be cleaned by contractor. The one exception is the kitchen portion (behind the serving counter) of the small concession cafeteria at HDRHC.

30. Does the County use a work order system to generate request for services, if yes do we have access to this system to close out work orders? No. Housekeeping work orders are primarily requested through phone call or e-mail.
31. Is there a specific tracking software for labor planning, if yes does the County supply or does the
vendor, if the vendor what type are, we to use? No – Contractor may choose its own tracking software.
32. Are two-way radios provided by the County for staff to communicate, if no is, they to use their
company or personal cell phones? HDHCG requires Contractor to provide communication devices for staff via use of pagers and company cellphones, must be in working order while on duty.
33. On the floor plans please indicate spaces that contracted EVS is responsible for both floor care and
cleaning, just floor care or not responsible for anything. All HDHCG facilities are full-service and the contractor is responsible for floor care and cleaning in all areas. One exception is the small concession cafeteria space at HDRHC, where the cafeteria contractor is responsible for cleaning and the housekeeping contractor is only responsible for the Terrazzo portion of the flooring.
34. Who does linen distribution at each location? The High Desert Regional Health Center Linen
Department distributes clean linen at each site. Soiled Linen is collected by the Housekeeping contractor.
35. How many births per year at each hospital location? ? N/A - none of the High Desert Health Center
Group (HDHCG) facilities are hospitals and there are no births. 36. Average daily census at each hospital location? N/A - none of the HDHCG facilities are hospitals so
there is not an average daily census. 37. How many private and semi private rooms at each hospital location? N/A - none of the HDHCG
facilities are hospitals. 38. How many surgery cases per year at each location? Based on the first five months of 2019, the
estimated number of surgical and GI procedures that will be performed in the HDRHC ASC in Calendar Year 2019 is 1,716.
39. How many ED visits per year at each location? None of the HDHCG facilities are hospitals and none
have Emergency Departments. 40. Please provide the type of paper / plastic / hand soap products used at each location At the present
time, the County only provides the soap/foam/hand sanitizer projects. The current vendor for soap/foam/hand sanitizer products is Gojo.
41. Does the vendor provide own communication devices at all locations? The County will provide a
phone for the housekeeping office. Any other communication devices will be the responsibility of the contractor.

42. How many discharges plus transfers per year at each location? ? None of the HDHCG facilities are hospitals. There are transfers from the urgent care clinics at HDRHC and South Valley Health Center to outside hospitals. Exact numbers are not available, but the average is less than 3 per day.

Contractor is responsible for the collection of medical and bio-hazardous waste. The Contractor will stage the waste in a designated storage area and a separate contractor will remove the waste.
The following areas must be cleaned during regular business hours when staff are present: Pharmacy, Sheriff Office, Gift Shop, Volunteer Office, Medical Records, Information Services, Facilities Management, Communications.
Contractor will spot clean all upholstered waiting room furniture quarterly, and as needed, in accordance with manufacturer's recommendations
Contractor will provide quarterly pressure washing for high traffic exterior sidewalk areas on a quarterly basis. Contractor will utilize industrial self-contained equipment 3,500 psi and 210 degree hot water or steam, as needed. Contractor will utilize approved soaps and degreasers to effectively remove dirt and containments. The pressure washing system must contain, collect, and clean the wastewather at the site of each cleaning operation.

REQUIRED SHIFT COVERAGE
Contractor will be responsible for providing a comprehensive floor care program, approved by the facility, for each flooring type vinyl tile, vinyl plank flooring, sheet vinyl, tile, terrazzo, etc.). The approved floor care program will specify the facility approved cleaning solutions, wax/sealer products, equipment, procedures, tasks and frequencies. All products and procedures must be consistent with manufacturer recommendations for each flooring type. Floor care program must include detailed cleaning that is completed around door frames, in corners and wastes receptacles and other items. Examples of tasks to be included in the floor care plan include mopping, buffing, application of wax/sealers, scrubbing, stripping, removal of marks, as applicable to the specific flooring type and each area and as approved by the facility contract manager. Comprehensive floor care plan will include all specialized flooring types, such care plan will include products, procedures and frequency for cleaning and sealing of grout in tiled areas.Contractor will clean Building A Atrium's high area once annually after hours. Cleaning includes dusting, cleaning of all window ledges and light fixtures and HVAC vents. A boom lift will be required to perform cleaning.

2FACILITY CLEANING REQUIREMENTS

The Facilities will be maintained with a clean appearance, and all work will be performed by Contractor in a professional manner using good quality equipment, supplies, and materials.

REQUIRED SHIFT COVERAGE
Contractor will provide quarterly pressure washing for high traffic exterior sidewalk areas on a quarterly basis. Contractor will utilize industrial self-contained equipment 3,500 psi and 210 degree hot water or steam, as needed. Contractor will utilize approved soaps and degreasers to effectively remove dirt and containments. The pressure washing system must contain, collect, and clean the wastewather at the site of each cleaning operation.
Contractor will be responsible for providing a comprehensive floor care program, approved by the facility, for each flooring type vinyl tile, vinyl plank flooring, sheet vinyl, tile, terrazzo, etc.). The approved floor care program will specify the facility approved cleaning solutions, wax/sealer products, equipment, procedures, tasks and frequencies. All products and procedures must be consistent with manufacturer recommendations for each flooring type. Floor care program must include detailed cleaning that is completed around door frames, in corners and wastes receptacles and other items. Examples of tasks to be included in the floor care plan include mopping, buffing, application of wax/sealers, scrubbing, stripping, removal of marks, as applicable to the specific flooring type and each area and as approved by the facility contract manager. Comprehensive floor care plan will include all specialized flooring types, such care plan will include products, procedures and frequency for cleaning and sealing of grout in tiled areas
