• A commitment shown by both the CCG and Specialist Diabetes Team

• Service Specification developed

• Management are now sorting out the contracts and finance

We hope to ….

• By the end of the year we will have at least 3 Specialist Diabetes Practitioners.

• Have identified Lead Diabetes GP and Practice nurse in each surgery.

• A survey undertaken to identify surgeries needs.

• Set education and update sessions for the following year.

• Reduced the DESMOND waiting list
