No: 0BORF LONG-TERM POWER SOLUTIONPage 3 of 41.0 PURPOSEThe main purpose of this document Inspection and Test Plan (ITP) is to ensure the qualitycontrol and testing activities are in-placed and performed with quality and safe in a differentactivities by the QA/QC inspection personnel of Fatimah Bersatu Sdn Bhd (FBSB) and ownersrepresentative.2.0 SCOPEThe inspection and test plan covers allinspections, checking and testing activities to beperformed at all activities related to Instrument Installationscope of works suchasSCADA-RTU, Fire & Gas Trip signaling and Process Control System Trip indication status of switchgear andetcc.Themainscopeis to ensure all Instrument installation such as SCADA-RTU, Tripsignalofswitchgear,feedbacksignaltoMainPCS,fiberoptics are inspected and tested according toowners company standards and engineering global standard.3.0 ABBREVIATION / DEFINITION3.1 MRR - Material Receiving Report3.2 P.O - Purchase Order3.3 QA/QC - Quality Assurance and Inspection Department3.4 MOC - Murphy Oil Company (Owner)3.5 FBSB - Fatimah Bersatu Sdn Bhd3.6 ITP - Inspection and Test Plan3.7 SESCO - Sarawak SESCO BerhadINSPECTION TEST PLANDocument No:MOM-SK309-64-IN-ITP-0001-00Rev. No: 0BORF LONG-TERM POWER SOLUTIONPage 4 of 43.8 WITNESS (W) - An identified critical step in process whereby an inspectionActivities must be witnessed, inspected, approved and signed byFBSB inspection teamand owners representative as mentionedin the Inspection Request Form (IRF).- I RF shall be submitted 48 hours before the date of the inspection,on case-to-case basis and agreed both FBSB and Ownersrepresentativethe Inspection activity may proceed withorwithout OWNERs representative.3.9 HOLD (H) - The same as Witness (W) except that no further work canproceed on the affected item, or part of an itemuntil the workhas been inspected and approved through Inspection RequestForm (IRF) that shall be submitted 48 hours before Inspectionto be carried out.3.10 SPOT WITNESS (S)- Also known as Surveillance, a work in progress is allowed tocontinue but where an FBSB inspector and ownersrepresentative may invoke Witness or Hold attribute asnecessary.3.11 INSPECTION (I) - The process of checking and verifying on physical which occursparallel with flowof work and carried out according to ProjectProcedures Method of Statement code and standard etc.3.12 RECORD (RC) - FBSB inspection personnel shall record all necessaryparameters, datas and results which have been inspected orwitnessed into the relevant documents as required andwitnessed by owners representative3.13 REVIEW(R) - An activity that occurs in parallel with normal flow or workapplied to cross checking and verify previous obtain datareports certificate test record etc.4.0 INSPECTION ANDTEST PLAN(INSTRUMENT)(see attachment)
