TRANSCRIPT
Given the option to use electronic tools such as Microsoft Excel, participants will develop and use a method for identifying, recording, maintaining and updating information on all program sites in Maryland including local school system, school or site name, address, program contact name, position, phone number, and email address, with 100% accuracy.
Logic Model Template
Inputs – resources (funds, staff, materials), activities, and strategies
as designed by program creator and experts and theorized in research (using credible, accurate, reasonable and supported sources)
based on implementation o using a method for
identifying, recording, maintaining and updating information on all program sites in Maryland
o using a tool for conducting site visits of programs in Maryland and observing and recording program activities in a manner that is objective and reliable
Outcomes – short-term, intermediate, and long-term changes in knowledge, attitudes, skills, and behaviors
as designed by program creator and experts and theorized in research
expected by program implementers (using interviews)
expected by key stakeholders (using surveys)
stated in grant applications
Relationship – a logical explanation of how the inputs lead to the outcomes
is there a direct cause-and-effect relationship?
what assumptions are implied?
will this relationship be true in any context?
what factors might confound this relationship?
Program Name and Information LESSON 2: In order to identify inputs and
outcomes, we started by locating appropriate
sources

Program Name and Information LESSON 3: After locating appropriate sources, we analyzed them to find
inputs

Program Name and Information Next we’ll look at inputs based on
implementation by conducting site visits – do you know all your
sites?
Where is your program? Do you know all the locations in Maryland? Do you have reliable, up-to-date information about all program sites?
Maryland-Map.org
If you do keep track of program sites, how do you do it?
Do you track only local school systems, or all program sites (schools, grantees, community-based organizations)?
Who is responsible?
What technology do you use?
How do you know it’s accurate?
Update on a regular schedule
Keep in shared location
Train staff to use
Quality control
Avoid asking twice
How could you improve your current method?
What further information or help do you need?
Can other people help?
(1) Formal assessment
(2) Share your tool or process for tracking program information with the class by uploading your completed Assessment to Kazoo or Facebook.
(3) Have you learned anything new? Reflect on what you’ve learned privately or with colleagues.
