Please fill the details in the above screen with caution.

The details entered above are reflects in your application and these are not alterable once if yougenerate challan.

The Details entered in the challan are automatically transferred to application and they are not

alterable.

Please select the Date of Birth from the calendar only (You will get popup when you click)

The challan no / Regd..ID is unique and it will be useful for future correspondence with the

University till the admission. So you please preserve the no and student copy of the challan

The challan will have 3 copies (on single page). First copy will be retained by the SBI Branch

where you pay the fee, second copy has to be retained by you and the third copy has to be sent

to university along with application and other documents required (Refer prospectus for details)

Dont write anything on the Barcode printer on challan.

Pay the fee on any SBI Branch before the last date specified (09 th Feb 2012)

Once pay the fee, the bank officials will give you a Journal no (it will be written on the challan),

you please check that the Journal no written by bank officials is readable and legible clearly.

After payment of challan, you have to wait up to the next day 12:00 Noon to fill the application

by online.

4 HOW TO SUBMIT APPLICATION

After payment of fee at State Bank of India, applicants have to visit the website(http://www.pgntruhs.org) on next working dayto submit the application

The login screen will appear for the candidate to validate the payment details. The following screen willbe displayed. The screen will ask for Application No as login Id and Date of Birth (DDMMYYYY) aspassword(Example: 02101984 for 02/10/1984) to proceed for the application submission.

If the candidate is tried to fill the application before payment of the challan in bank, he will be informedwith the message Please wait, You have not paid the fee / Your challan information is not updatedfrom the bank, please wait upto 12.00 Noon of the next date of challan paid date. (sample Screen shotas below)

Note : Bank challan information will be updated every next day 12.00 Noon for the applicants who paid the fee today.(for ex. For the candidates who paid the fee on 01/02/2012 will be updated on 02/02/2012 at 12:00 Noon.)

5 Applicant can also see the display of message Fields marked with * are compulsory. Click on OK button toproceed to fill the online form to fill the application.

6. For candidates who studied MBBS in State wide institution have to submit the following information. It is Compulsoryfor considering the application.

7 The candidates who studied in India has to submit the marks obtained with subject names, maximum marks inthe screen provided (sample screen as below). Click the Button for calculating the Percentage and Total Marksfor your verification.

8. Upload Passport size Photograph with Signature

NOTE: You must upload photograph and signature to correct specified fields. Do not make any mistake in

uploading candidate signature and photograph. Photo/Sign should be in .jpg format.

1. Candidate must have scan / soft copy of passport size photograph photo.jpg and signature (sign on white paper

and scan it) as per the sample given below. Keep size of photograph and sign maximum size limit is 200 KB.

2. Put candidate signature in an area of 2 X 1 on paper with a black ball point pen . Scan that paper. Cut Signature

area of 2 X 1 and save it as Signature.jpg. Keep size of Signature maximum size limit is 100 KB.

3. Copy the signature in to the Clipboard (Using Ctrl + C) and open the photo.jpg file, and paste the Signature at the

bottom of the photograph as shown below and save the file as PhotoSign.jpg

4. To upload PhotoSign.jpg Click Browse Button right to the photograph field. Select the file from saved location

and click Open Button.

A sample of quality of acceptable and non acceptable photographs/signatures is given below

for the guidance of the applicants.

9 Sample Screen Shots for uploading Photo with SignatureThe applicant has to upload his/her Photo with Signature in the prescribed format by clicking on the browsebutton in the application form. By clicking on browse button the following screen appears.

11. Click on Check box to give declaration. Click on save button to proceed further. Sample screen is displayedbelow.

After completing the data entry in all pages of the online application and after submitting the data through

online, the candidate should take the print-out of the filled-in online application form (2 Copies). The

candidate should note the application number and e-challan (Appl.No) number which will appear on the

printout of application for future reference.

The candidate should paste a recent passport size photograph (identical to the photo that used for scanning)

in the space provided in the print out of online application form and to be attested by a gazzetted officer.

The candidate will be allowed to take a copy of printout at any time, but editing / adding information is notallowed once the application is submitted.

The candidate should enclose the attested copies of required Certificates SBI e-challan University copy

(original) for Rs. 1050/- for OC/BC categories and Rs.850/- for SC/ST categories to the print-out of the online

application form in support of his/her claim.

The candidate should sign at the declaration area in the print out of the online application form without fail.
