Action Learning is a focused strategy for organizational learning and improvement.

It is used to support teams who engage in collaborative learning as they solve defined problems or pursue specific opportunities

Data Across Sectors for Health • DASH, a national program of the Robert Wood Johnson

Foundation, was launched to align health care, public health, and other community sectors to compile, share, and use data to address social determinants of health and advance health equity.

• The Illinois Public Health Institute, in partnership with the Michigan Public Health Institute, serves as the National Program Office for DASH.

Assist n4a’s Aging & Disability Business Institute in:

• developing decision support and implementation tools for other CBOs making choices related to HIT and health care contracting

• creating a cohort of aging and disability CBOs with increased capacity to mentor and support other CBOs in this area

• document the experience of CBOs as they position themselves for successful service delivery partnerships, especially related to health care transformation
