Inspection Summary Purpose of this inspection was to conduct a full II and get the Inspection Scoping Forms completed for all of Williams - Northwest PL (WNW) Units. The team spent a total of sixteen weeks doing a very thorough review of WNW procedures, records, and field assets. There were a number of items found during the inspection that required attention from WNW. All items needing attention were addressed and found satisfactory by the end of the inspection and only one item was noted as a concern. I would recommend a LOC letter for the item of concern. The recommendation is based on the fact that WNW had already identified the valve concern and they have an active program in place to address it by 2019. However, the larger issue as a P&M measure to obtain response times within one hour has not been satisfied and this is where the concern is noted.

Result Issue Summary These valves should be replaced and made into RCV/ACV based on current conditions and response times. Estimated response times are most likely in excess of two plus hours to close them.

Result Notes Operator noted via their Risk Analysis that RCVs or ACVs were not required. However, while inspecting the area it was obvious that most valves could not be reached and operated safely. Every valve had the packing stem seals degrading where the operator required this void to be filled with heavy oil to make an affective seal when the valve was operated. This is shown by two valves that have been added to the valve stem in the attached picture to allow for the heavy oil to be injected in to the stem packing area.

While records were reviewed the list was used to verify that employees performing covered tasks and signing off on the records were qualified to perform the assigned covered tasks.
