Ineligible Providers

This communication is a reminder (see Blue Shield Reports S-09-03 and S-4-10) that the following providers are not considered eligible providers as defined in the local Blue Cross and Blue Shield of Kansas (BCBSKS) member contracts or for the Federal Employee Program (FEP). Their services cannot be billed incident to an eligible provider if they provide services. Athletic trainers, massage therapists, exercise physiologists, occupational therapy aides, physical therapy aides and chiropractic assistants. Services performed by these specialties are considered patient responsibility and should not be billed to BCBSKS. Ordering/Referring Provider Name and NPI As stated in Policy Memo No. 1, the name of the ordering provider, when applicable, (including NPI or specific performing provider number, except when exempt by law) must appear on every claim. The ordering/referring provider’s name and NPI should appear in Box 17/17b of a paper claim. Referring provider information should be submitted in the 2310A and/or 2420F Loop, and ordering provider information should be submitted in the 2420E Loop. Please refer to the 837P implementation guide or contact your vendor for more information.

NOTICE: AMENDMENT to Policy Memo No. 4 Effective Feb. 1, 2013 The Accountable Care Act requires health plans to be accredited through one of two United States Department of Health and Human Services (HHS) approved accrediting entities in order to be designated a Qualified Health Plan (QHP), and

thus eligible to offer its products on the exchange. BCBSKS has chosen URAC as the approved entity through which we will pursue this accreditation. As part of that accreditation process, we must credential our provider network. Currently, BCBSKS only credentials the TRICARE network as part of our responsibilities in accordance to the TriWest Network Subcontractor agreement. In order to begin credentialing our CAP network, we must formally amend Policy Memo No. 4 of our contracting provider agreement to reflect the new process. The CAP contract requires a 30-day written notice to providers to change any Policy Memo. This newsletter article serves as the 30-day notice. BCBSKS is making this Policy Memo change off cycle to our annual changes due to the time table we are operating under to become accredited. Our network must be credentialed by the time we undergo the URAC on-site review scheduled in June 2014. We currently have 4,800 providers credentialed in TRICARE (also in CAP), and we will need to credential an additional 4,500 providers. Based on the new workload and the ongoing recredentialing volumes, we will need to begin the credentialing process effective February 1, 2013 in order to be completed in advance of the URAC on-site review.

NOTICE: AMENDMENT to Policy Memo No. 4 Effective Feb. 1, 2013, continued If you are a provider who is not credentialed for TRICARE and do not have a Council for Affordable Quality Healthcare (CAQH) application, we encourage you to take this opportunity to complete the online CAQH application. This step will simplify your credentialing process when we begin credentialing the additional 4,500 providers. Access the CAQH online application at http://www.ksinsurance.org/industry/company/healthcomp.htm as referenced in the KID Bulletin 2007-2. Please contact us at 800-432-3587 or 785-291-4135, option 3, before completing the CAQH application to be assigned a CAQH ID number. A new section, VII. CREDENTIALING, will be added to Policy Memo No. 4. The verbiage for this new section follows: VII. CREDENTIALING

This process consists of an initial full review of the provider’s credentials at the time of application and at the time of recredentialing which occurs every 36 months. Credentialing criteria are available on the BCBSKS Web site at: http://www.bcbsks.com/CustomerService/Providers/Publications/professional/PolicyMemos/credentialing-criteria.htm

A. Initial/Recredentialing Applicant

The BCBSKS Corporate Credentials Committee (Committee) reviews each provider’s credentialing file in accordance with BCBSKS criteria, as well as BCBSKS and URAC standards. If a provider does not meet these standards or there is evidence that he/she does not adhere to BCBSKS policies and procedures, the Committee may deny or restrict participation in a BCBSKS network. If the provider disagrees with the denial or restriction and has additional information, he/she may request reconsideration by the Committee. In the absence of additional information, the provider may submit a written request for a first-level appeal.

B. Reconsideration If the Committee denies or restricts a provider’s participation status, the Committee will allow the provider to submit additional supporting documentation for reconsideration. If the denial or restriction is upheld by the Committee, the provider may submit a written request for a first-level appeal.

C. First-Level Appeal Panel All appealed disputes are referred to a first-level appeal panel consisting of at least three qualified individuals, of which at least one must be a participating provider who is not otherwise involved in network management and who is a clinical peer of the participating provider that filed the dispute.

D. Second-Level Appeal Panel If the First-Level Appeal Panel upholds the denial or restriction, the provider may submit a written request for a Second-Level Appeal. This provides consideration to a second-level appeal panel consisting of at least three individuals as defined in the first-level appeal panel and that were not involved with the first-level appeal panel.

The result of the appeals process shall be binding on the provider and BCBSKS subject only to the provision for binding arbitration previously stated in Policy Memo No. 1.

NOTICE: AMENDMENT to Policy Memo No. 4 Effective Feb. 1, 2013, continued The Credentialing Criteria, as stated in the policy above, are available on the BCBSKS Web site, at the URL noted above. Those providers currently in a credentialing cycle as a result of participation in TRICARE need to do nothing. Your recredentialing will occur on the same 3-year cycle. The credentialing department will follow a schedule to credential all other providers. You do not need to do anything to initiate this process; you will be contacted when information is required. Only if a new provider joins your group after February 1, 2013 do you need to contact Provider Network Services, 800-432-3587 or 785-291-4135, option 3, to discuss the credentialing process for the new provider. Precertification Precertification is for inpatient admissions, home health, and hospice cases only. The Precertification line is now functioning with a new phone prompt system. These prompts provide a quick access to Customer Service, for callers with specific questions regarding drugs, DME, outpatient procedures, and testing. These services would be considered possible Pre-auth/Predetermination questions and are not a Precertification function. If you need to speak to someone regarding any of these functions, please remember to follow the prompts when calling 800-782-4437.

Claim Status Detail Information on Availity Update Changes are coming to the Availity web portal for contracting BCBSKS providers. Providers can look forward to finding claims status detail information on the Availity web portal for BCBSKS members in 1st Quarter 2013. Providers can continue to view the claim status detail information in BlueAccess until further notice. BCBSKS will notify providers who are signed up for the Latest News on our e-mail notification system (eNews) of this upcoming change in early 2013 when providers are able to view the BCBSKS claim status detail information on Availity. Register today to access Blue Cross and Blue Shield of Kansas member eligibility and benefits and claims inquiry through Availity, a multi-payer portal that gives providers secure access to multiple payers’ information. Visit www.availity.com for additional information and to get registered today.

External Ocular Photography – 92285 Photographs in general are used by a variety of providers as an optional method of documentation. With the exception of external ocular photography – 92285, photographs have been considered content of service and not separately reimbursed. Effective January 1, 2013, in keeping with reimbursement policy regarding photographs in general, BCBSKS will consider external ocular photography content to the E&M service, and not separately reimbursed.

New Patient Conversion As outlined in Policy Memo No. 2, BCBSKS follows CPT rules for recognizing new patient visits: “a patient who is new to the practice/physician or a patient who has not been seen for three or more years. NOTE: Within a group practice, a consulting physician of a different specialty can bill a new patient office visit if the above criteria have been met. This does not apply to covering arrangements.” Effective January 1, 2013, BCBSKS will be converting any inappropriately billed new patient E&M to the corresponding established visit E&M. Place of Service Coding Using the correct place of service (POS) code on the claim form is an integral part of claims processing, and is necessary to avoid incorrect/improper payments. BCBSKS reimburses several procedures differently depending upon the location where they were rendered. When a procedure is performed in a physician’s office, urgent care center or independent clinic, the allowable charge accounts for the overhead expenses incurred in these locations. When the service is performed in a “facility”, such as a hospital outpatient department, the allowable is reduced to reflect the absence of overhead expenses to the physician or professional provider. The facility bills BCBSKS for the technical charges associated with the overhead component. BCBSKS does not require that a physician own the space they call an office; however, there should be financial liability (i.e., reasonable rent, staffing, mortgage, etc.). The physician typically employs the staff used in that space as opposed to the staff being employees of a facility. In most cases, when these provisions are not being met, the facility is charging BCBSKS a “room fee”, and therefore, the place of service would be considered out-patient hospital (22).

For diagnostic services, such as lab and x-ray, the POS code should be the setting in which the patient received the technical component. For example, a patient undergoes an MRI in an outpatient facility owned by the hospital, and the technical component is billed by the hospital. The correct place of service for the professional component billed by the radiologist would be 22 (outpatient hospital). Not on Availity Yet? Hundreds of Blue Cross and Blue Shield of Kansas contracting providers have been using the Availity® Health Information Network multi-payer portal for certain transactions with BCBSKS since early 2012. In early 2013, a new feature will be added, allowing providers to get additional claim detail when making a claims status request. Providers also access Availity for eligibility and benefits verification. For other online services, such as pre-certification and referrals, providers are re-directed to the secure section of the BCBSKS Web site without having to sign-on again. Increasing the online, self-service adoption rate among BCBSKS contracting providers is expected. However, these efficiencies and cost-savings can not be realized until all contracting providers begin using Availity. Therefore, beginning in the first quarter of 2013, BCBSKS will begin redirecting providers registered with Availity to the Availity Web site for all secure BCBSKS transactions. During the third quarter of 2013, BCBSKS will require all contracting providers to begin accessing secure information on their BCBSKS patients through Availity. Getting started with Availity is easy. Go to www.availity.com, click on the registration link and follow the instructions. Prescription Drug Changes for 2013, continued

Diabetic Test Strip Coverage Changes Effective January 1, 2013

Effective January 1, 2013, Roche and Bayer diabetic testing products will be preferred by Blue Cross and Blue Shield of Kansas (BCBSKS). Other products will be allowed on a limited basis, upon prior approval. What are the new preferred brands of diabetic test strips? ACCU-CHEK® Nano SmartView (Roche) ACCU-CHEK® Aviva Plus (Roche) CONTOUR® (Bayer) CONTOUR® NEXT EZ (Bayer) BREEZE®2 (Bayer) Why is Blue Cross and Blue Shield of Kansas requiring my patients to switch to the preferred products? These products provide quality glucose measurement and were determined to be the lowest cost products by competitive bid. What will I have to do to transition my patients to the preferred products? Write a new prescription for one of the preferred products. Will my patients be required to change if they are on insulin pumps that require the use of non-preferred test strips? No. Members with pump supply claims have been identified and will be exempt from this change.

Will I be able to prescribe a non-preferred diabetic test strip other than the Roche and Bayer products? Other brands may be approved through physician-initiated prior authorization with documentation of ONE of the following:

• Patient has visual impairment, OR

• The patient’s medication history includes use of any preferred glucose test strip/disc or meter product in the past ninety days, OR

• Patient uses an insulin pump that is not accommodated with a preferred glucose test strip/disc or meter, OR

• Patient has a physical or mental disability. How do I obtain a prior authorization form? Forms are available at www.bcbsks.com under Prescription Drugs. What support will my patients receive from Roche and Bayer, the makers of the preferred products? Each member will be able to receive a new meter and starter supply of one of the preferred products at no charge. Members currently using a non-preferred product have been notified of this change and given the information below to obtain a preferred meter.

Testosterone Therapy Effective Jan. 1, 2013, BCBSKS’ prescription drug benefits will include a prior authorization program for topical, oral and injectable androgens and anabolic steroids. Medical policy for approval, the prior authorization form, and a table of quantity limits are located at www.bcbsks.com. Drugs will be approved that are determined to meet the BCBSKS approved medical policy. For males to meet the criteria for testosterone replacement, the patient must have a measured pretreatment total serum testosterone level that is less than 300 ng/dL or a free serum testosterone level that is below the testing laboratory’s lower limit of the normal range. Workshop Opportunities 2013 Provider Workshop Offerings Once again, the professional relations department of BCBSKS will offer educational workshops to review BCBSKS programs, general benefits, current guidelines, proper billing and coding requirements as they apply to BCBSKS, and contact information to address future questions. In addition, BCBSKS will be offering some specialty workshops for 2013. The workshops, with the exclusion of the specialty provider workshops for Chiropractic, PT, OT, SP, AUD, DME, and Sleep Labs, will provide 3.5 hours of continuing education units (CEUs) pending approval from the American Academy of Professional Coders (AAPC). Granting of prior approval in no way constitutes endorsement by AAPC of the program content or the program sponsor. You must attend the full session in order to receive your CEUs and your certificate of accreditation. All insurance billing workshops are offered free of charge.
