Accountable Health Communities Program Now at Senior Neighbors

I am excited to inform you that Senior Neighbors has collaborated with Health Net of West Michigan to expand Health Net’s Accountable Health Communities program, a pilot project by CMS. The goal of Accountable Health Communities is to improve individuals’ overall health by meeting their health-related social needs. These needs include food, housing, transportation, utilities and safety. Participants become involved with the program by completing a screening tool, at certain health providers, to assess their need for resources. If the participants indicate that they have significant needs in any of the five health-related social need categories, they are then referred for navigation services. This means that the participant will partner with a Case Manager for up to a year to connect with resources to meet their needs. The primary goals are to improve
the participant's chances for better health outcomes and save money. Senior Neighbors is excited to be given the opportunity to offer this service in the community and help our clients maximize their health.

opportunity to attain their full health potential and that no
one should be disadvantaged from achieving this potential

Fitness Classes Far and Wide! It’s been a busy spring in the Health and Wellness department. We’ve started many new fitness classes and more are coming in the fall! It’s always exciting to know we are able to reach the needs of more people! Physical activity and fall prevention are so important for older adults. Just this week I saw a news story that shows the rate of fatal falls (falls that ultimately result in death) has increased significantly. United States statistics showed 8,600 fatal falls in 2000. In 2017, a total of 26,440 older adults died as the result of fall related injuries. That’s a lot of fall related deaths! Did you also know that falls have now surpassed motor vehicle accidents as the number one cause of brain injuries? I’ve said this before, and I know you will hear me say it again—these fall facts are scary. But, the good news is that most falls can be prevented. The most important thing you can do to prevent a fall is to be physically active. You don’t have to be a superstar, just do what you can. Every little bit helps. Get up and get moving as much as you can. There’s a lot of great ways to stay active, without even having to get out of your easy chair or turn off the television. Keep your leg muscles strong by practicing your sit to stands. And keep you ankles flexible by doing those ankle circles. Work on balance activities while you do dishes, make dinner or stand in the check out lane. But, if you are looking for a way to get more active and like to participate in group exercise classes, we have so many options for you! There are no set fees for these classes. Donations are accepted, and so is Silver&Fit, allowing you to use your benefit and save money!
Last month, I shared information with you on all our
EnhanceFitness locations. There are many to choose from, but since we have so many new sites I want to just
highlight them. Class times have changed, in some cases. And, I’m going to give you a sneak peak of things to come this fall! As always, if you have any questions or would like a complete schedule of classes, you can feel free to

From the Center Coordinator’s Desk I'm kind of nervous to admit it but it’s actually summer weather outside. Whew and right on time. We have a bunch of exciting things going on this month at our center. On Friday, July 19th we have our Annual GR Center Picnic at John Ball Park where we will have live entertainment, games and prizes. The following Friday will be our All Centers Picnic out at Grandville High School. Both events are going to be epic and you definitely don’t want to miss them. And as always if you have any questions or concerns my door is always open. Stop in and say Hi.

Summer Fishing on the Big Lake Love to fish? Have an interest in fishing the Big Lake? Then do we have something for you! It’s a little expensive but we
thought if you knew in advance you could save up for this event. Last year we chartered a boat for a Fishing Trip on Lake Michigan for $100 per person. We would like to do this again in August. So if you’re interested, save those pennies and look for the sign up sometime in June! If you have questions please talk with Perry Hughes.

Happy Independence Day to everyone as we watch our summer fly by! I hope you all are enjoying your summer and getting lots of opportunities to go outside to enjoy our beautiful weather.
We were able to get out and about around town in June and had a couple of events outside of the Center where we had a lot of fun together!
We have had a lot of new faces stopping by the Center and we hope this continues! If you know of anyone who might enjoy joining in on our fun, please invite them along with you!
We have a lot to offer seniors in this community, whether it be a fitness class, a delicious meal, a trip, or a great game of Hand & Foot. We encourage anyone to stop by and check us out! Happy 4th of July!

Retired Residents with the Golden Age Card. This card entitles retirees age 60 & older who live in the
Grandville School district free admission to all home games and most away games in our conference
division. Stop by the Community Education Office for your card.
RideLink Information:
The Rapid participates in RideLink, a network of area transportation providers. RideLink offers transportation for persons aged 60 or older to any destination,
including doctor visits, recreational visits, and shopping trips within Kent County. Providers include Hope Network, Hope Network Volunteer Transportation, Senior Neighbors, Kent County Community Action, United Methodist Community House and The Rapid. These agency’s vehicles provide trips for RideLink.
You must pre-register to use RideLink.
Rides are scheduled through RideLink. Call 774-1288 no
later than noon of the workday before your appointment.
• RideLink passengers are encouraged to make a $2.00
donation per trip. Donations are an important part of this program. The money goes directly back into the program to allow for continued service.
• Service hours are Monday – Friday, 7:30 a.m.– 4:30 p.m. and Sunday, 8:00 a.m. – 2:00 p.m.
• Rides must be cancelled through RideLink.

If you have concerns about falling, this class may be helpful! This eight week award-winning program for adults age 60 or older will focus on learning ways to manage fears, become more active and feel more confident in preventing falls. Exercises are done weekly from sessions 3-8. Each week’s topics build from the previous week, so attendance at all or most eight sessions is important. Space will be limited to the first 12 people who sign up directly with Senior Neighbors. Donations are accepted. Please call Senior Neighbors at 616-233-0283 to sign up or for more information.

This month’s volunteer is Jim VanStee.
Jim has been attending the Grandville Senior Center for the past 7 years.
Jim is a very active volunteer for the Center. He is President of the Advisory
Council.
Jim enjoys lunch, listening to music, outings and has been serving on the Advisory
Council for the last three years. Jim enjoys many activities outside the Center but the ones he cherishes the most are time spent
with his family.

RideLink Information:
The Rapid participates in RideLink, a network of area transporta-tion providers. RideLink offers transportation for persons aged 60 or older to any destination, including doctor visits, recreational visits, and shopping trips within Kent Coun-ty. Providers include Hope Network, Hope Network Volunteer Transportation, Senior Neighbors, Kent County Community Action, United Methodist Community House and The Rapid. These agency’s vehicles provide trips for RideLink.
You must pre-register to use RideLink.
Rides are scheduled through RideLink. Call 774-1288 no later than noon of the workday before your appointment.
• RideLink passengers are encouraged to make a $2.00
donation per trip. Donations are an important part of this program. The money goes directly back into the program to allow for continued service.
• Service hours are Monday – Friday, 7:30 a.m.– 4:30 p.m. and Sunday, 8:00 a.m.– 2:00 p.m.
• Rides must be cancelled through RideLink.
• This service is available Sunday through Friday

SENIOR IN THE SPOTLIGHT TERESA POTTS
Teresa has been volunteering at the Lowell
Senior Center for about three years. She enjoys coming
regularly and averages two to three times per week. She
helps serve coffee and lunch as well as cleaning up after lunch. She has even started her own jeopardy game each month with prizes! She recently joined the Advisory Council as well and serves as our council secretary! Teresa is available and always willing to lend a hand. Her help is
so appreciated at our Lowell Center.
When asked why she volunteers, Teresa said, “To keep busy and because I
enjoy it here at the center.” I also asked Teresa what she
enjoys about volunteering and she said, “I like helping people and I also like
meeting new people.”
We really appreciate Teresa helping out each week, just as we
appreciate all of the volunteers who help.
When Teresa is not volunteering at our Lowell Center she enjoys sewing and
reading!
We thank Teresa for volunteering. Her friendliness and care for others adds to the
fun we have each day at our Lowell Senior
Center.

is Delores Dykstra
Delores has been
a volunteer at Walker Firehouse Cafe for about 9 years. She has been attending
almost since we opened about 11 years ago.
Delores was born in Grand Rapids, but has lived in Muskegon, Mexico and
Texas. Delores has been back to Grand Rapids for about 70 years.
She really enjoys living on the West Side.
In her spare time Delores enjoys cards, crafts and taking trips.
Most of all she loves people.
