Introduction to the Report

The following report contains a site profile and summary of non-conformities identified during an onsite assessment commissioned by theAlliance for Bangladesh Worker Safety (Alliance) and conducted by a third-party Qualified Assessment Firm (QAF). The assessment wasconducted against the Alliance for Bangladesh Worker Safety Assessment Protocols (APs) and Fire Safety and Structural Integrity Standard,which is harmonized with the factory assessment guidelines developed by Bangladesh University of Engineering and Technology (BUET) forthe Bangladesh National Tripartite Plan of Action (NTPA). The goal of the Alliance process is to provide clear and practical technicalrequirements by which Bangladeshi Ready Made Garment (RMG) Factories producing for Alliance members may be consistently and fairlyevaluated for fire, structural, and electrical safety in a non-duplicative manner. Each assessment will prompt action plans that will be used byRMG factories to systematically and sustainably improve safety conditions for garment workers. Beyond tracking and reporting on actionsteps taken in a transparent manner, the Alliance organization and its members will seek to further support factory improvements throughtechnical assistance, training, implementation support for functional Worker Committees, and in some cases financial assistance and wagesupport for workers if factories are closed for remediation.

The contents of the report do not constitute a guarantee of compliance with the applicable laws, the Alliance Standard or the absolute orcontinued safety against fire, electrical and/or structural integrity issues that may lead to injury or loss of life. Fire Protection Construction

Question: Are openings and penetrations through rated walls and/or assembliesprotected?

Description: 01. Exits are not fully protected by continuously fire rated assemblies in allfloors. 02. Glass window in the separation wall of void at 2nd & 3rd floor 03.Penetration through the separation wall of sub-store at 3rd floor 04. Noseparation wall between the cutting section and stair of North-East corner at2nd floor. 05. Also non rated glass opening was found between cutting sectionand storage area at 2nd floor.

The fire doors shallbe side-hinged doors that swing in the direction of egress with automaticcloser, latching fire exit hardware, and no locking arrangements with aminimum width of 1 m (39 in.). Every door in a stair enclosure serving morethan 4 stories will need to be provided with re-entry provision. Consult aqualified fire protection engineer to design the required rated constructionbarriers with opening protection.

Description: A vertical void was found in this factory building connecting all floors. This voidwas not provided with proper fire barrier separation and opening protection.

Question: Is the fire resistance materials of structural members in good condition andfree of damage?

Description: R.C.C frame building (Ground to 5th floor). From visual inspection all structuralmembers were not found free of damage during assessment. Vertical crackswere noticed in some of the columns and concrete spalling from roof slab insome place. Also the tin shed room on roof deck was non-rated.

As per Alliance standard part 3 section 3.6.2.4: Non-rated construction shallnot be allowed for high-rise building. As the construction on roof deck (Brickwall with tin shed) is non-rated construction, it needs to be removed. Also adetailed structural assessment and retrofit of the structure per the instruction ofa qualifying structural engineer with full set of documentation needs to beprovided.

Standard: BNBC Part 3 Chapter 3

Question: Are separations between hazards provided with fire-resistive ratedconstruction barriers.



Description: No fire separation has provided between sub-store, finished goods store,fabric store, padding store, maintenance room, boiler room with otherproduction area (finishing section, swing section and daycare etc.) at 3rd and2nd floor. The generator has been placed in the open space near the egresspath discharge for the middle stair at the ground floor. This was not incompliance with Alliance Standard Part: 3 Section: 3.4 Mix Use.

Rooms used for storage of combustible materials, boiler, permanent hot workarea (maintenance room with metal cutting, grinding machine) needs to beseparated from the surrounding occupancy with a minimum 1-hour fire ratedconstruction with 0.75-hour fire rated opening protection. Generator needs tobe separated by 2-hour fire rated construction with 1.5-hour fire rated openingprotection. Consult a qualified fire protection engineer to design the requiredrated construction barrier.

Description: The standpipes are not in compliance with the Alliance Standard. Only 25 mmhose connections were found in the 3rd floor and only a hose box withouthoses was found on the 2nd floor. No Class-I (65mm) or Class-II (40mm)standpipes were found in the factory building.

Inmost of the cases stand pipes have been disconnected with the fire hose and25mm standpipes have been connected with the 40mm fabric hose bysubstandard fittings.

Connectionsneed to match the Fire Service and Civil Defence hose thread standard.Ensure reservation of the required amount of water for fire-fighting. Question: Exit access corridors serving an occupant load exceeding 30 are separated bywalls having a fire-resistance rating of 1 hr.



Description: Exit access corridor serving an occupant load more than 30 was found at 2nd& 3rd floor of east middle portion of this 6-story building. These corridors werenot separated by proper fire rated barrier as per Alliance standard part 6section 6.3.1.1.

Question: Doors are not locked in the direction of egress under any conditions. All hasps,locks, slide bolts, and other locking devices have been removed whererequired.



Description: Not in compliance with Alliance standard part 6 section 6.8.2. Doors in thepath of egress were found unlocked in the direction of egress only duringworking hours. Hasps, locks, other locking devices were present on all egressdoors.

Question: Exit discharge is directly to the exterior of the building, unless the requirementsof 6.17.2 are met, at grade or provides direct access to grade. Exit dischargeshall not reenter a building.

Question: Handrails are provided on both sides of each stairway. Intermediate handrailsare provided when the stair width exceeds 2.2 m (87 in.). Handrails are notmounted lower than 760 mm (30 in.) or higher than 1100 mm (44 in.).



Description: Not in compliance with Alliance Standard Part 6 Section 6.9 Stairs and 6.12Handrails and Guards. Handrails have been provided on only one side of eachstairway having minimum height 711 mm (28 in.) and maximum height 889mm (35 in.); no handrails have been provided in a few levels of stair-01 (north-east corner). Also the spacing between the vertical members of some stairswas 900 mm (36 inch.)

Description: The egress path between office and finishing section near north-east cornerstair has been separated by particle or artificial wooden board at 2nd floor.This construction material was also noted at the egress path of South-Weststair at 3rd floor. These were not in compliance with Alliance Standard Part 6Section 6.3.2 Interior Finish.

Description: Need to test the interior finishes materials for means of egress as per ASTME84. If does not meet the rating requirements for Class A, B and C materialsthen replace non-compliant finishes with finishes that have a maximum flamespread index of 75 and smoke developed index of 450 .

Source of Findings: Visual Assessment: No emergency light was found during inspection.


Install appropriate means of illumination at the noted locations. Emergencyillumination shall provide not less than 10 lux for corridors, doors, and stairs,and 2.5 lux for aisles. After installing need to develop a testing andmaintenance program that ensures the operation of all exit signs andemergency lighting are verified at least once per year. If battery-operated signsare used, these lights shall be tested on a monthly basis. Functional testing ofbattery powered signs shall be provided for a minimum 90 min once per year.

Description: This factory is located on the 2nd floor of a 6-story building. The 3rd floor isoccupied by another garment factory, Uma Manufactures & Exporters Ltd. The4th & 5th floors are vacant. The ground and 1st floor are occupied by aresidential hotel, office and market place. The fire safety management forthese facilities are not the same, there is no full separation between thevarious tenants of this building.
