Within NHS Tayside and based on the western edge of Dundee, Ninewells Hospital

1 JOB IDENTIFICATION Job Title Consultant Clinical Scientist (Clinical Biochemistry) and Clinical Lead

2 JOB PURPOSE

The level of responsibilities is as outlined in Department of Health, Advanced Letter AL(SP)1/90. The post holder will act as Clinical Lead, Clinical Biochemistry Modality, and Professional Head of Service.

3 ORGANISATIONAL POSITION

4 SCOPE AND RANGE

The post holder will assume all the duties and responsibilities for direction of a comprehensive Clinical Biochemistry service to NHS Tayside, with some additional national services. The service provides in excess of 8 million test results per annum, arising from some 750,00 requests. The service is provided from two sites, and includes the Scottish Bowel Screening Laboratory. Duties and responsibilities will be commensurate with those outlined in Standard B1 of the CPA UK Ltd Publication Standards for Medical Laboratories: - “The responsibilities of the laboratory director or designee(s), shall include clinical, scientific, professional, consultative, advisory, organisational, administrative and educational activities relevant to the services provided”

5 MAIN DUTIES AND RESPONSIBILITIES

1 As Clinical Lead, post holder will demonstrate professional excellence and the ability to deliver effective care, bearing responsibility for the delivery of a modern, accurate and efficient Clinical Biochemistry service, including the following: -

Develop and expand the service in line with (and contribute to) local, regional and national policy for diagnostic services

Act as the representative of the speciality, and develop professional relationships across the organisation and beyond Tayside, throughout Scotland and the UK, contributing to national policy making through professional bodies

Participate in and foster regional and national shared services initiatives.

Chair departmental meetings and prepare formal minutes.

Write the annual quality report and disseminating to users through the intranet and for circulation to all consultant level medical staff and all general practitioners.

Lead clinical governance reporting for the service, and participate in the Diagnostics Improvement Group. 2 The post holder will

Lead the medical and clinical biochemist staff in the performance and development of the Duty Team role.

Arrange complex Duty Team, weekend and out of hours rosters, taking leave and desires into account.

Chair meetings of the Duty Team and prepare formal minutes of these and other similar meetings.

Lead and participate in training and monitoring of new staff in the workplace.

Ensure new staff members have initial training and ongoing support for weekends/evenings following departmental policy.

3 The post holder will

Participate in the Duty Biochemist roster, providing advice on the selection, performance and interpretation of clinically relevant tests.

Independently provide expert clinical guidance and scientific advice to clinicians including senior medical staff, in both primary and secondary care, in order to facilitate the diagnosis, treatment, monitoring and understanding of disease, and to provide senior expert opinion, in highly complex situations, where expert opinion may differ.

Actively contribute to the provision of an associated consultant-level advisory and interpretive service on a 24/7 basis in a 1 in 4 roster, including ward rounds as required, providing advice on investigation and treatment in complex cases across all areas of own responsibility.

Deal with requests for samples from police/CID/forensic pathologists. 4 The post holder will

Work with other Clinical Leads in Blood Sciences,

Take an active role in ongoing management of the Managed Services Contract, and provide leadership and expert opinion in procurement of a new Managed Service Contract for Blood Sciences and other departments within Diagnostic Services

In association with the Quality Manager, take a lead role in quality management in the speciality.

Lead and encourage service design and redevelopment.

Develop long and medium term regional strategies for the speciality and be responsible for their delivery.

Oversee and approve selection of equipment and analytical methods.

Chair appropriate management meetings within the service.

Participate in the monitoring of the department budget, which includes Clinical Biochemistry, Haematology and Immunology services.

5 The post holder will

Lead long and medium term planning and strategy to ensure quality management system moves forward.

Lead in the maintenance of the UKAS ISO 15189 based quality management system of the service.

Lead the completion of forms for UKAS, correspond with UKAS and lead the preparation of annual reports.

Inspect all returns from external quality assessment schemes for all three laboratories.

Keep documents up to date, including appraising and updating SOPs, where relevant. Initiate and stimulate quality improvements.

Be familiar with service IT systems in order to develop and create statistical reports requiring formulae 6 The post holder will

Co-ordinate and implement programmes in the service, for independent or joint research and development on relevant scientific and clinical problems.

Initiate and secure funding, and direct research.

Disseminate knowledge gained through presentation at local, national and international meetings and publication in peer reviewed media.

Be responsible for granting approval for clinical researchers and medical students to access results in laboratory information system for research and development projects.

Be responsible for the supervision of postgraduate projects and dissertations required by professional bodies for postgraduate trainees.

7 The post holder will

Co-ordinate Tayside participation in activities of ACB Scottish and national audit groups.

Co-ordinate responses to UKNEQAS for Interpretative Comments.

Circulate results to all medical, clinical biochemist and BMS staff.

Initiate and participate in clinical audit pertinent to Clinical Biochemistry and other Blood Sciences specialties as appropriate.

8 The post holder will

Provide relevant teaching and training to medical, nursing and midwifery and other undergraduates.

Participate in the training of BMS, Clinical Biochemists and medical staff in training.

Participate regularly in the Departmental arrangements for CPD, giving presentations on a regular basis, and

Supervise the postgraduate projects of Clinical Biochemist Trainees. 9 The post holder will

Be line manager of all clinical staff.

Be responsible for allocation of their duties and supervision, appointments, job descriptions, appraisals, delegation of tasks, discipline, promotion, encouragement, supervision of work projects, dealing with all types of leave.

Supervise the Grade A/B Trainee Clinical Biochemists and Clinical Biochemists in Higher Specialist Training.

10 The post holder will

Be responsible for the management of the Scottish Grade A/B Clinical Biochemist Training Scheme (10 – 12) posts across NHS Scotland, including: -

co-ordination of Trainees’ activities and Supervisors; participation in medium and long term training policies and strategies with National Health

Services Education for Scotland(NES); responsibility for the budget; responsibility for appointments, advertising, liaison with ACB Regional Tutor and University of

Surrey; preparation of reports for NES.

11 The post holder will

Regularly review SIGN, NICE, ACB and other national and international guidelines and recommendations on best practice and assess both whether the guidelines can be followed and whether they should be implemented in Tayside.

Liaise with relevant senior clinical staff regarding strategies for adoption of new or modified practice.

Develop strategic plans for the introduction of new procedures and ways of working to ensure best practice is instituted and maintained.

Prepare written documentation on new tests and procedures, as well as bulletins for all clinical staff in NHS Tayside.

Actively promote and develop Demand Optimisation strategies for the service 12 The post holder will participate in clinical liaison by

Organising meetings with individual and groups of users when required.

Developing clinical guidelines for testing strategies and the development of clinical protocols within the Department.

Leading the preparation of the Laboratory User guide and other material for users.

Encouraging preparation of Biochemical Medicine Bulletins and organise their circulation.

Dealing with problems and complaints raised by users.

Dealing with confrontational situations between users of the laboratory and laboratory staff members.

Determining the best combination of tests included in request profiles, the method of test requesting, sample handling and result reporting.

Leading efforts in workload management and demand optimisation

General Duties: -

1. The post holder will liaise with staff from other Departments concerning the provision of a comprehensive service.

2. The post holder will lead/participate in appointment and selection of staff and Trainees.

3. The post holder will participate in appropriate meetings in Blood Sciences, Diagnostics Group and other disciplines. The post holder will participate at Health Board and national level.

4. The post holder must possess HCPC registration and must show evidence of participating in continuing education (e.g., the CPD scheme of the Royal College of Pathologists).

5. The post holder will participate in professional affairs.

6. The post holder is required to take reasonable care to avoid injury or accident, and must undertake NHS

Tayside mandatory training. Duties must be performed in accordance with Departmental Codes of Practice and comply with normal regulations.

7. The duties and responsibilities of the post will be reviewed as part of annual joint staff review. The post holder will also undertake annual appraisal of other staff.

8. This job description is an outline of the current position and may be amended in detail or emphasis in the light of the future requirements of the service. All amendments and changes to the job description MUST be agreed with the post holder.

9. The post holder will undertake other duties commensurate with the grade.

6 COMMUNICATIONS AND RELATIONSHIPS

Provide and receive highly complex and contentious information; highly developed tact and persuasive skills required; bridge barriers to understanding; present complex and contentious information to large groups; communicate highly complex, sensitive and contentious information in an antagonistic atmosphere. The post holder will need to interact at all levels of organisation in NHS Tayside both as a professional in Clinical Biochemistry in delivery of a clinical service, as a scientist/clinician and manager. The post holder will need to communicate with healthcare professionals, other professionals, students, trainees, with patients and their relatives/carers, and lay individuals to enable understanding on a range of complex scientific, clinical and management issues. The post holder will undertake relevant presentations/representations at a local and national level within the NHS organisation and at local, national and international level to relevant professional bodies/professionally organised events. The post holder will participate as a member of the Diagnostics Group and local Clinical Leads meetings, and other relevant NHS Tayside committees as required by the Clinical Group Director or Medical Director.

7 KNOWLEDGE, TRAINING AND EXPERIENCE REQUIRED TO DO THE JOB

The post holder will have undertaken training as Clinical Scientist and registered with the Health Care Professions Council in the modality of Clinical Biochemistry. They will hold a post graduate qualification at Doctorate level (or equivalent) in Clinical Biochemistry, would be expected to have extensive experience at consultant clinical scientist level, and have competence at the level of the Fellowship of the Royal College of Pathologists or equivalent. They will therefore have extensive knowledge and expertise in Clinical Biochemistry, and relevant management experience in delivery of Clinical Biochemistry services. The post holder will be competent in the use and application of IT systems within the department, and have knowledge of management theory, including personnel management, risk management and health and safety. The post holder will under be able to demonstrate participation in continuing professional development.

8 SYSTEMS/EQUIPMENT/FINANCE

As Clinical Lead the post holder will have responsibility for the Clinical Biochemical modality of Blood Sciences. This is a highly automated and computerised service based on two sites, with an annual budget in excess of £4 million per annum. Changes within NHS Tayside management structures envisage that this post will be the budget holder.

Responsibility for Records Management All records created in the course of the business of NHS Tayside are corporate records and are public records under the terms of the Public Records (Scotland) Act 1937. This includes email messages and other electronic records. It is your responsibility to ensure that you keep appropriate records of your work in NHS Tayside and manage those records in keeping with the NHS Tayside Records Management Policy and with any guidance produced by NHS Tayside specific to your employment.

9 PHYSICAL DEMANDS OF THE JOB

Prolonged periods of working in a restricted position (at a VDU or complex analytical machinery) with loss of sleep when on call; advanced keyboard skills for working with IT systems extant in Blood Sciences. EMOTIONAL DEMANDS OF THE JOB Frequent (weekly) exposure to distressing or emotional circumstances: this may include dealing with complaints, difficult staffing issues, and managing change. Dealing with failure of complex systems may have immediate clinical impacts on the management of large populations of patients; such situations may involve dealing with many groups of individuals under high pressure, demanding immediate answers and reassurances, and require a need for difficult decisions with wide reaching impacts.

Conditions of Service

The terms and conditions of service for this post are those determined by the NHS Staff Council.

Remuneration The current salary scale for the post is £68,599 to £84,932 per annum (pro rata for part-time staff). Placing on the scale on appointment is normally at the minimum but may be higher subject to verification of previous relevant service. Salary is paid monthly by Bank Credit Transfer.

Hours of Duty The hours of the post are 37.5 per week. The contracted weekly hours may be worked over any reference period, e.g. 150 hours (pro rata for part time staff) over four weeks or annualised hours, with due regard for compliance with employment legislation, such

as the Working Time Regulations. Start and finish times will be determined by the needs of the service.

Superannuation New employees are automatically enrolled into the NHS (Scotland) Superannuation Scheme, but may opt out later should they choose. The pension scheme is provided by Scottish Public Pensions Agency. All benefits are explained on the SPPA website www.sppa.gov.uk Employee contribution rates with effect from 1 April 2018 are as follows:

Once this decision has been made, all shortlisted applicants will be emailed to invite them to attend for interview. It is important that you check your emails regularly, including your junk/spam folder. This process takes approximately 6 weeks. Therefore, if you have not been contacted regarding interview within 6 weeks of the closing date, your application has been unsuccessful and you will receive no further correspondence from HR & OD Directorate.
