Programme Aims

Islington CCG and London Borough of Islington (LBI) Council are working together to deliver: • Interoperability and information

exchange between GP’s, providers and citizens/population

• Having a digital person held health and social care record (PHR) for the people of Islington

What will the Person Held Record enable

• Providing facilities for patients to manage long-term conditions such as being able to record their own results and comments into the record, virtual appointments and use of apps as an example.

• Plug individuals health information into apps and tools that help them set and reach personalised health goals.

• Ability to share their record with their doctor and trusted family members or caregivers.

• Patients have the ability to check to make sure the information, such as medication list, is accurate and complete.

• Be able to make contact and interact with health and care providers virtually and support new ways of working.

• Provide information for patients on helping them to manage their own care.

Person Held Record Requirements

• Enhancing the partnership between patients and their clinicians.• Enable peoples’ medical history to be available in case of emergency,

when travelling, seeking a second opinion, or switching health providers.• An option for patients to record difficulties they have accessing care e.g.

some patients find it difficult to get through to their GP by phone.• Be able to make contact and interact with health and care providers

virtually and support new ways of working.• Access test results approved by health professionals.• Use of specific patient condition questionnaires.• Sharing care package information between providers of care.• Portability of care assessment plans.

Benefits• PHR would allow citizens to go online (Web portal or app) and

access or download their health records• Edit and upload other information so they can use IT to improve

their health, • Have more control over their personal health information and

their family’s healthcare. • Facilitate empowerment of patients/citizens.• Patients/citizens at the centre of their care• Patient/citizen friendly to use.• Patients/citizens have control of their records.• Ongoing development of the software by users.• Patients/people have governance to choose who has access to

their records and input to their records such as carers and parents

Where we are now

• Closing date for bid submissions was the 29th April. • OJEU opened on the 27th February - 63 companies registered an

interest• 8 Suppliers submitted a response to tender• Maximum of 6 to go forward to presentation stage• Evaluation now taking place

• 25 Stakeholders, 2 of which are patient reps• Clarification questions sent to suppliers

• financial and delivery responses due 26th May• Moderation Meeting 4th June• Supplier presentations 11th and 12th June• Award recommendation and report to July Governing Body

meeting• Contract expected to start August 2015• Draft project plan available – Full project plan once supplier

appointed
