Activity of daily living (Adl) monitoring

Do you worry if your loved one is ok living at home?

Motion SensorsMotion sensors are useful for assuring that your loved one is OK and moving around as you would expect. Checking in to see that your loved one is getting up at the usual time, and moving around in a normal, predictable way, can provide peace of mind. Changes in

routine can be identified by the system which can be set to alert caregivers by phone, text, or email.

Door SensorsYou can place sensors on doors, windows, cabinets, or refrigerators to detect when they have been opened or closed. The sensors can help

you see if a caregiver or service provider has arrived when they were expected, or if someone is leaving at an unexpected time, such as the middle of the night. You can make sure a pill cabinet has been opened at the proper time, or whether a pantry door or refrigerator has been opened at meal times.

Appliance SensorsAppliance sensors can turn off and on small appliances, e.g. an electric kettle or iron, and be used for a variety of applications.

Button and Wearable SensorsSometimes referred to as “Call Me” buttons, these devices can let you know when your loved one needs help, wants you to call, or that a caregiver performed an activity. These sensors

can be placed in a convenient spot like next to the bed, hooked on a belt or strap, kept in a pocket, or worn like a necklace. Rules can be set to send out alerts when the button is pushed. The alerts are fully customizable and can be sent by email, text, or phone to single or multiple recipients.

Bed, Chair & Mat SensorsBed sensors help you know whether your loved one is going to bed at the usual time and is staying in bed during the night. Activity graphs help you

determine whether your loved one is sleeping calmly or restlessly. You can set alerts to let you know if something is not right, for example if your loved one has left and not returned to bed. You can also notice whether your loved one is staying in bed excessively, or returning to bed during the day. These sensors can be used in other locations such as on chairs, on sofas, or under mats.

Discreet activity sensors and action buttons report wirelessly to the System located in the residence. Through the Care Menu, family, friends and caregivers can log in to assess activity levels, pin-point specific problems, and revise notification rules.

Tele-health Sensors

Is your loved one taking their medications?

medication managementSmall wireless sensors can determine whether a medication drawer, cabinet or container has been opened at the correct designated time.

The System can also prompt a resident with medication instructions including a picture of the medication to be taken.

If medications are not accessed, the System can send an audio/visual message to the GrandCare touchscreen, call the loved one with a reminder or alert an authorized caregiver.

Wireless digital health devices such as weight scales, pulse oximeters, glucometers and blood pressure monitors can automatically record and store readings on the System. Authorized family and health care providers can log into the Care Menu to view trending health data or receive email, phone or text alerts. E.g. A daughter can be notified if a loved one failed to take his or her blood pressure reading.

Smart home devicesTemperature SensorsIf you are concerned that your loved one’s home might get too hot or too cold, our wireless temperature sensors can help you. The sensors report the current indoor temperature, as well as graphs of the temperature over time. You can set limits for how high or low the temperature can be, so you and other caregivers can receive alerts if those limits are exceeded.

Caller ID SensorsThe system’s Caller ID feature makes your loved one’s Caller ID even more powerful, through phone logs, pictures, descriptions, and alerts. Each time a telephone call comes to your loved one’s home, a screen will display on the system that shows the name and phone number

of the person, as well as a picture and description (e.g. “Your Son” or “Your Mechanic”), which can be useful cognitive assists. If the person who calls is not on the Known Callers list, the screen will say “Unknown Caller.” The System can automatically email this information to the caregiver responsible for the loved one’s safety.

Lighting ControlYou can improve safety by setting lights to respond to activity within the home. For example, you can:

n Turn on the hallway light automatically if your loved one gets up in the middle of the night

n Turn on a stairwell light automatically when the bedroom door opens during the night

n Turn a bathroom nightlight on automatically at dusk, and off automatically at dawn

n Turn a lamp on or off at scheduled times

Wouldn’t you like to know if your loved one is feeling lonely or isolated?

WHY it matters?Cognition Set up appointment reminders via the Calendar, Events, and/or Message functions. In addition, improve and maintain cognitive health with brain exercises.

SocializationLoved ones can access news, weather, and spirituals while family and friends from near or far can send personal photos, videos, messages with optional voice recordings, and emails.

The System provides one-button video chat (utilizing Skype), and can connect with Facebook to automatically download photos.

EntertainmentThe entertainment features in the GrandCare Bermuda System includes: favourite websites, word definitions, videos, music, trivia, multimedia slide show, and games like Solitaire, Checkers, Mastermind, and Naught & Crosses (X’s and O’s).

Users of the GrandCare Bermuda System can designate rules as to who, when, and how persons can be alerted about unusual activities.

Alerts and notifications can be sent in many forms — text, email, and phone call (including escalation call out lists), and on-screen to the System in the loved one’s home. Other options include flashing a lamp or set of lights.

Sample rules:Weather Example (insufficient heat)

n If the bedroom temperature goes below 74 degrees, show the “do you have your sweater on” message to GrandCare system screen for 20 minutes and text son.

Alert Pendant Example (call me)n When neck pendant is pressed, send the

“I am lonely” email message to care centre, son Mark and daughter Karen.

Telephone Example (unknown caller)n When a known caller is received, turn light

on and show the “answer phone” message on GrandCare system screen. When caller is not in contact list then, send email to caregiver, and send email to son.

The System facilitates peace of mind through accountability notifications, virtual visits, non-erasable care notes, time stamps for caregiver activities, separate and varying access privileges per family member or caregiver, punch clocks, live show replays, varied reports, and the ability to customize the system to match different levels of involvement.

Medication Sensor Example (medication not taken successfully)

n If on-screen button is not pressed, call caregiver’s cell phone and email care centre.

Bed Sensor Example (awake during night)n When Mary is out of bed between 10pm

and 5am, turn bed lamp on, email care centre and call son Mark (to wake him up), send message to Mary’s GrandCare system screen, “Mary, if you are ok, press your button” (button pressed will text “Mary is ok” message to Mark and email care centre).

Wellness Example (blood pressure)n When blood pressure value is recorded

send email to doctor’s office and sister Cynthia.

If additional care management is required GrandCare Bermuda’s Care Managers can provide:

n Access to all needed resources (community, medical equipment, home adaptation, etc.)

n Coordination with all involved in the care (physicians, attorneys, therapists, etc.)

n Escort of loved one to medical appointments and provide follow-up communication

n Medication oversight (set up, organize, coordinate, refill, etc.)
