Summary
SUMMARY
Nature has designed peptides for a large variety of specific functions. They
regulate many functions by acting as chemical messengers, neurotransmitters and
highly active stimulators or inhibitors. They bind to their specific receptors, which
mostly belong to the superfamily of G-protein coupled receptors. Peptides can be
considered as ideal agents as carrier for diagnostic and therapeutic applications,
adopting Paul Ehrlichs concept of the “magic bullet”. Tumor tissues expressing
receptors for a peptide can be addressed by the peptide or differently labelled
analogues. Depending on the label, the tumor can be visualized or therapeutically
treated. The specific binding to receptors can be used to meet the key requirement
in tumor targeting: selective addressing of neoplasm. Peptides are readily
synthesized and can withstand harsh conditions for modification and labelling.
Because of their small size, peptides exhibit fast blood clearance and high target-
to-background ratios compared to macromulecular carriers such as antibodies.
Peptides are less likely to induce immunogenic response, and tissue penetration
and tumor uptake are faster.
In radiopharmacy, these advantages have been attended, and radiolabelled
receptor-binding peptides have emerged as a new class of radiopharmaceuticals.
Over the last years, nuclear medicine has evaluated various peptides for tumor
scintigraphy, e.g. somatostatin, vasoactive intestinal peptide, bombesin,
neurotensin, substance P, cholecystokinin and many more. In chapter I, the latest
achievements in peptide-guided tumor diagnosis and treatment are summarized in
a review, and the prerequisites, advantages and specific pharmacokinetic
properties of peptides as carriers are discussed critically.
Chemotherapy has been the main therapy for treatment of cancer. One of the
major problems in classic chemotherapy is the non-specific toxicity of most
anticancer agents against normal cells. Efficient drug targeting could be of great
benefit for patients, reducing the severe, dose-related side effects. In order to
increase their therapeutic index, drug delivery systems are needed, which
selectively transport the antineoplastic agents to the tumor. In chapter 2 it is shown
that coupling of cytotoxic drugs to peptides is a promising approach to obtain

selectivity. The application of neuropeptide Y (NPY) as a carrier peptide has been
tested for the first time. NPY is a 36-amino acid peptide of the pancreatic
polypeptide family. It is expressed in the peripheral and central nervous system,
and is one of the most abundant neuropeptides in the brain. Five distinct NPY
receptors have been cloned, which have been named Y,, Yz, Y,I., Yg and y6
receptor subtype. These receptors are produced in a number of neuroblastoma
and the thereof derived cell lines, making them feasible targets for tumor targeting.
Daunorubicin and doxorubicin, two widely used antineoplastic agents in tumor
therapy, were covalently linked to NPY via two spacers that differ in stability. The
different conjugates were able to bind to the Y, receptor with high affinities, but
only the compound containing an acid-sensitive linker showed cytotoxic activity. As
the cytotoxicity was confined to cells expressing the Y1 receptor subtype,
selectivity was achieved by this peptide carrier approach.
For therapeutic applications, internalization of the peptide conjugate is an
absolute precondition, as the cytotoxic moiety has to reach its target within the
cell. Therefore, internalization studies on SK-N-MC cells were performed using
fluorescent and tritium labelled NPY (chapter 3). It was shown that approximately
20”30% of NPY is taken up by the cells by receptor-mediated endocytosis,
rendering NPY a suitable carrier for specific tumor targeting.
Subsequently, the use of NPY as a radiopeptide for scintigraphy was
investigated (chapter 4). A novel ggmTc(l) aqua complex and 2-picolyamine-/V,/V-
diacetic acid (PADA) as bifunctional chelating agent (BFCA) were tested.
[K4(PADA),A26]-NPY was specifically postlabelled, while maintaining high binding
affinity. Furthermore, the prelabelling approach has been applied for the shortened
analog [Ahx5*24]-NPY. The resulting Ac-[Ahx5~24,K4(ggmTc(C0)3-PADA)]-NPY was
produced with a yield of about 16%. Therefore, postlabelling was applied for the
short analog as well. Competitive binding assays using ‘85Re as a surrogate for
ggmTc showed high binding affinity of Ac-[Ahx5~24,K4(‘8”Re(C0)3-PADA,A26)]-NPY.
Internalization studies with the corresponding ggmTc labelled analog revealed
receptor-mediated internalization. gg”Tc-labelled NPY derivatives seem to have
promising characteristics for future applications in nuclear medicine.

The same radionuclide and BFCA have been exploited for the labelling of a
bombesin analog (chapter 5). The use of this peptide as radiopharmaceutical has
been suggested, as a variety of tumor tissues have been found to express
bombesin receptors, for instance lung, prostate, breast, pancreas and colon. The
spacer Ava (5”aminovalerianic acid) was introduced between the BFCA and the
peptide in order to avoid interference of the chelator moiety with the receptor-
binding C-terminus of the peptide. [ggmTc(l)-PADA-Ava]-bombesin(7-l 4) was
characterized extensively and revealed good properties for a scintigraphic
radiopeptide (chapter 6).
Most peptides and hormonal messengers are recognized on the cell surface by
their G-protein coupled receptors. Subsequent signal transduction via different
cascades triggers for instance cell growth and differentiation. Exact coordination
between the components of the signalling pathways is crucial for normal cell
growth. Any disturbance may cause uncontrolled proliferation resulting in the
establishment of cancer. Thus, the MAP-Kinase cascade and the inhibition of
forskolin-stimulated adenylate cyclase were investigated by using lipopeptides
derived from Ras and the C-terminal sequence of the Y1 receptor (chapter 7). Both
signal transduction pathways were affected by the synthetic short lipopeptides. As
structural requirements for the peptides sequence specificity for the investigated
receptor and lipidation were identified.
In conclusion, this work provides new insights and developments in the field of
specific tumor targeting. New “g’nTc-labelled peptides in nuclear medicine and
novel peptide conjugates for selective chemotherapy may augment the
armentarium in the dreadful fight against cancer.

Die Natur konzipierte Peptide fur eine Vielzahl spezifischer Funktionen. Sie
regulieren zahlreiche Funktionen, indem sie als chemische Botenstoffe,
Neurotransmitter und hochaktive Stimulatoren oder Inhibitoren agieren. Sie binden
an ihre spezifischen Rezeptoren, welche meistens zur Superfamilie der G-Protein
gekoppelten Rezeptoren gehbren. In Anlehnung an Paul Ehrlichs Konzept der
Wunderwaffe (magic bullet) konnen Peptide als ideale Trtiger fur diagnostische
und therapeutische Anwendungen angesehen werden. Tumorgewebe, welches
die entsprechenden Rezeptoren fur das Peptid exprimiert, kann durch das Peptid
oder seine verschieden markierten Analoga adressiert werden. Je nach
Markierung kann anschliessend der Tumor sichtbar gemacht oder behandelt
werden. Das spezifische Binden an die Rezeptoren kann angewendet werden, urn
die Hauptanforderung des Tumor Targetings zu erfullen: das selektive Ansteuern
der Krebszellen. Peptide sind einfach zu synthetisieren und halten harte
Modifikationsprozeduren aus. Da sie so klein sind, zeigen sie im Vergleich zu
makromolekularen Tragern wie Antikorper ein schnelleres Verschwinden aus der
Blutbahn und ein hoheres Ziel-Hintergrund-Verhaltnis. Zudem induzieren Peptide
weniger Immmunantworten, und sie dringen schneller in Gewebe ein, weshalb
eine hohere Tumoraufnahme gewahrleistet werden kann.
All diese Vorteile wurden in der Radiopharmazie aufgegriffen, und
radiomarkierte, rezeptorbindende Peptide haben sich zu einer neuen und
eigenstsndigen Klasse der Radiopharmazeutika entwickelt. Die Nuklearmedizin
hat in den letzten Jahren zahlreiche Peptide zur Anwendung in der
Tumorszintigraphie getested, bespielsweise Somatostatin, Vasoaktives
lntestinales Peptid (VIP), Bombesin, Neurotensin, Substanz P, Cholezystokinin
und viele mehr. Im ersten Kapitel dieser Arbeit sind die neusten Erkenntnisse auf
dem Gebiet der peptidvermittelten Tumordiagnose und -therapie in einer
ubersicht zusammengefasst, und die Voraussetzungen, Vorteile und spezifischen
pharmakokinetischen Eigenheiten von Peptiden als Tragersubstanzen werden
erortert.
6 Zusammenfassung
Chemotherapie ist die meistverwendete Therapieform bei Krebs. Eines der
grdssten Probleme in der klasischen Chemotherapie ist die unspezifische Toxizitat
der meisten Zytostatika gegentiber normalen Zellen. Effizientes Tumor Targeting
konnte fur die Patienten sehr hilfreich sein, da die schweren, dosisabhgngigen
Nebenwirkungen vermindert wtirden. Urn den therapeutischen Index der
Medikamente zu erhohen, sind passende Arzneistoff-Transportsysteme
notwendig, welche das Zytostatikum selektiv zum Tumor bringen. Im zweiten
Kapitel wird gezeigt, dass das Koppeln von Zytostatika an Tragerpeptide ein
vielversprechender Ansatz in dieser Richtung ist. Die Anwendung von Neuropeptid
Y (NPY) als Carrier wird erstmalig vorgestellt. NPY ist ein 36 Aminosauren langes
Peptid, welches zur Familie der pankreatischen Polypeptide gehiirt. Es wird im
zentralen und peripheren Nervensystem exprimiert, und ist eines der haufigst
vokommenden Neuropeptide im Gehirn. Ftinf unterschiedliche Rezeptoren wurden
kloniert und Subtypen YI, Yp, Y4, Y5 und y6 genannt. lnteressanterweise werden
diese Rezeptoren such in verschiedenen Neuroblastoma und den davon
abgeleiteten Zellinien gefunden, weshalb sie mogliche Zielstrukturen fur Tumor
Targeting darstellen. Daunorubicin und Doxorubicin, zwei weitverbreitete
Zytostatika in der Chemotherapie, wurden kovalent an NPY gebunden, wobei man
zwei unterschiedlich stabile Spacer verwendete. Die verschiedenen Konjugate
konnten mit hoher Affinitat an den Y1 Rezeptor binden, jedoch nur die Verbindung
mit einer s&relabilen Spacergruppe zeigte zytotoxische AktivitX Da sich diese
Zytotoxizitat auf Zellen beschrankte, welche den Y1 Rezeptor exprimieren, wurde
Selektivitat durch diesen peptidvermittelten Ansatz erzielt.
lnternalisierung des Konjugates ist unbedingt notwendig fur therapeutische
Applikationen, denn der Arzneistoff muss sein Ziel innerhalb der Zelle erreichen.
Deshalb wurden lnternalisierungsstudien mit fluoreszenz- und tritiummarkiertem
NPY an SK-N-MC Zellen durchgefuhrt (Kapitel 3). Es konnte gezeigt werden, dass
ca. 20-30% des NPY durch rezeptorvermittelte Endozytose in die Zellen
aufgenommen wird, was NPY zu einem passenden Transportvehikel fur
tumorspezifisches Targeting macht.

Anschliessend wurde die Anwendung von NPY als Radiopeptid fur Szintigraphie
untersucht (Kapitel 4). Man testete einen neuen ggmTc(l)-Komplex und PADA (2-
picolyamine-N,N-diacetic acid) als bifunktionellen Chelator. [K4(PADA),A26]-NPY
wurde spezifisch postgelabelt, wahrend es hohe Bindungsaffinitat beibehielt.
Zudem wurde der Prelabeling Weg fur das vergurzte Analogon [Ahx5-24]-NPY
angewandt. Das resultierende Ac-[Ahx5-24 4 ggm ,K ( Tc(CO),-PADA)]-NPY erhielt man
mit einer Ausbeute von nur 16%, weshalb such hier ein Postlabeling versucht
wurde. Kompetitionsbindungsstudien, bei welchen 185Re als Erstaz fur das heisse
ggmT~ verwendet wurde, zeigten hohe Bindungsaffinitat van Ac-[Ahx5-24,
K4(‘85Re(C0)3-PADA,A26)]-NPY. lnternalisierungsstudien mit der entsprechenden
““Tc markierten Verbindung ergaben rezeptorvermittelte Internalisierung.
Deshalb konnten ggm Tc markierte NPY-Derivate vielverprechende Kandidaten fur
zukiinftige Anwendungen in der Nuklearmedizin darstellen.
Dasselbe Radionuklid und derselbe Chelator wurden fur die Markierung eines
Bombesinderivates benutzt (Kapitel 5). Die Verwendung dieses Peptides als
Radiopharmazeutikum war vorgeschlagen worden, da viele Tumorgewebe
Bombesinrezeptoren exprimieren, beispielsweise in der Lunge, Brust, Prostata,
Pankreas und Colon. Urn lnterferenzen zwischen dem Chelator und der
rezeptorbindenden, C-terminalen Sequenz des Peptides zu vermeiden, wurde der
Spacer Ava (5-aminovalerianic acid) eingeftihrt. [ggmTc(l)-PADA-Ava]-Bombesin(7-
14) wurde eingehend charakterisiert und zeigte gute Eigenschaften zur
Verwendung als Radiopeptid fur die Szintigraphie (Kapitel 6).
Die meisten Peptide und hormonalen Botenstoffe werden durch ihre G-Protein-
gekoppelten Rezeptoren an der Zelloberflache erkannt. Die anschliessende
Signaltransduktion via verschiedener Kaskaden lost zum Beispiel Zellwachstum
und -differenzierung aus. Exakte Koordination zwischen den einzelnen
Komponenten der Signaltransduktionswege ist essentiell fur eine normale
Zellentwicklung. Jedwelche Storung konnte eine unkontrollierte Proliferation
bewirken, was zu Krebs fijhren wit-de. Deshalb untersuchte man die MAP-Kinase
Kaskade und die Inhibition van Forskolin-stimulierter Adenylatzyklase mit
Lipopeptiden, welche von Ras respektive der C-terminalen Sequenz des Y,
Rezeptors abgeleitet waren (Kapitel 7). Beide Signaltransduktionswege wurden

durch die synthetischen Lipopeptide beeinflusst. Als strukturelle Anforderungen an
solche Peptide wurden Sequenzspezifittit und ein Lipidrest identifiziert.
Zusammenfassend kann gesagt werden, dass diese Arbeit neue Einblicke und
Entwicklungen im Gebiet des spezifischen Tumor Targetings liefert. Neue 9g’nT~
markierte Peptide in der Nuklearmedizin und neue Peptidkonjugate zur selektiven
Chemotherapie konnten effektive Waffen im Kampf gegen Krebs darstellen.
