Background

Individuals from the MA public health and dental provider communities joined forces with the Head Start community to address health disparities and barriers to care resulting in high rate of dental disease in MA HS children.

Efforts began in 2002 and included the MA HSSCO and HS Association. In 2005 the MA OH Collaborative conducted an open mouth survey of HS children and produced the MA OH report for the Association – 37% of children had experienced dental disease; 8% had urgent dental needs, and 38% had white spots or early decalcification that could be arrested with early treatment. One of the impt successes in MA were the relationships built among this group of individuals: MA OOH, MADPH, MDS, BU Goldman School of Dental Medicine, Tufts U. Community Outreach program, Dental Services of MA, CMOHS

Focus Groups with Family Service Staff Asked for a “toolkit” to use in talking with families

about the importance of routine oral health care and how to minimize tooth decay

In 2007 a survey of health managers -100% participation - identified two major challenges to compliance with the Dental PPS – lack of dentists accepting Medicaid and parents lack of understanding about the relationship between OH and children’s overall wellness and resulting failure to follow through with dental care, even when appts were made. High rates of parent cancellations/no-shows is also a reason stated by many dental providers for not wanting to see the Medicaid population as it is a financial hardship for them due to lost billable hours.

DentaQuest Foundation grant awarded to the HS Association in fall of 2007 on behalf of the MA Early Childhood OH Consortium, representing our original partners and new coalition members, provided the funds to produce the educational materials. This allowed us to hire Deidre Callanan as the MECOCH OH Coordinator.

Pilot was based on Cavity Free Kids, evidence-based curriculum produced by WA Dental Service Foundation, and a training for children with special health care needs developed by MA OOH, MDPH with input from the Consortium and extensive consultation with Dr. Douglass who reviewed to make sure all info was evidence-based (at least one study). Tension between OH advisors who wanted more and complex info and HS field asking for more visuals and appropriate literacy level for HS families.

Need a trusted group of expert advisors and one individual who will take the lead to bridge different opinions. Need to balance the tension between the OH messaging recommended by the dental provider community and the “Keep It Simple” approach recommended by the HS practitioners who emphasized the need for lots of visuals, reduced text, and materials at an appropriate literacy level. Recommend a designer. Incorporate materials to maintain validity. Multi-lingual materials are key. Everything takes longer than expected.
