•Learn from the experience of children, young people with SEND, their families and practitioners to continuously improve practice•Secure the organisation and cultural change needed to ensure that the reforms make a real impact on the life chances of children and young people with SEND.

communities •The reforms introduce new category of SEN, Social, emotional and mental health needs (SEMH)•Children, young people, families able to access information and advice through local offer and new SEND Information and Advice Service.

• We want children and young people with SEND to be happy and successful in their early years, at school and at college, to achieve the best that they can, find employment and go on to live happy and fulfilled adult lives.
• We want children and young people with SEND and their families to have choice and control over their support to instil confidence and trust across the system.
Shared Vision for a Better Future
• Children and young people with SEND are as likely to enter full-time education, employment or training when they leave school as their peers
• Young adults with SEND for whom it is appropriate can secure appropriate living arrangements
• Wherever possible young adults with SEND are able to be financially independent
• Children with SEND are no more likely to be excluded from school, or placed in alternative provision than other children
Measures of Success
• Children, young people, parent carers and education providers consistently express confidence and trust in how we meet SEND in Swindon
• Very few parents and carers need to seek formal mediation or recourse to SEND tribunal
• Children with SEND make good or outstanding progress
• The attainment gap for children with SEND is lower than in most other areas
• Children with SEND have good attendance
Measures of Success
• 4 Population Level Outcomes – describe the impact the reforms are designed to have on the life chances of children, young people with SEND and their families
1. Children and young people with SEND and their families trust and have confidence in the support they receive
2. Children and young people with SEND have great life chances3. Children and young people with SEND are happy and enjoy their
education and social life4. Young adults with SEND live happy and fulfilled lives
• Primary indicators for each outcome that:• Say something of real importance about well-being and future life
chances • Represent areas in which children and young people with SEN
consistently achieve disproportionately poor outcomes.
