• Corporate Business Plan - Outcome 10Children and vulnerable adults are cared for in Swindon because the reforms require us to:

•Learn from the experience of children, young people with SEND, their families and practitioners to continuously improve practice•Secure the organisation and cultural change needed to ensure that the reforms make a real impact on the life chances of children and young people with SEND.

• Corporate Business Plan - Outcome 10Children and vulnerable adults are cared for in Swindon because the reforms require us to:

life, introduction of EHCP will help to ensure that children and young people with SEND and their families are active and empowered.

Priority 3.• Improved health outcomes for disadvantaged and vulnerable

communities •The reforms introduce new category of SEN, Social, emotional and mental health needs (SEMH)•Children, young people, families able to access information and advice through local offer and new SEND Information and Advice Service.

• We want children and young people with SEND to be happy and successful in their early years, at school and at college, to achieve the best that they can, find employment and go on to live happy and fulfilled adult lives.

• We want children and young people with SEND and their families to have choice and control over their support to instil confidence and trust across the system.

• Children and young people with SEND are as likely to enter full-time education, employment or training when they leave school as their peers

• Wherever possible young adults with SEND are able to be financially independent

• Children with SEND are no more likely to be excluded from school, or placed in alternative provision than other children

• Children with SEND have good attendance
