It has recently come to our attention that the Queen Liliuokalani Trust Well 1, State Well No. 4057-01, is being operated by the Hawaii County Water Supply.
1\ Condition 11 of your Pump Installation Permit dated July 7, 1994, requires that an
as-built section ofy-our pump installation be submitted within 30 days of completion of work. As your pump installation permit expired on July 7, 1996, your deadline to submit the as-built was August 29,1996 (30 days later). Attached is a copy of your Pump Installation Permit for your reference.
The well owner is required to submit monthly water use reporting. We have to date not received any report forms for this well. We have enclosed a water use report form for your use.
Please clarify these issues to us in writing no later than March 15, 1997. We have enclosed a Well Completion Report for your use in lieu of preparing an as-built of the pump installation. You are reminded that pursuant to HRS 174C-15, any violation of your permit may be subject to fines of up to $1,000 per day per violation.

Transmitted herewith please find the laboratory analysis report for toxaphene and total coliform to complete the water quality analysis for the well. The sample was obtained during the initial well pump start-up. The results indicated that toxaphene was not detectable and the total coliform was at negligible levels.
Please advise us whether the well will be acceptable as a new source of potable water. The construction of the well is scheduled to be completed in a week. The Department of Water Supply has requested that the well be placed on-line as soon as possible, as other sources in the area have not been able to connect to their system. An expeditious response will be greatly appreciated by all parties. If you have any questions or require any additional information, please contact our office. Thank you.
