CORRIGENDUM AND ADDENDUM TO THE SEARCH FOR THE 2018 ACHIEVEMENT OF GREAT INSTRUCTIONAL LEADERSHIP AWARD (AGILA) FOR TEACHERS,

SCHOOL HEADS AND PUBLIC SCHOOLS DISTRICT SUPERVISORS

This is in reference to the Regional Memorandum Number 155, s. 2018 and in line with the Revised Policies on Employee Suggestions and Incentive Awards System (ESIAS) provided under Civil Service Commission (CSC) Resolution No. 010112 and CSC MC No. 01,s. 2001, this Office adopts the herein Program on Awards and Incentives for Service Excellence (PRAISE) to be referred to as the "SEARCH FOR THE 2018 ACHIEVEMENT OF GREAT INSTRUCTIONAL LEADERSHIP AWARD (AGILA) FOR TEACHERS, SCHOOL HEADS AND PUBLIC SCHOOLS DISTRICT SUPERVISORS."

2. The search aims to give due recognition and incentive to public school teachers, school heads and public schools district supervisors who have concededly demonstrated an exemplary achievement in the Division.

3. Enclosed are the Guidelines, Criteria, and Nomination Form relative to the conduct of the 2018 AGILA AWARDS. The softcopy can be browsed and downloaded at www.depeddigoscity.org-Resource Panel.

4. The provisions of the Division Memorandum Number 375, s. 2018 that was released on April 26, 2018 are repealed.

5. Immediate and wide dissemination with this Memorandum is desired.

SEARCH FOR THE 2018 ACHIEVEMENT OF GREAT 1NSTRUCT1ONAL LEADERSHIP AWARD (AGILA) FOR TEACHERS, SCHOOL PRINCIPAL, PUBLIC

SCHOOLS DISTRICT SUPERVISORS AND DIVISION EDUCATION PROGRAM SUPERVISORS

To: Regional Office XI Division Chiefs Schools Division Superintendents Public Elementary and Secondary School Principals All Others Concerned

The activity is in line with the Revised Policies on Employee Suggestions and Incentive Awards System (ESIAS) provided under Civil Service Commission (CSC) Resolution No. 010112 and CSC MC No. 01, s. 2001, this Regional Office XI adopts the herein Program on Awards and Incentives for Service Excellence (PRAISE) to be referred to as the "SEARCH FOR THE 2018 ACHIEVEMENT OF GREAT INSTRUCTIONAL LEADERSHIP AWARD (AGILA) FOR TEACHERS, SCHOOL PRINCIPALS, PUBLIC SCHOOLS DISTRICT SUPERVISORS AND DIVISION EDUCATION PROGRAM SUPERVISORS." The Search will be spearheaded by the Quality Assurance Division of the DepEd Regional Office XI.

The search aims to give due recognition and incentive to public school teachers, school principals, public schools district supervisors and division education program supervisors who have concededly demonstrated an exemplary achievement in Region XI, during the World Teachers Days Culmination on October 3, 2018.

The entry folder shall contain the following documents and must be submitted in one (1) original and Two (2) certified true copy, to wit

1. A clear, simple, straightforward and understandable Executive Summary of not more than Five (5) pages in A-4 bond size, double spaced, describing the entry based on the set of criteria;

2. Properly accomplished entry form (ANNEX A) to be attached in the document folder;

The 11 Schools Division Offices (SDOs) of DepEd Region XI through the Division Selection Committee (DSC) shall conduct evaluation and selection process to come up with

a Division winners representing the Thirteen (13) categones and shall endorse the final results attached to the hard copy folder document to the Regional Office XI attention: Quality Assurance Division (QAD) OlC Chief Dr. Roy T. Enriquez on August 30 -31, 2018.

The Regional Screening Committee (RSC) shall conduct a Regional evaluation search from among the Division winners endorsed by the Schools Division Superintendent to choose the top 3 winners for each category on September 3 -7, 2018.

The RSC shall conduct ocular or on-site visitation to the top three (3) winners for each category to validate, conduct interview from among the internal and external stakeholders on the evidences presented by the Division winners on September 10 - 14, 2018 and subsequently conduct the finaf defiberation to choose the Most Outstanding 2018 AGILA Awardees representing the 13 categories on September 17-21, 2018.

It is encouraged that the 11 SDOs shall participate in the Search following the attached Guidelines and Criteria and to come up with a Division winner to represent each category.

Immediate dissemination of this Memorandum is desired.
