On -11/08 at' 1716 hours, with the Unit in Cold Shutdown(Operatin/89g ' Mode 5), a routine radiation barrier check identified! p'- a- faulty door locking mechanism. The north barrier door to the

East Valve Trench Area on the 722 foot elevation of the Primary' Auxiliary Building was found closed and unlocked. This door was,

L last . verified at 0808 hours this same day. This door restrictsaccess to a High Radiation Area. -A High Radiation Area is anarea' -with .a radiation field of greater than 100 millirem per '

hour- but, less than 1000 millirem per hour (mrem /hr). TechnicalSpecification 6.12.2 requires High Radiation Areas to havelocked. doors preventing unauthorized entry. This event is beingreported in accordance with 10CFR50.73.a.2.1.B, as a condition.in violation of Technical Specifications. The cause for this_ event- was attributed to a faulty door mechanism. The lockingmechanism would- not operate correctly due to welded latchingmechanism plate which interfered with the locking action of thedoor. The locking mechanism was modified. The interferinglatch was removed. All involved individuals were counseled withregards to the proper verification of HicJh Radiation Areabarrier doors. There were no safety implications as a result of

- this event. Personnel exposure records of individuals in-controlled areas during this time period were reviewed and noanomalies were identified.

DESCRIPTIONv

On .11/08/89 at 1716 hours, with the Unit in Cold Shutdown,

1 (Operating Mode 5), a routine radiation barrier check identifieda faulty door locking mechanism. The north barrier door to theEast Valve Trench Area on the 722 foot elevation of the Primary' Auxiliary Building was found closed and unlocked. This door wasverified locked at 0808 hours earlier the same day. This door,

. ROOT CAUSE OF THE EVENT

The cause for this event 'was attributed to a faulty doormechanism. . The locking mechanism would not operate correctlydue to welded latching mechanism plate which interfered with thelocking action of the door. The latch was found to flip back

QORRECTIVE ACTIONS!'The following corrective actions have been taken as a result of

Immediate:

|1. The area was immediately searched to ensure nounauthorized individuals were in the area. No Iindividuals were in the area. I

|2. All individuals that signed out RB1X keys wereinterviewed to identify individuals who accessed the Iarea.

3. Personnel exposure reports for individuals working incontrolled areas, including those who has signed outRB1X keys were reviewed and no anomalies wereencountered.
