CPP occurs in 1 out of every 5,000 to 10,000 children.

Puberty that starts too earlyDuring puberty, girls begin producing extra estrogen in the ovaries and boys begin producing extra testosterone in the testicles. These hormones produce the physical and emotional changes associated with puberty, such as breast development in girls and enlarged testicles and penis in boys.

When certain hormones are released too soon, a child can enter puberty earlier than he or she should. This may be the result of a condition called central precocious puberty (CPP). Your child’s healthcare professional should do tests to make sure your child has CPP before beginning treatment. There is no direct cause for CPP in most cases, but in rare cases, it may be due to an underlying medical condition.

What is central precocious puberty (CPP)?

SUPPRELIN® LA. SUPPRELIN® LA lasts for 12 months. One implant will give the medicine for12 months. After 12 months, SUPPRELIN® LA must be removed. The doctor may insert a newSUPPRELIN® LA at this time to continue treatment.

• SUPPRELIN® LA is placed under the skin of the inside of the upper arm. The doctor will numb the armof your child, make a small cut, and then place SUPPRELIN® LA under the skin. The cut may be closedwith stitches or surgical strips and covered with a pressure bandage.



Why is it important to treat CPP?

Treatment can help stop puberty symptoms until your child is the appropriate age

Your child’s full growth potentialPuberty usually causes a growth spurt. So, right now, your child might be taller than his or her peers. However, if a growth spurt happens too early due to CPP, your child could end up shorter as an adult than he or she might otherwise have been.

CPP treatments stop the puberty-causing hormonesYour child’s healthcare professional may recommend SUPPRELIN® LA, which is a GnRH agonist. This is a medicine that can help stop puberty by blocking the release of sex hormones in the body. When treatment ends, puberty can begin as normal—usually around age 11 in girls and age 12 in boys.

Medication delivery and duration of action are importantYour child may require treatment for a year or several years, depending on when the child begins therapy. If your child’s healthcare professional diagnoses your child with CPP, and if treatment is recommended, SUPPRELIN® LA can help suppress hormones associated with puberty. Your child’s healthcare professional will determine how long he or she will need to be on treatment.

If CPP is not treated and your child starts and completes puberty too early, his or her full adult height potential may be affected.


IMPORTANT SAFETY INFORMATION ABOUT SUPPRELIN® LA (cont)• Your child should keep the arm clean and dry and should not swim or bathe for 24 hours after

receiving the SUPPRELIN® LA implant. The bandage can be removed after 24 hours. Do not removeany surgical strips. They will fall off on their own in a few days. Your child should avoid heavy playor exercise that uses the implanted arm for 7 days. You and your child should routinely check theinsertion site until the cut is healed. After the cut has healed, your child can go back to his or hernormal activities. The doctor will give you complete instructions.

Demonstrated in a Clinical Study:* A clinical study was conducted in 36 children (33 females and 3 males), 4.5 to 11.6 years of age, to evaluate the effi cacy and safety of SUPPRELIN® LA for the treatment of CPP. Effi cacy assessments measured the suppression of gonadotropins (luteinizing hormone [LH] and follicle-stimulating hormone [FSH]) and gonadal sex steroids (estrogen in girls and testosterone in boys) on treatment. The primary measure of effi cacy was LH suppression. The initial duration of therapy was 12 months (plus a 1-month follow-up period). At the 12-month visit, all patients had their initial implant from Day 1 removed and those patients who continued to meet all effi cacy and safety parameters (as determined by the investigators) were eligible to receive a new SUPPRELIN® LA implant, with the possibility of rolling over into multiple 12-month extension phases.

SUPPRELIN® LA SHARES Copay Assistance ProgramDon’t let a copay stand in your way

Eligible patients may save up to $2,000 on each SUPPRELIN® LA after a $10 copay. The SUPPRELIN® LA Support Center will contact you if your child is eligible for this copay assistance program.

The majority of patients enrolled in the SHARES Copay Assistance Program pay $10 a year for SUPPRELIN® LA. For those who exceed the $2,000 amount, they may be eligible for additional assistance.

SUPPRELIN® LA. SUPPRELIN® LA lasts for 12 months. One implant will give the medicine for12 months. After 12 months, SUPPRELIN® LA must be removed. The doctor may insert a newSUPPRELIN® LA at this time to continue treatment.

• SUPPRELIN® LA is placed under the skin of the inside of the upper arm. The doctor will numb thearm of your child, make a small cut, and then place SUPPRELIN® LA under the skin. The cut may beclosed with stitches or surgical strips and covered with a pressure bandage.

• Your child should keep the arm clean and dry and should not swim or bathe for 24 hours afterreceiving the SUPPRELIN® LA implant. The bandage can be removed after 24 hours. Do not removeany surgical strips. They will fall off on their own in a few days. Your child should avoid heavy playor exercise that uses the implanted arm for 7 days. You and your child should routinely check theinsertion site until the cut is healed. After the cut has healed, your child can go back to his or hernormal activities. The doctor will give you complete instructions.

• The most common side effects of SUPPRELIN® LA are skin reactions at the place where the implantis inserted. These reactions may include pain, redness, bruising, soreness, and swelling in andaround the implant site. Call your child’s doctor if your child has bleeding, redness, or severe painwhere the implant was inserted.

Endo Pharmaceuticals Inc., the maker of SUPPRELIN® LA, wants to make the process of getting treatment as simple as possible.

Submitting the Form

Your healthcare provider may submit a Service Request Enrollment Form to the SUPPRELIN® LA Support Center to verify insurance coverage, benefits and any next steps, such as prior authorization or product access options.

Communications

A staff member from the SUPPRELIN® LA Support Center will provide you and your physician with information about your insurance coverage, and other access or assistance details that may assist you in understanding the treatment journey.

About 11 months after researching your insurance coverage, the SUPPRELIN® LA Support Center will remind your child’s doctor that it’s time to evaluate your child’s CPP treatment plan and will repeat the insurance and access research as instructed by the physician.

The risk of seizures maybe higher in people who have a history of seizures, epilepsy, brain or brain vessel (cerebrovascular)problems or tumors, or who are taking a medicine that has been connected to seizures such asbupropion or selective serotonin reuptake inhibitors (SSRIs). Seizures have also happened in peoplewho have not had any of these problems. Before your child receives SUPPRELIN® LA, tell thedoctor if they have a history of these problems, as well as all the medicines they take. Call yourchild’s doctor right away if your child has a seizure while taking SUPPRELIN® LA.
