What is hyperbaric oxygen therapy?Hyperbaricoxygentherapy(HBOT)–orhyperbaricmedicine–involvesthemedicinaluseofoxygenatlevelshigherthanatmospheric pressure. The treatment enhances the body’snaturalhealingprocessthroughtheinhalationof100%pureoxygen. The operation is performed to a predeterminedschedulebytrainedpersonnel,whomonitorthepatientandadjustthescheduleifrequired.

Under normal circumstances, oxygen is transportedthroughout the body only by red blood cells. With HBOT,oxygen is dissolved into all of the body’s fluids, plasma,central nervous system, bones and the lymph, and can becarried toareaswherecirculation isdiminishedorblocked.In thisway, extraoxygen can reachall damagedtissueandthebodycansupportitsownhealingprocess.Theincreasedoxygengreatlyenhancestheabilityofwhitebloodcellstokillbacteria, reduces swelling and allows newblood vessels togrowmorerapidlyintotheaffectedareas.Itisasimple,non-invasiveandpainlesstreatment.

Benefits of HBOTIthaslongbeenknownthathealinginmanyareasofthebodycannot takeplacewithout appropriateoxygen levels in thetissue.Most illnessesandinjuriesoccur,andoftenlinger,atthecellularortissuelevel.Inmanycases,suchascirculatoryproblems,non-healingwoundsandstrokes,adequateoxygencannot reach the damaged area and the body’s naturalhealingabilityisunabletofunctionproperly.HBOTprovidesthisextraoxygennaturallyandwithminimalsideeffects.

HBOT improves the patient’s quality of life in many areaswhen standardmedicine is insufficientor ineffective.Manyconditions such as stroke, cerebral palsy, head injuries andchronicfatiguehaverespondedfavourablytoHBOT.

ConditionsHBOTisusedtotreatconditionsthatbenefitfromincreasedtissue oxygen availability, as well as infections where itcan be used for its antibiotic properties, either as primarytherapy or in conjunction with other drugs. Depending onyourcountry/insurance,agrowingnumberofconditionsforHBOTarecovered forpayment,andscientific researchalsodemonstratesthebenefitsofHBOTforotherconditionsnotyetcovered.Thenumberofpeopleutilisingthistherapyhascontinuedtorise.

ExamplesofconditionsthatbenefitfromHBOT:• air or gas embolism• carbonmonoxidepoisoning• decompressionsickness(bends)• diabeticandotherwounds• exceptionalbloodloss(anaemia)• gas gangrene• intracranial abscesses• necrotisingsofttissueinfections• osteoradionecrosisandradiationtissuedamage• osteomyelitis(refractory)• skingraftsand(compromised)flaps• thermal burns.

RegulationsIHCHytechhyperbaricoxygen(HBO)chambersaredesignedand constructed to meet the latest stringent rules andregulations for medical technology in Europe, and complywith annex IV of the Council Directive 93/42/EEC and theEN guidelines 14931 (MDD). This means that IHC Hytech’schambersareassembledandmanufacturedusingthehighestqualitymaterialsandcomponents,allofwhichareacceptedbythenotifiedbodiesforuseinhyperbaricchambers.

The pressure vessel of a hyperbaric oxygen treatment system is designed and manufactured according to the EuropeanPressureEquipmentDirective(PED)2014/68/EU,andallpartsof theHBOchamberaredesignedaccording to thehigheststandards.Forexample,theHBOchambersareoutfittedwithahyperbaricfirefightingsystembuiltaccordingtoEN16081.However,IHCHytechisbynomeanslimitedtothisEuropeanstandard,andcanalsomanufactureinaccordancewithotherinternational standards such as the ASME. A third partyapprovestheentirehyperbaricsystemaftercompletion.

Themanagement system of IHCHytech has been awardedwith ISO 9001:2008 and ISO 13485:2012 certificates. Thelatter places emphasis on: meeting regulatory as well ascustomer requirements; riskmanagement; andmaintainingeffective processes – namely, the processes specific to thesafedesign,manufactureanddistributionofmedicaldevices.

IHC Hytech hyperbaric oxygen treatment chambers are manufactured to exact customer specifications, withdiametersvaryingfrom1,700to3,000mm,andanadaptablelengthtosuitthenumberofpatientsand/orthesizeofthebuilding.Round-shapedchambersweighlessandusefewermaterialsthanrectangularchambers,andbothareavailablewith double or triple lock systems. A triple lock chamberfeatures two entrance lock compartments, each of whichcan normally accommodate two people and has the samecommunicationdevicesasthemainchamber.

The doors of the chamber can be fitted with hinges orintegrated as a sliding door to reduce unused space. Therectangulardoorframeiswideenoughtoalloweasyaccessforintensivecarepatientsonfull-sizedhospitalbeds.Treatmentsessions are performed in the main chamber, and theentrancelockisusedtolockpatientsorpersonnelinoroutofthemainchamberwhenitispressurised.Comfortableseatsforpatientsareeasilyremovedfromthechamberinordertoallow for theuseof full-sizedhospitalbeds,wheelchairsormedicalstretchers.

IHCHytechisnotonlylimitedtodesigningandmanufacturinghyperbaricchambers.Itcanalsoprovideturnkeysolutions–forexample,architecturalsupportonthelayoutofafacility,andassistancewithfloor loadassessments. Inthisway, IHCHytechbecomesanextensionofthecustomer’sorganisationin order to ensure the end product is utilised in themosteffectiveway.

Safety features inside the chamberIHCHytechHBOchambersareequippedwithseveralreliablesafetyfeatures.Onlythehighestindustrialstandardmaterialsare used, such as corrosion-resistant Tungum alloy tubing,which is suitable for oxygen/breathable air. Flexible oxygenandairsupplyhosesarenotusedinsidetheHBOchamber.

All electrical cabling is composed of halogen-freematerial,whichisnon-toxicintheeventofafire,andallseatsaremadeof fire-retardant fabric. When the water fire-suppressionsystem(sprinklers)isactivated,waterfog(70%)anddroplets(30%)covertheentireareaofthechamber.Eachcompartmenthasitsownsystem.Inaddition,UVsensorsformpartoftheadvancedfire-detectionsystem.

Patient comfortThe chamber’s interior and exterior design can be chosenbythecustomer.This includesuniquehospitalorcorporatecolours,ora thematicapproachtocreateapatient-friendlyappearance. Efficient LED lights are installed inside thechamberandoptionalgradienttemperatureshelptoreducethecold,white-lighteffect.Eachpatientisprovidedwithanindividualseatwithfoldablearmrestandintegratedheadrest,andthefabriciseasytoclean.Anoptionalfootrestprovidesmaximum comfort.

Patients are offered an individual entertainment systemconsistingofaheadsetwithaudioconnections,videochannelconnections,volumecontrols,achannelselectionswitchfortheaudiosystemandanindividuallightoutputcontrol.

Breathing systemIHCHytechHBOchambersaresuppliedwithasophisticatedoxygen breathing system, allowing patients to breathcomfortably,withalowinhalationandexhalationresistanceof3mbar.Thetherapeuticgas(oxygen)issuppliedtopatientsbymeansofanoral/nasalmaskoranoxygenfree-flowhoodsystem,whereby the flow to each patient can be adjustedseparately.

Connections for the oxygen supply, entertainment systemandpatientmonitoringdevicesareinstalledintheoverheadpanel. Connection points are also available for the supplyofoxygenand/orair tomedical equipment suchaspatientventilators.

LCD touch screenThechambercanbecontrolledfromtheinsideusingtheLCDtouch screen. It displays session parameters such as depthandtemperature,andtheattendantinsidethechambercancontroltheinletandexhaustvalves.

After a session starts, the display will show the PatientInformation Screen. This presents information includingpressure,temperature,elapsedandremainingsessiontime.The attendant in the chamber can switch to the DoctorInformationScreen,whichshowsmoreinformationaboutthesession,including:• pressure graph• depth• ascent/descentspeedinmsw/minute• oxygen level• CO2 level• temperature• PPO2(partialpressureoxygen)level• PPCO2(partialpressurecarbondioxide)level• humidity(relative).

Vacuum systemThere are two vacuum control panels inside the mainchamber.ThevacuumsystemisdesignedtoworkwithBaxterMedevacorequivalentcontainers.

IHCHytechHBOchambersaresuppliedwithstate-of-the-artcomputercontroltechnology.Integratedintoanergonomicallydesignedstand,thecontrolpanelhasallcontrolsanddisplaysrequired to operate the system and monitor the patientsundernormalworkingconditions.

Thecontrolpanelconsistsofthefollowingitems.

• Touch-screen operating interface Various functions are controlledusing the touch screen,

suchas:compressors;coolingunits;heaters;CO2andO2 levels;water levels; alarm statuses; pressure levels; andlightinginsidethechamber.

• Data recorder Adatarecordertakesnoteofthepressuredataduringa

session.

• Analysation Twoanalysationpointsassessthequalityofenvironmental

gasinsidethechamber.

• Operator presence confirmation (OPC) button Anadditionalsafetyfeature,thisbuttonmustbepressed

toconfirmthepresenceoftheoperatorwhenrequiredbythe system.

• Emergency telephones Three battery-powered handsets, which are connected

together.

• Firefighting controls Thesprinklerinstallationisactivatedintwoways:byusing

eitherthecontrolpanelorviaabuttoninsidethechamber.

• Observation system Several cameras are installed inside the main chamber

andentrancelock.Monitorsaremountedonthecontrolpanel and canbeoperated via the controlson theharddiskrecorder.

• Breathing gas selection Theoperatorcanselectwhichbreathinggasissuppliedto

themasks(airoroxygen).

• Supply selection A valve is used to select betweenmain and back-up air

supplies.

• HBO communication system Communicationispossiblebetweenthecontrolpanel,the

entrancelockandthemainchamber.Thisincludesdiscretecommunicationbetweenoperatorandpatient,aswellasopencommunicationtoallpatients.Abuilt-inpatientcallsystemenables a patient to contact theoperator at thetouchofabutton.Eachpatientcanalsochoosefromfiveentertainmentaudiochannels.

• Flush control The chambermust be flushed to provide the occupants

withfreshair.

• Entertainment system controls Controlfunctionsfortheradio,CDandDVDplayers.

• Profile controllers Manualandcomputer-controlledoperationispossible.

IHC Hytech also offers a manual chamber-control systemwithoutthecomputerinterface.Intheeventthatthenormalpressurecontrolsystemfails,orinanothertypeofemergencythat requires the chamber tobedecompressed faster thanusual,anumberofextravalvesareinstalledonthemanualcontrol panel.

PLC computer programIHC Hytech uses a reliable industrial PLC (programmablelogic controller) for the computer steering systems of both chamber compartments. A selection of pre-programmedtherapyprofilesisincludedinthesoftware,andtheoperatorcan also create or load other suitable profiles. The PLCcontrols the pneumatic valves that regulate pressurisationand depressurisation of the chamber, but it is possible tooverride the computer at any time and assume manualcontrol of the system.

The computer and monitor installed in the control paneloperate as a feedback system for the PLC and display thefollowingparameters:

• actualpressureagainsttime• lapsedtime/pressureandtime/pressuretogo• pressure increase/decrease of both compartments and

speedofpressurisation• chamberpressuresinbothcompartments(msw/bar)• temperatureandrelativehumidity• oxygenpercentageandpartialpressure.

Atanytimeduringatreatmentsession,thepressureortimeprofilecanbechanged,andrelevantcommentsandpatientdatacanbeenteredforfutureuse.

As the pressure inside the chamber increases, so does thetemperature.Thisiscausedby:• incoming air from the compressors, which will be

compressedinthechamber• thebodytemperatureofthepeopleinthechamber.

TheIHCHytechcoolinginstallationisdesignedtocontrolthetemperatureinsidethechamber,mainlythroughcooling,andtocontrolthelevelofrelativehumidity.Thissystemcontainsthefollowingfundamentalcomponents.

Cold water machineFittedwith a circulation pump, and storage and expansiontanks.

Hot water unitWiththetemperaturesetat55°C.

Heat exchange unitA radiator-style heat exchange unit, provided with aircirculationchannelsandfans.Thehotairispumpedintothepatient chamber via the overhead gutter system, which isoutfittedwithelectricfans.Thesystemisinstalledinspecialsilenced housing in the main chamber, and is capable ofmaintainingthetemperature insidethechamberatapprox.21°C.

HP and LP supply systemThe high-pressure air supply system consists of one HPcompressorforpressurisingHPaircylinders,andseveralHPpressurecylindersforthemainandback-upsupplies.

The low-pressure air supply system consists of one LPcompressorforpressurisingLPbuffers,several low-pressurevesselsandonefilterunit.

D-MAS HyperSatThe DanMedical hyperbaric remote patient monitoringsystem (D-MAS HyperSat) is a computer-based device thatprovidesmedicaldiagnosticsandpatientmonitoring inoneself-contained unit for use in saturation diving applicationsand hyperbaric chambers. It is intrinsically safe for use inpressurisedenvironments,anddeliversafullrangeofremotehealthcare diagnostic andmonitoring support optionswithbuilt-inclinicalinterpretation.ThesystemisMicrosoft-basedandcanbeloadedwiththecustomer’sownsuiteofsoftwaretools, including desktop sharing and video conferencing.Divers can be viewed and monitored in real time fromanywhere in theworld via a standard internet connection.D-MAS HyperSat meets the requirements for physiologicalmonitoring, recordinganddata transfer for remoteviewingas part of health surveillance for professional divers. Thestandard D-MAS HyperSat is installed with D-MASmedicalsoftware suite 2015. Medical devices now included asstandardare:• bloodpressurecuffsinthreesizes(S,M,L)• pulseCO-oximetrywithMasimoSETfingerclipsensorfor

SPO2(saturatedoxygen)• restingECG(10-lead)• extendedECG• core temperature measurement• D-MASwebcamforstillimages• D-MASsoftwarelicencefor12months.

Several other medical functions are available as additionalextras.

Masks and hoodsIHCHytechcandeliverarangeofoxygenmasksandhoods.Themaskcoversnoseandmouthandissuitableforvariousrespiratory applications. The oxygen treatment hood hasbeendesigned for use in clinical hyperbaric chamberswiththe patient’s comfort in mind, and is easy to use for theattendant.Thehoodismadeofclearmedical-gradevinylwithanoptical-quality,extra-largeviewingwindowon the front.Itissimplefortheattendanttocleanbecauseitisweldedtotheneckring.Thisalsoreducestheriskofbacteriabecomingtrappedbetweentheneckringandthehood.

DisinfectantsSaniZidePlus®disinfectsanddeodoriseswithoutabrasivesorbleach,andkillsTB,HIV,CA-MRSA,MRSA,HepatitisA,B,andC,andH1N1.Itisanon-flammable,alcohol-freeformulation,andcanalsobeusedtocontrolmouldandmildew.

ClothingIHCHytechcanprovidespecialclothingforpatientstowearinhyperbaricchambers.ManufacturedbyMedline,theclothingis ceil blue in colour and the reversible hyperbaric scrubtop featuresa leftchestpocket– insideandout–andset-insleeves.It isfittedwithaV-neckcollarwithcolour-codedneckbiasforeasysizeidentification.Thepantsaremadeofafour-panelconstructionwithbackpockets,bothinsideandout,andallcomponentsincludingthread,tagsandtrimaremadeof100%cottonwithsoil-releasefinish.
