OBJECTIVES OF THE STUDY
• Identify the risk factors of hydrocephalus .• Be able to provide Nursing care for an easy
recovery of the client .• Be able to discuss the management of the
condition .• To device and implement a Nursing care plan
appropriate to a client.

Nursing Diagnosis
1.Ineffective cerebral tissue perfusion related to decreased arterial or venous blood flow
Measurable goal After 8 hours of nursing intervention the we
should assess the client activity and colour haemodynamic status to avoid complication
such as increased ICP
NURSING INTERVENTION
• Assessed the condition of the baby • Monitored vital signs of the baby • monitored head circumference of the baby
to assess whether increasing intra cranial
pressure • Neutral position maintained avoid increase
intra cranial pressure (immobility)
Evaluation
• After 8 hours of Nursing intervention babies hemodynamic status is improved
Nursing diagnosis
2 Alteration in thermoregulation ,hyperthermia related to increase vascular resistence
Measurable goal To maintain temperature in anormal range
within 4 hours
Intervention
• Assessed the general condition , colour of the baby
• Monitored vital signs , Hemodynamic status • Administered Antipyretics as advised By
doctors • Provided calm and quiet environment • Monitored skin turgor and mucous membrane
to maintain fluid and electrolyte balance
Evaluation
• Based on the intervention , goal babies condition improved and temperature maintaining within normal limits
In effective breathing pattern Due to Respiratory distress

To maintain Normal Breathing Pattern Within # Days
Intervention
• Assessed the geneal condition of the baby • Tried to decrease the oxygen consumption
which will help to reduce intra cranial pressure
• Administered Diuretics as adviced By doctors to reduce cerebral congestion
Evaluation
• After all these measures Discontinued continues oxgygen Requirement
Patient Family Education
• Instructed the parents about the use of medications • Adviced the parents to maintain hygiene , and avoid skin break
down to avoid infection • make the Parents to understand about the measures to avoid
increasing intra cranial pressure • Parenteral teaching gven how to give bottle feed and gavage
feeding• Instructed the parents about newborn cues,
