The Leamington Fire Services provides awide range of services to the 30,000 pluscitizens of the Municipality of Leamington.

Leamington is made up of both Urban & Ruralresponse areas and encompasses 92 squaremiles including 28 miles of Lake Erie shorelineand Point Pelee National Park.

The department operates from a singlestation that houses one Heavy DutyRescue/Command Unit, one 95 footAerial/Platform/Pumper unit, one 50 footSnorkel/Pumper unit, two Heavy Duty Pumperunits, one 1,500 gallon Pumper/Tanker unit.The department also operates three 4 wheeldrive units, (1 Chief's SUV, 2 D/Chief's

Pickups), one Fire Prevention Mini Van andfrom April to October, one 23 foot RescueBoat stationed at the Leamington Marina.

The department does not run medical callsand averages 400 - 500 call outs per year.

Code 4 has been our heavy duty rescueequipment supplier since 1980 when Chrissold us our first Hurst 32A hydraulic unit. Thedepartment has used nothing but HurstRescue Equipment and continues to updateand add to our inventory all purchased fromCode Four Fire and Rescue. Code 4 contin-ues to deliver first rate training and service onall equipment they have sold the departmentand remains our supplier of choice.

Today, fire departments seem to have little problem selecting theSpreader they want to buy. Ditto for the Power Unit. Even with

Rams, though there is more choice, they really are shorter or longer,but essentially, they have comparable push-pull forces.

With Cutters, for the first 22 years, you could only buy our 0-150Cutter (after the Spreader mounted shears were withdrawn. There wasa smaller O-90 that was of little demand).

However, when it comes to Cutters there is a lot of choice and a lotof variation.

A carpenter faces similar choices. Does he buy one saw to cut wood(2 x 4s require a different saw than that demand-ed by a fine finishing carpenter). The answer is no,because each saw has its own limitations in vari-ous applications.

Rescue cutters have limitations too. If you wantto consider the most powerful cutter in our HighPressure line, the S511 Cutter generates anastonishing 236,000 pounds of cutting force (in

our Low Pressure format,it is the MOC Ultra.

The trade-off is two-fold: Because the toolrequires larger Blades anda larger Piston, it 12pounds heavier than our30 pound S311 Cutter (orLow Pressure X-Tractor II

version) that generates 99,000 pounds.The 236,000 pound S511 will virtually cut

anything, but it will not overlap its blades as wellas its lightweight counterpart.

However, the less powerful S311 it is our bestselling cutter along with its Low Pressure counter-part - the X-Tractor II.

The S510 (MOC II in Low Pressure) is inbetween. It has gobs of power @ 169,000 lbs,and has a superior blade overlap to the Moc Ultra,though not as good as the S311 or X-Tractor II.

The S530 (X-Tractor S in Low Pressure) isstraight bladed from the notch outwards andHawk-Billed at the tips of the Blades. It is great incutting flat stock, but when cutting in many ele-ments of Auto Extrication, like a “C” Post, this cut-ter is prone to twisting - not a desired outcome.

Summary & Trends:As you can see, the Rescuer with one Cutter facessimilar challenges to the Carpenter with one saw.

Irecently spoke with a colleague regarding an extrication in which he wasinvolved and during the course of our conversation, I thought to myself (I dothat a lot)…”hey this would make a good piece!” So…here we are, dis-

cussing potential findings when we have thoracic involvement as the result ofa collision.

It’s no mystery as to when our patient assessment begins. We have dis-cussed in previous issues how to assess the vehicle(s) involved in order todetermine locations impacted, degree of damage and interior intrusion andestimate the energy transfer to the patient that was located in or near theimpact zone. Don’t forget to factor in other variables such as loose objectswithin the vehicle including unrestrained occupants.

In the situation as was discussed with my colleague, this was a frontalimpact with “massive” interior intrusion of the dash, steering wheel, and fire-wall. He described the space in which the patient was trapped as “so nar-row it was unbelievable that anyone was still alive.” Further details cannot bereleased due to the ongoing Police investigation.

Obviously, with a collision this severe, multi-system injuries are inevitable,however, today we will focus on the chest. Where do we start when we actu-ally get our hands on the patient? I remain faithful to the ABC’s and suggestthat is our starting point. Once we are able to maintain the airway as required,assessing the patient respiratory effort gives us a wealth of information.Whether they are conscious or not, performing the “look-listen-feel” test indi-cates potential or actually respiratory focus points. When looking at the chest,ask yourself, “Does the chest rise adequately? The rule of thumb is chest riseof approximately one (1) inch or better. Assess if the chest rises and fallsequally. Be wary of a possible chest wall flail segment that will reduce theeffectiveness of chest expansion and also internal pressure gradients. Isthere any visible reddening, bruising, swelling or penetrating injuries? Is thepatient exerting him/herself to breathe? Is there accessory muscle use? Alsocontinually assess the patient’s airway for the presence of blood or pinkish orreddened sputum. In many cases, listening to the patient’s breathing is quitea challenge in the vehicle, but it may yield very useful information.Auscultation of all lung fields should be done early in the assessment if pos-sible. Use these findings as a baseline in order to determine any changes asthe extrication progresses or during ongoing patient care. Be sure to note anyunusual breath sounds, particularly wheezes, stridor in the upper airway, andof course, decreased or absent sounds anywhere. As far as “feel” goes, wetend to restrict this assessment to just the volume of air exhaled by thepatient. However, if your chief concern for your patient is thorax involvement,

then it may be advisable to physically palpate thechest to determine the integrity of the chest walland rule in or out possible fractures in the ribs orsternum. Positive findings in these areas mayexplain why the patient is taking shallow, guard-ed breaths due to the pain they may be experi-encing during inspiration and expiration. I am notsuggesting discarding any other portion of yourrapid trauma survey, but if you feel this is yourtarget zone and breathing seems to be the mainproblem, then deciding the use of air medivacand trauma centers can be made expeditiously.

What are some specific problems that can be created following thoracictrauma? We can discuss a number of potential injuries, but due to spaceconstraints, I will focus on just a couple. An often overlooked injury isPericardial Tamponade. This injury should be a concern when a patient, com-monly the unrestrained driver, strikes the steering wheel with their chestwith/without airbag deployment. The transfer of energy can create bruisingto the heart muscle, however, should blood begin to leak between the mus-cle and the Pericardial Sac (sac that surrounds the entire heart,) it can createinward pressure on the heart itself. This building pressure restricts the abili-ty of the heart to effectively expand for blood filling which in turn reduces theamount of blood ejected during ventricular contraction. Expected signs andsymptoms may include those common to shock such poor skin colour, coolextremities with reduced or absent peripheral pulses, dyspnea, decreasedLOC, along with possible jugular vein distention (JVD.) Pain in the anteriorchest along with possible sternal step-like deformity may also present.

Another potential injury that must be suspected if your patient sustainseither blunt force or penetrating chest injuries may involve the Pneumothorax.This condition occurs when air is permitted to enter the “potential” spacelocated between the visceral and parietial layers surrounding the lung andinner chest wall. As air begins to build, it separates these two linings creatingan actual space. The increasing pressure begins to compress the affectedlung and reduces its ability to expand and take in air. This restriction thereforereduces the available volume required for effective exchange of oxygen andcarbon dioxide. As air continues to apply pressure to the affected side, thelung may collapse completely and begin to compress the heart and traveltoward the opposite lung. This progression toward the unaffected side isknown as a Tension Pneumothorax. Initial signs and symptoms may includevisible blunt or penetrating trauma, difficulty breathing, poor skin colour,decreased to absent breath sounds on the affected side. If using SpO2 mon-itors, watch the patient’s oxygen saturation levels for progressive deteriora-tion. If the injury progresses to become a Tension Pneumothorax, monitor forcirculatory changes such as shock-like findings and with visible tracheal tug-ging toward the uninjured side along with possible distended jugular neckveins (JVD.)

Air is not the only material capable of occupying this new actual space.Blood can also leak into the area creating the same restrictive compressionof the affected lung. This injury is known as a Hemothorax. Expect roughlythe same signs and symptoms, but circulatory compromise will occur morerapidly as the patient is losing blood volume into the chest cavity.

These are just a few potential injuries that you as responders mayencounter out there on the streets. I encourage you to take it upon yourself toreview these injuries among others, including both standard care and anyadvanced protocols you may have in your jurisdiction to treat your findings.In many cases, suspecting and rapidly finding these life threatening condi-tions may make the difference in your patient’s outcome. Assess all yourpatients with all your senses (well…exclude taste) and be suspicious at alltimes.

New Glasgow Fire Department, Island EMS andR.C.M.P were dispatched around 12:00pmSaturday May 9th to a 2 vehicle MVC on Rte #6 in

Cavendish involving a tractor trailer and a Mini-van withone person reported trapped in the Mini-van. NewGlasgow Fire department responded with rapid responsevehicle # 4

Engine 1 and Heavy Rescue 6 to the scene. Uponarrival they found one female patient pinned in the mini-van in a ditch with the tractor trailer pressed hard to thedriver side and a hydro pole on the passenger side.Island EMS arrived and were placed in the vehicle with

the patient while fire fighters started the extrication.Extrication was done with the Hurst high pressure

Centaur system. Due to the position of the hydro poleand guy lines firefighters made a purchase point on thepassenger side using a fender squeeze with the spread-er thus exposing the front door hinges. Once that movewas completed, the C6 Cutter was used to cut the frontdoor hinges and the door was removed, Paramedicsrequested further room for extrication and at that pointthe cutters were used to remove the B post with cuts atthe rocker panel and at the top of the B post just belowthe air bag cylinder that was identified during the bomb

check. Fire fighters and paramedics were than able tomaneuver the female patient onto the long back boardand than transported to Queen Elizabeth Hospital inCharlottetown. The patient has since made a fullrecovery.

The Windsor Fire Department is a volunteer departmentwhich has served the Town of Windsor since 1881.

The Windsor Fire Department members have also serveda portion of the Municipality of the District of West HantsDistrict since 1951. More recently the department wasincorporated as a Society in 1994. The 85 members ofthe department protect an area of over 200 square miles,including the 3800 people in the town and the 8000 peo-ple in the surrounding county. Coverage for this largearea is done out of one main station within the town andone substation, Station 2, within the county coveragearea. Within the fire district the department covers 31 kmof Provincial Hwy 101. This is the major route for alltrucking and traffic through the Annapolis Valley and forthe south western side of the province. This stretch ofHwy 101 demands that the department respond to vehi-cle fires, rescues, mvcs, and what ever else it brings. Toaccomplish this task WFD has a wide selection of firefighting and rescue equipment on hand.

Under the command of Interim Chief Scott Burgess,the Windsor Fire Department’s dedicated membersrespond to approximately 350 to 400 calls per year, andis dispatched from the Valley Communications DispatchCentre, in Kentville N.S.

The Windsor Fire Department has been the site for and co-host-ed with the Hantsport and Brooklyn Fire Departments the T.E.R.C.Vehicle Extrication Tournaments in the years 2007 and 2008.

Air Bags – Look to the Future

From Air Bags to Seatbelt Pretensioners who would ever think thatsafety could change that much in today’s motor vehicles. June15th, 2009 in Stockholm Sweden, Autoliv introduced their high

performance reversible seatbelt system.It is a new high performance reversible pre-pretensioner system for

seatbelts that resets itself automatically after triggering. During a mul-tiple collision impact, the high performance system will pull back theoccupants during the usually softer first impact to allow more optimumprotection from the airbags and seatbelts in secondary impacts.

The enhanced system is a pneumatically actuated reversible seatbeltpre-pretensioner. The belt pretensioning force comes from a com-pressed air motor and generates more than twice the tension forcesprovided by conventional electric pre-pretensioners. The higher forcesignificantly improves the performance especially in multiple impactcollisions. During the first impact, which can be relatively soft and with-out an airbag deployment, the occupant is pulled back into an up-right,safe position, thanks to the high force applied from the pneumatic pre-tensioner. With such a seatbelt system the occupant can be consider-ably better positioned for the second, more violent, impact when theairbag deploys for additional occupant protection.

Autoliv accident research data indicates that 25% of all accidentsare of multiple impact nature. They result in a 3-times higher risk forsignificant injuries to vehicle occupants and a 4-times higher risk forfatal injuries.

Studies by Autoliv Research indicate that the average time-gapbetween the first and the second impact is approximately one second.Thanks to the higher force (300N) of the pneumatic system the pre-pretensioner needs only 0.36 seconds to pull the occupant back into a

near upright position. The higher force willallow the seatbelt to pull back the occupant in90% of the cases before a potential subse-quent impact.

There are two other pneumaticallyreversible safety systems that are displayed intheir Demo Car which will be featured at theDetroit International Auto Show next Januaryin Detroit: A hood lifter for pedestrian protec-tion and a seatbelt buckle presenter whichmakes it easier to attach the belt when buck-ling up.

Pedestrian ProtectionFrom almost certain death to less than a 15%risk of life-threatening injuries in a car-to-pedestrian impact at 40 km/h. This is howefficient Autoliv's pedestrian protection sys-tem is that includes; the Active Hood and thePedestrian Protection Airbag (PPA). ActiveHood Autoliv's Active Hood raises instantlywhen a pedestrian is hit. A sensor system

placed in the vehicle's front bumper sends an electrical impulse to twosmall airbags which lift the rear part of the hood making the pedestri-an's head contact a deformable and flexible surface instead of a hardand rigid one. The sensor is so accurate that it can differentiate betweena lamp post and a human leg.

Pedestrian Protection Airbag (PPA)Autoliv has further enhanced its first pedestrian protection system bydeveloping a pair of airbags, one at each windshield pillar. As vehiclehoods tend to become shorter with every new model change, the riskfor pedestrians hitting the hard structures around the windshieldincreases, but this can be prevented by the PPA.

The windshield itself is a deformable surface and this, in combina-tion with a deformable hood and energy absorbing airbags at the sidepillars, result in a combination that is both very efficient and compre-hensive in terms of addressing the vehicle areas that are most danger-ous for the pedestrian.

In my next article I will discuss more changes to the airbag systemsand passenger safety.
