This is an exciting new post aimed at addressing the health inequality and health access needs of the Learning Disabled population of Orkney. The post holder will deliver a specialist Learning Disability Nursing Service to people with learning disabilities in Orkney and support their families/carers and involved agencies. A key part of this post necessitates a close liaison and co-ordination role with the learning disability specialist support services from Grampian. The post holder will be a confident and self motivated practitioner who is able to work within the national integration agenda, and is keen to reach out to a multiplicity of partner agencies and stakeholders in order to ensure that services for people with learning disabilities in Orkney are holistic and person centred. They will also be a registered Learning Disability nurse with current NMC registration and minimum experience of one year within the specialty. 2. JOB PURPOSE

To deliver a specialist Learning Disability Nursing Service, to people with Learning Disabilities in Orkney and to support their families/ carers and .involved agencies.

To organise and facilitate the learning disability specialist support services from Grampian.

To support, advise and liaise with all other Health Professionals to ensure the health needs of people with a learning disability are addressed.

To work closely with Primary care services in order to support them to understand and address the physical and mental ill health pre-dispositions of this population and the causes of health inequalities.

To work in an effective, multi-agency, multi professional way with services who support, manage and provide services for individuals who have a learning disability.

Within existing resources and work load capacity, to develop/ enhance current services within Orkney Health and Care and Orkney Islands Council, and to reflect the priorities of national documents/ Scottish Government agenda relating to the support, care and treatment of people with learning disabilities.

3. DIMENSIONS

The post holder offers a specific learning disability nursing service to both children and adults Orkney wide who have a recognised learning disability. There is a defined referral criteria which is operated under an open system. The post holder will offer support and advice in both the development and delivery of care plans.

4. ORGANISATION CHART

Operationally: reporting to Operational Manager, All Age Learning Disability Services

Professionally: reporting to Strategic Learning Disability Nurse (currently All Age Learning Disabiliies Service Manager)

5. ROLE OF THE DEPARTMENT

The Community Learning Disability Nurse is a member of the All Age Learning Disability Service, which also comprises of day service provision, support living accommodation, adult short break and child respite and family focus services, as well as a Social Work Team

The All Age Learning Disability Service supports people of all ages with a Learning Disability in Orkney. This is an integrated service which engages across organisations, agencies and sectors in Orkney in order to promote a holistic, person centred approach to care and support.

The service’s identified aspirations and objectives are articulated within a high level All Age Learning Disability Blue Print, local Service Delivery Plan, and are encompassed within the Integrated Joint Board Strategic Commissioning Plan. These documents reflect local and national drivers for Learning Disability services.

6. KEY RESULT AREAS

Responsible for the triage, assessment, planning, implementation and evaluation of all client referrals and required treatments..

To respond to requests for specialist consultancy regarding clinical presentations, specific learning disability information, or resources.

Provide data/ figures and contribute information for national or local databases, freedom of information requests.

Respond following direction/guidance of Strategic Learning Disability Nurse, regarding Scottish Government directives/ recommendations or local strategies/ policies and act as learning disability specialist representative on strategy/ policy groups (following consultation with Service Manager).

Plan, prepare and deliver specialist teaching regarding client specific treatment plans or tailored education sessions. Education programmes will be to a wide range of staff, organisations or client family members.

Responsibility for communicating effectively with a wide range of professionals, organisations, parents and carers, for the benefit of the client or service development.

Responsibility for the coordination and planning for visiting specialist services. This includes directing or facilitating referrals to their service, planning of clinic lists, appointments, case discussions/ conferences regarding clinical need.

Responsibility for identifying gaps/ updating clinical knowledge/ training need/ service need.

Responsibility for maintaining current professional knowledge and competencies.

Responsibility for ensuring that clinical supervision is maintained on at least a 6-8 weekly basis.

Where feasible, to link with other learning disability professionals as a professional networking resource

To meet with Strategic Learning Disability Nurse and Lead Nurse (OHAC), as necessary, to discuss professional nursing issues.

To be responsible for responding to legal requests e.g. when working with forensic need. This may involve giving evidence in court, writing reports, and making assessments.

Supporting carers and providers with the management of offending behaviour. Liaise with legal services and the police if required.

Involvement in Child/ Adult Support and Protection investigations/ protection plans/ involvement.

Involvement in Welfare Guardianship applications/ review process and assessments for Adults with Incapacity.

Following consultation and guidance from Strategic Learning Disability Nurse lead/ AALD Service Manager/Operational Manager, to conduct, local audits re Learning Disability Service health issues as well as implementing national and OHAC specific audits.

7. EQUIPMENT AND MACHINERY

The post holder is required to drive a car so they can access all areas of Orkney. Driving can be in difficult/ severe driving conditions such as high winds, rain, snow and ice.

Computer skills are essential, all client records are electronic and a number of different electronic databases are used daily. E-mailing, report writing, etc. are required to be conducted electronically.

The post holder is responsible for identifying and sourcing specific equipment/ resources required to carry out the job.

Video camera for clinical observations, laptop

Additional equipment/ machinery that belong to the client/ other services can be used to facilitate specific patient interventions e.g. wheelchairs, sensory equipment such as snoezelen, and specific augmentative communication aids.

When delivering training, lap tops, projectors, flip charts etc. have to be transported and assembled.

8. SYSTEMS

Systems that the post holder is responsible for utilising include electronic nursing records, electronic caseload database(s), visiting specialist clinic schedules. The post holder clinically decides which clients are to be seen, plans appointments, books venues, and sends out appointment letters.

The post holder will access Paris and other electronic client record systems as needed.

9. ASSIGNMENT AND REVIEW OF WORK

Client referrals can come from a wide range of sources e.g. education, health, social care, family or self-referral. The post holder is solely responsible for carrying out a specialist clinical assessment which will determine if the referral meets the referral criteria.

10. TRAINING AND EDUCATION

Content can be either client based or reflect a wider area of practice relevant to the field of Learning Disabilities such as behaviour management.

National documents/ policies – the post holder will, in consultation with the Service Manager and/or Operational Manager, reflect national priorities and apply these to the local context.

The post holder, in liaison with strategic LD Nurse Lead/Service Manager, is accountable for the delivery of the specialist Learning Disability Nursing Service. This includes caseload management, the assessment, planning, implementation and evaluation of complex/specialised programmes of nursing care and advice, and delivery of specialist advice in child/ adult support and protection scenarios.

11. DECISIONS AND JUDGEMENTS

The post holder is accountable for their own actions and will work under their own initiative. Where advice or support needs to be sought regarding operational matters the post holder will receive this via the Operational Manager for AALDS. Where support is required regarding professional issues the post holder fulfilling the strategic LD nurse role will provide this advice. The OHAC Lead Nurse will also be available to support the post holder regarding general, non-specialist nurse issues.

Deciding in consultation with others, when to call planning or case conferences with other areas/ agencies.

Being able to work autonomously when required, within remit of lone working policy.

The post holder needs to be alert and resilient enough to manage professional isolation and stress.

The post holder has the freedom to interpret policies and documents for the specialist learning disability nursing service i.e. developing own joint (with OIC) risk assessments, interpretation and implementation of Adults with Incapacity legislation Driver legislation and guidance such as ‘Keys to Life’ (2013)

The post holder needs to be competent in assessing and managing risk.

The post holder needs to function within the remit of their professional competencies and role.

12. MOST CHALLENGING/ DIFFICULT PARTS OF THE JOB

Being an autonomous practitioner, often working in isolated circumstances

Having to respond to varied demands to find solutions to potentially volatile situations that client's, carers and other professionals may highlight. This involves the use of problem solving skills, risk management, de-escalation techniques and interpersonal counselling skills in therapeutic relationships

13. COMMUNICATIONS AND RELATIONSHIPS

The post holder will be required to use communication skills to de-escalate conflict situations and to ensure the safety of clients, self and others by managing those situations appropriately. This can be demanding both emotionally and physically.

The post holder may be required to convey information of a particularly sensitive manner which does not align with client/carer/family expectations and desires.

The post holder is required to explain complex ideas and concepts to clients in relation to their health and treatment in clear and simple language in accord with the individual’s level of comprehension and via the most appropriate communication system(s).

14. PHYSICAL, MENTAL, EMOTIONAL AND ENVIRONMENTAL DEMANDS OF

THE JOB

Driving challenges: this involves continual risk assessment i.e. driving to visits/ meetings in adverse weather conditions, rural areas with poorly maintained tracks and the various hazards which arise when driving.

Dealing sensitively with clients who have high levels of anxiety and aggression which may result from causes such as pain, dementia, limited mobility, poor comprehension and communication skills, isolation and stigmatization

A portion of the job entails physical work and the post holder would require moving and handling skills which should be kept up to date as per NHS Orkney policy

At times the post holder will be required to assist clients with little/ limited mobility to move from one area to another.

The post holder may occasionally work within confined spaces, working with clients who are potentially a risk to themselves or others

At times violent/aggressive incidents may occur, and sustained effort or sudden intense effort in the management of these incidents may be required.

The post holder will be required to be able to initiate and maintain appropriate first aid emergency care.

There is a need for accuracy in the use and appropriate maintenance of all clinical equipment such as needles and syringes.

The post holder will meet the requirements of the European Work Time Directive.

15. MOST CHALLENGING/DIFFICULT PARTS OF THE JOB

Being an autonomous practitioner, often working in isolated conditions, and in so doing, having to respond to varied demands/ find solutions to potentially volatile situations. This involves the deployment of problem solving skills, competence in risk assessment and risk management skills, familiarity with de-escalation techniques, and adept use of interpersonal counselling/therapeutic skills.

Being a sole practitioner within the field, (other than the Strategic Learning Disability Nurse), this requires the practitioner to be aware of, and practice within, the remit of professional boundaries and competence. It also requires the practitioner to adopt a reflective practice approach, and to be a clear and assertive communicator.

16. KNOWLEDGE, TRAINING AND EXPERIENCE REQUIRED TO DO THE JOB

The post holder is required to be a registered nurse with current Nursing and The Midwifery Council registration under the Learning Disability part of the register.

Previous experience as a trained nurse for a minimum of 1 year within the speciality is essential.

The following are essential aspects of knowledge and experience:
