. Program Performance Award Official Entry Form Program The Program Performance Award recognizes and rewards County programs that show significant improvement in performance measure results of that sustain a high level of performance over a period of me (at least one year). Selected programs receive a cash award and Countywide recognion. Rating Criteria All programs meeng the first two (2) elements will be evaluated for the following four (4) areas by a panel of senior managers. Qualifying Criteria 1. Program - The submission meets the following program definion: An established and ongoing set of related measures and acvies aimed at accomplishing a clear business objecve, with dedicated staff, metrics/an evaluave process, and strategic oversight. 2. Time Requirement - The program had been established for a minimum of one year and has served customers in FY218-2019. Rang Criteria 3. Program Performance – Data is used to measure, evaluate, and improve program performance. Trend data demonstrates stellar improvement and/or exceponal performance. 4. Impact & Outcomes – The program aligns with the County’s Shared Vision 2025 and has significant impact at a program, department, or County level, benefing a specific populaon. 5. Collaboraon – The program works effecvely and collaboravely with stakeholders; the program leverages partnerships to make a greater impact. 6. Connuous Improvement – There are plans in place to measure and sustain high performance. Submission Process & Instructions Program submissions must be complete and no longer than three pages. The Program Performance Award recognizes and rewards County programs that show significant improvement in performance measure results of that sustain a high level of performance over a period of time (at least one year).

Selected programs will be notified. STARS recipients will do a 2-3 minute for Executive Council and the Board of Supervisors. Awards will be presented in January/February.
