How to Enroll Your Child in Polk County Public Schools Preschool Programs

1. Obtain and print out your child’s VPK (Voluntary PreKindergarten) Certificate of Eligibilitywww.floridaearlylearning.com/vpk Questions? 863-577-2450

To be eligible for the VPK Program your child must be 4-years-old on or before September 1, 2018 must reside in the State of Florida

Documents needed: • Child’s birth certificate• Proof of residency

2. Review Program options and locations• All programs require 2019-20 VPK Certificate of Eligibility issued by State of Florida for Polk.• Program options are limited to specific service locations.• No transportation is provided

a. AM or PM VPK: Part day program (3 hours) at no cost to parentsb. Full School Day: Extend the free VPK day by paying a fee of $12 per day (monthly payment $216).

• Payment agreement with signature and first month’s payment ($216)• PCSB employees – requires auto payment withdrawal plan

c. Title 1 PK Grant: Federally sponsored full day program at no cost. Must present two official documentsas proof of residence within the specific school zone identified as a Title 1 PK grant location.

3. Complete and submit the Polk County Schools Preschool Programs applicationa. Application cannot be completed or submitted electronicallyb. Documents needed:

• Original VPK Certificate of Eligibility (issued for Polk) signed and dated• Copy of child’s certified Birth Certificate (not hospital record)• Parent/Guardian picture ID• Proof of Residence (two documents) must include name and current street address

• Payment agreement and 1st payment if choosing Full School Day optionc. Submit the completed application, documents, and applicable payments to

4. If selected, you will receive a letter of acceptance from Polk County School Preschool Programscontaining the next steps to enroll at the assigned school. This letter will explain all the documents needed toenroll your child in the assigned school such as shot records, birth certificate, etc.

Para inscribir a su hijo en unos programas de Polk County School VPK o Titule 1 Programas Preescolares

1. Obtener e imprimir Certificado VPK (Voluntaria PreKindergarten) de Elegibilidad

o Certificado original de VPK de elegibilidad o Copia certificada acta de nacimiento (no registro de hospital) o Identificación de imagen de padre / tutor o El comprobante de residencia (dos documentos) debe incluir el nombre y la dirección actual de la calle o Acuerdo de pago y el pago del primer mes, si elige la opción del día escolar completo
