how to create and assign forms

How to create, publish and assign forms.
First thing we need to do is create a form. Click on the “Quick
Launch” icon in the top right.
Then select the “Forms” option.
In the screen that appears, click on “Forms” on the left hand side near the bottom,
then click on the blue button labelled “New Form” to the right.
A window opens which allows you to create your form. It’s quite self-explanatory,
but may take a while to get used to.
This is an example of a finished form. When done, click “Save Form” in the top left.
That is your form created.
There are a few ways for forms to work in Frog.
The first we’re going to look at is how to PUBLISH a form by inserting it into a page.
With this option you will have to tell people where to go in Frog to fill out the form.
Start by creating a new site or page, click “Edit” on the left hand side and click and
drag the “Forms” widget into the page.
Click on the widget to bring up its settings on the left hand side.
Click on “Import Form” to bring up this window. Select the form you want to be in
your page and click on the “Use” button to the right hand side.
You will then see your form inserted into the page. Click “Save Changes” at the top
and then “x” to close the Edit panel.
The last thing to do is publish the page so people can see it and submit the form.
Click on the “Share” icon at the top of the page.
Ensure that the page is set to “Public in the school”, or invite the people who you
want to see it. Make sure that the option “Can contribute” is selected from the drop
down menu, otherwise people will be able to see the form, but won’t be able to
submit it.
When people fill in and submit the form, you’ll get a notification like this in the top
right hand corner, letting you know who filled in what form.
By clicking on the notification you’ll be taken to the form. To retrieve the data /
results from the form, click “Edit” on the left hand side and select the “Form” widget
on the page, you will see a screen like this. Click the option “Edit Form”.
You’ll get a warning pop up like this, select “Export Data”.
You’ll then be presented with a graph of how many people have submitted your
form and other useful information. At the bottom will list it shows who has submitted
your form and what answers they gave. There is a cog icon to the top of this window
that exports the data into a spreadsheet, although at the moment it’s not very clear.
The next page will show how you can assign
specific people a form to fill out.
With your form already made, select the “Quick Launch” icon at the top and click
on “Forms”.
You will be taken to this window listing your forms. Click on the form you wish to use
the click on the cog icon to the left. From the drop down menu select “Assign”.
You can name the assignment what you like and associate it with a certain subject if
you wish. In the “Add people” field you can either add individual people or whole
classes or every year groups. When done click “Next”.
The next page gives you the option to set the assignment start and due date, as well
as giving people the choice to upload files to the activity. When done click on
“Assign”.
This is what a student will see. They’ll get a notification to say they’ve been issued the
form to fill out.
When they click on the notification they’ll be taken straight to the form. They fill out
all the required fields, click “Submit” at the bottom of the form, then “Complete
Assignment” at the top of the page.
As a member of staff, when you log in you’ll see notifications to say when people
have filled out the form.
Clicking on that notification will take you to your form, with the addition of
“Assignment” in the top left. Click on this and select “Data Viewer”.
You will then get this screen which shows you a graph and other useful information
as well as the answers and results of the form.
