These guidelines render eligible physicians to receive incentive payments through timely follow-up with patients after discharge from the hospital or emergency room. This ensures a seamless transition of care. The barrier to physicians receiving the incentives is that they are often not aware their patients were discharged from the hospital which makes it challenging for follow-up. Additionally, current alert capabilities stop short of workflow integration thus making them less usable for physicians. SmartStream for Transitions of Care HealthInsight Nevada and the HealtHIE Nevada HIE are using Cognosante’s SmartStream, an Intelligent Health Information Delivery system. This allows physicians to move beyond simple notifications and instead receive actionable information in real time. Customer Story

HealthInsight Nevada also serves as the Regional Extension Center and the Quality Improvement Organization in the state. With their vast knowledge of clinical and administrative processes, and experience in deploying quality initiatives, they understood that providing notifications of significant events was simply not enough. HealthInsight Nevada began working with Cognosante to design a system to bring significant value to the physicians they serve. Armed with input from their clinical team, the project team designed a solution with the physician’s workflow in mind.

How it WorksWhen a physician’s patient is discharged from the hospital, SmartStream™ extracts data from a real-time feed provided by the HIE. Using this data, SmartStream then generates and sends notifications to physicians by Direct secure message. Simultaneously, the physician is notified by text or by unsecured email with a link to the physician’s Direct secure account. A key advantage is the physician will immediately know if there is important information in his/her Direct secure account. After receiving an alert, physicians are asked to acknowledge what action was taken. Using this control feedback mechanism, SmartStream tracks action taken and sends reminders, as needed, from the time of discharge to completion of the follow-up appointment. Dashboards are available at any time to view the volume of discharges versus alerts sent out, how physicians responded, and within what time period. Reports providing historical information are available, and are critical in evaluating the effectiveness of the system and how it can be optimized.

Implementation SuccessThe pilot project went live in early January 2014 at five hospitals in the Las Vegas area. The expected benefits of deploying the SmartStream Transition of Care solution are:

• Increased Compliance meeting CMS guidelines regarding patient follow-up after discharge

• Increased Revenue Generation helping physicians qualify for CMS reimbursement. Additionally, the Transition of Care solution helps hospitals avoid costly CMS penalties

• Increased Quality of Care ensuring proper follow-up upon patient discharge• Reduced Costs automating and streamlining manual processes currently

employed by hospitals and physicians to coordinate care and prevent 30-day hospital readmissions

• Increased Operational Efficiency pushing critical patient information to physicians in real-time using a closed loop system to manage tasks throughout the Transition of Care process

• Increased HIE Usage bringing the data in the HIE alive at the point of care
