Thank you for your participation in the Houston Children’s Festival 2012 presented by Baker Hughes, benefiting Child Advocates, Inc. The Festival is bigger and better than ever! We look forward to having you on our team. This is a time of great excitement and fun, but also a time when each volunteer’s time and expertise is essential. We are counting on you- rain or shine! WHAT YOU NEED TO KNOW! ON-SITE CHECK IN Volunteers will report to Volunteer Headquarters located at Lamar and Bagby. From the parking garage (located at W. Dallas and Clay) walk east on W.Dallas, which turns into Bagby, heading north. Proceed north on Bagby to Lamar. Volunteers Headquarters will be at the corner. Look for directional signs once leaving the garage. . Remember to bring your signed volunteer agreement to check-in. After you are checked-in you will receive your festival wristband and T-shirt. VOLUNTEER PARKING FREE Volunteer Parking is available in the surface lot at Dallas & Clay and in the Met Garage across from the surface lot. Print your pass out and place on the dash or your car! YOU MUST FILL YOUR NAME AND SHIFT TIME ON THE PASS. Your volunteer name will be verified at the parking entrance WHAT TO WEAR Wear nice pants or shorts and comfortable shoes. Fanny packs are a good idea. At check- in you will be given your FREE festival T-shirt. Food & Beverage Volunteers must wear a HAT (NO VISORS) All volunteers should wear or bring a hat with you if you are not assigned inside an admission or coupon booth. WHAT YOU NEED TO KNOW!

In consideration for being allowed entry into and participation at the Houston Children’s Festival,

benefiting and provided by Child Advocates, Inc., I hereby acknowledge the following by my signature

below:

1. Only volunteers that have signed this agreement and receive the official volunteer sticker at volunteer

headquarters on site can work in the assigned areas.

2. I will arrive thirty minutes before my shift and will report to Volunteer Headquarters.

3. If I cannot report for my shift, I will try to find a replacement and if possible, give them my shift time,

wristband and where to check-in. I will notify my director or coordinator (or other Festival official) as

soon as possible.

4. I am responsible for insuring that my conduct, behavior and actions will reflect the highest professional

manner as a representative of both the Houston Children’s Festival and Child Advocates, Inc.

5. RELEASE OF LIABILITY AND INDEMNITY AGREEMENT

I (on my behalf and on behalf of my heirs, personal representatives, executors, administrators,

successors and assigns) hereby agree to RELEASE, INDEMNIFY, SAVE AND HOLD

HARMLESS The Houston Children’s Festival, Child Advocates, Inc., Spectrum Catering and any

sponsor of the Event or any other persons or entities associated with the Event as well as each of

their respective officers, agents and employees (the “Released Parties”) of and from any and all

losses, claims, damages, demands, liabilities and/or lawsuits for personal injury (including bodily

injury and/or death), property damage or other losses, including any costs, expenses and

attorney’s fees (“Claims”), which may in any way arise from or in connection with any of the

activities and/or responsibilities which I may be involved with as a volunteer of the 2010 Houston

Children’s Festival, INCLUDING CLAIMS OCCASIONED WHOLLY OR IN PART, BY ANY

ACT, OMISSION OR NEGLIGENCE OR STRICT LIABILITY OF A RELEASED PARTY.

6. I further understand and acknowledge that I am not an employee of Child Advocates, Inc., the Houston

Children Festival or Spectrum Catering., and certify that I am physically fit and competent to perform the

work as a volunteer of the Houston Children’s Festival 2012. I agree to act in a reasonable and

responsible fashion, and agree that I will not undertake or perform any activities or duties as a volunteer

while under the influence of alcohol or drugs.

7. I certify that I have never been convicted, or received deferred adjudication, as a sex

offender or of any crime related to abuse, assault of violence toward a child. Further, I

authorize Child Advocates to review any sex offender registries under my name. 8. I hereby certify that I have read and understand the terms and provisions of this agreement, including the

release/indemnity provisions set forth hereinabove, and I agree to assist as a volunteer in providing a safe

and fun environment for all volunteers, patrons, employees and others associated with or attending this

event.
