SECONDARY PREVENTION IN HIGH RISK PATIENTS WITHATRIAL FIBRILLATION AND AFTER ACUTE CORONARY SYNDROME:
FROM DISCHARGE TO CARDIAC REHABILITATION
LA PREVENZIONE SECONDARIA NEI PAZIENTI ADALTO RISCHIO DOPO SINDROME COR ONARICA ACUTA: DALLA DIMISSIONE ALLA CARDIOLOGIA RIABILITATIVA
A satellite symposium of the 2019 autumn meeting of the Section Nucleus of Secondary Prevention and Rehabilitation of the European Association of Preventive Cardiology (EAPC)
There is current need for discussion of structured secondary prevention intervention in high risk patients after acute coronary syndrome (ACS) and/or with atrial fi brillation. Particularly in the fi eld of thrombotic and lipid management, since the availability of modern strategies (i.e. new antiplatelet agents, novel anticoagulants, and PCSK9 inhibitors), a tailored prescription based on individual clinical and risk profi le is mandatory. Together with appropriate drug prescription, there is also need to promote better patient’s adherence – both embracing treatments and lifestyle – in order to ensure favourable outcomes and to reduce the global burden of cardiovascular disease in the population. As a satellite symposium of the annual autumn meeting of the Section Nucleus of Secondary Prevention and Rehabilitation of the European Association of Preventive Cardiology (EAPC), the present appointment provides the opportunity to discuss real life situations in the context of acute cardiology and cardiac rehabilitation, with the presence of national and European experts.

CONCEPT
• How to perform a “modern” risk estimation after acute coronary syndrome
• Antiplatelet therapy in the fi rst year and in the long-term after ACS
• Epidemiology of atrial fi brillation after ACS or CABG and how it affects cardiovascular risk, comorbilities, disability, and quality of life
• Recommended strategy of anticoagulation in atrial fi brillation after ACS or CABG
• Current lipid lowering modalities in CAD, in both ACS and SCAD, in Italy and across Europe: a snapshot of real life
• Focus on new lipid lowering strategies for high risk groups: who does what
• How to evaluated exercise tolerance and prescribe physical activity in high risk patients after ACS
• How to integrate the acute and cardiac rehabilitation setting for ensuring better prognosis in high-risk ACS patients
• Guided clinical cases on “high needs patients” after ACS or CABG: 1) patient who needs prolonged antiplatelet regimens; 2) patient who needs triple antithrombotic regimen; 3) patient who needs new lipid lowering regimens.
