    Dear Sir,

    Sub: Renewal of our Trade Licence

    This Hospital is a non profiteering charitable Hospital. Service not earning money has alwaysbeen the motto of the Hospital. A Pen Picture of the Hospital showing its working is enclosedherewith along with a list of Doctors attending the Hospital.

    The Trade Licence of the Hospital is not being renewed for the reason of not having a FireLicence. We have already applied to the Fire Department of the Government of West Bengal. AXerox copy of the receipted application is enclosed herewith. The issue of Fire Licence is alengthy process is not unknown to you & it will take a lot of time. The renewal of other licenceshave been held up without the Trade Licence although we have deposited the requisite renewalfees of all the licences.. We fear that the Hospital may have to be closed down for want of thedifferent licences.

    We assure you that we will forward you the Fire Licence as when received by us & request youto kindly issue a Provisional Trade Licence to continue.

    AGREEMENT OF TENANCY

    THIS AGREEMENT OF TENANCY made this 1st day of April Two Thousand Eleven between SriSubodh Kumar Das son of Late Phatik Chandra Das of Gopalpur, P.O.Rajarhat, Gopalpur, North

    24 Parganas, Pin Code 743 510, hereinafter called the LANDLORD of the First Part.

    and

    Smt. Sushila Devi Keshan wife of Sri Krishna Kumar Keshan residing at 3, Goenka Lane, Kolkata 700 007 hereinafter called the TENANT of the Other Part.

    WHEREAS the First Party is the owner of the Land & Shed lying and/or situated at Gopalpur, P.O.Rajarhat-Gopalpur, North 24 Parganas Pin Code 743 510 and the First Party is in the lawfulpossession of the said land. The First Party has expressed his intention to let the said land, morefully described in the schedule, to the other party above named on the monthly tenancy basisand the other party has agreed to take the said Land & Shed on the following terms andconditions:-

    1. The above mentioned tenancy will commence on and from the date 1 st day April 2011 asagreed between the Landlord and Tenant above named.

    2. The other party/tenant shall pay to the Landlord above named a sum of Rs.4235/- andRs.1000/- for Tower Plus 7%, Tax 4% Khajna on rent from April 2011 of the said Land & Shedmorefully described in the schedule without delay or default. The rent of every month ispayable within 14 th day of next following month as English Calendar month.

    3. The Tenant shall occupy the said Land & Shed for her own business purpose and shall haveno right to sub-let or assign the same except without permission from the Landlord.

    4. The Tenant shall have the right to make necessary repairs and replacement of the land fullydescribed in the schedule at her own costs and for this purpose the tenant will obtainproper permission from the Landlord.

    5. The Tenant will have no right to store highly inflammable articles in the said Shed. TheTenant shall be liable to the Landlord for all losses and damages of Shed & others caused byreason of violation of any of the four terms and conditions of this agreement.

    6. The Tenant shall keep all Land & Shed morefully described in the schedule in good repairs ather own costs subject to normal wear and tear.

    7. The Tenant shall not carry on or cause to be carried on or allowed to be carried on thedemised premises any business trade or vocation of any offensive or illegal nature nor shallshe do any act, deed or thing which may be calculated as nuisance or disturbing peace of the neighboring public.

    8. The Tenant will have right to collect/pay Trade Licence and any rates & taxes of Municipalities/Government for the using of rented shed for commercial purposes.

    9. The Tenant do hereby undertake to increase the existing rent at the rate of 10% on theexpiry of every 2(Two) years Rs.1000/- for Tower p.m., plus Tax & Khajna 7% on rent amountp.m. and the Tenant will pay the rent at the enhanced rate to the Landlord.

    10. The Tenant has paid at the time of execution of this agreement a sum of Rs.11000/- (ElevenThousand) only as advance and the Landlord doth hereby acknowledge the receipts of the

    same and undertake to refund the same without interest to the Tenant at the time of receiving vacant possession of the tenanted land fully described in the schedule.

    11. That if the Tenant surrender the above named tenancy then the said Tenant will have togive Two month prior written notice and if within the said period the advanced sum not yetliquidated the Landlord will pay the balance amount to the Tenant at the time of delivery of possession of the tenanted land.

    12. If the Tenant fails and neglect to pay the monthly rent as mentioned above for consecutivethree months and/or violated any of the terms and conditions as stated above Landlordhave the right to terminate the tenancy by giving one months notice and to re -occupy thesaid land in due course of law.

    THE SCHEDULE OF LAND ABOVE REFERRED TO:

    One pacca shed measuring about 1600 square feet lying at Gopalpur, P.O.Rajarhat-Gopalpur, North 24Parganas, Pin Code- 743 510 butted and bounded on the East and South:- Factory of Sri Subodh KumarDas sole proprietor of M/s. Jay Industries and on West:- Village Road.

    I am happy to appoint you to the post of a Part Time RMO in the Hospital initially for one month at aremuneration of Rs.15000/-. The duty hours will be 10a.m. to 6p.m.

    The existing RMO will handover the charge to you at 10a.m. & you will also handover the charge to thenext RMO at 6p.m.daily.

    I am confident that you will be sincere to your service & try to do all efforts for the betterment & theHospital.
