PHYSICIAN, PHARMACIST & NURSEThe Evaluation, Credit Claim & Certificate forms are in your conference folder. The Evaluation Form was created by the UAMS College of Medicine and is required to receive CE credit. Physicians, pharmacists and nurses must submit their Evaluation Form along with their Credit Claim Form at the conference to receive CE credit. You should indicate on the Credit Claim Form the sessions attended including hours earned and total hours earned on the form, as well as completing the remaining fields. Then the total number of hours earned (as indicated on the credit claim form) should be recorded on the Certificate. KEEP the certificate for your record. Submit the Evaluation and Credit Claim Forms at the designated Registration table at the end of the conference (at end of day if not returning the following day).

ABOUT THE CONFERENCE

THE PURPOSE OF THE CONFERENCE is to share evidence based information in pain & symptom management and end of life care, along with communication tools to better care for patients with serious illness. At the conclusion of this activity, the participant will be able to:

z Develop practical strategies for introducing discussions of a patient’s end of life care plan. z Develop practical techniques to improve advance care planning. z Acknowledge updates in quality, regulator and compliance areas of hospice. z Manage the physical, psychological, and social causes of distress in life limited patients and their families. z Increase knowledge of drug classifications/medications. z Increase knowledge of symptom management for the seriously ill.

TARGET AUDIENCE is Physicians, Physician Assistants, Pharmacists, Nurse Practitioners, Nurses, Nursing Home Administrators, Social Workers, Chaplains and other healthcare professionals interested in hospice, palliative care and/or issues related to serious illness.
