How Big is the Problem?
• AF is the most common arrhythmia
• Affects more than 6 million indiv iduals in the U.S.
• Projected to increase to 16 million by 2050
• Lifetime risk in men and women >40 is 1 in 4
• Patients with AF hav e a 5-fold higher risk of stroke

• Other concerns and contraindications with OAC include:1-2
• Patients with renal and liver dysfunctions (for NOAC)
• High risk of falls
• Noncompliance
• Those requiring dual antiplatelet therapy after stenting
• For warfarin:
• Additional issues with drug and diet interaction

Conclusions: In patients with NVAF at increased risk
for stroke or bleeding who are candidates for chronic
anticoagulation, LAAC resulted in improv ed rates of
hemorrhagic stroke, cardiov ascular/unexplaine d death,
and nonprocedural bleeding compared to warfarin.

Left Atrial Appendage Closure What Have We Learned
• The hypothesis that in patients w ith
nonvalvular atrial fibrillation that stroke results from LAA thrombus is correct
• Benefit of LAA occlusion remains stable
• >90% of patients can be taken off of warfarin w ithout harm and some patients
may never need warfarin at all if they
receive a device
• There is a learning curve, the more you do,
the better you are at doing it

Indications for Use
The WATCHMAN Device is indicated to reduce the risk
of thromboembolism from the left atrial appendage in patients with non-valvular atrial fibrillation who:
• Are at increased risk for stroke and systemic
embolism based on CHADS2 or CHA2DS2-VASc scores and are recommended for anticoagulation therapy;
• Are deemed by their physicians to be suitable for
warfarin; and
• Have an appropriate rationale to seek a non-pharmacologic alternative to warfarin, taking into
account the safety and effectiveness of the device compared to warfarin

• Implanted in a hospital with an established structural heart disease and/or electrophysiology program and performed by an interventional cardiologist, electrophysiologstor cardiovascular surgeon meeting the following criteria
• Has received training prescribed by the manufacturer on the safe and effective use of the device prior to performing LAAC
• Has performed ≥≥≥≥25 interventional cardiac procedures that involve transseptal puncture through an intact septum
• Continues to perform ≥≥≥≥25 interventional cardiac procedures that involve transseptal puncture throughan intact septum, of which at least 12 are LAAC, overa 2-year period

• The Centers for Medicare & Medicaid Services (CMS) covers percutaneous left atrial appendage closure (LAAC) for non-valvular atrial fibrillation (NVAF) through Coverage with Evidence Development (CED) under 1862(a)(1)(E) of the Social Security Act with the following conditions:
• LAA closure devices are covered when the device has received Food & Drug Administration (FDA) Premarket Approval (PMA) for that device’s FDA-approved indication and meet all of the conditions specified below:
• CHADS2 – ≥2
• CHADS2DS2-VASc – ≥3
• Shared decision making with independent non-interventional MD
• Suitable for short-term AC but deemed unable or unsuitable for long-term
• Trained physicians
• Enrolled in a registry

• LAAO with Watchman Dev ice was approved by FDA in 2015 based on PROTECT AF and PREVAIL RCT’s and companion Registries
• Dev elopment of PMS Registry a condition of approv al planned to begin enrollment 2nd quarter 2016
• Between approv al but before PMS study, commercial cases continued and were entered into a procedural data base by Watchman clinical specialists using standardized forms

• Watchman dev ice: self expanding nickel titanium structure ranging in diameter from 21-33 mm, cov ered on left atrial surface by permeable fabric cov er and anchored at the LAA ostium
• IFU for FDA approv al identified patient selection criteria to include
• High risk for stroke
• Increased risk for bleeding
• Able to take short term anticoagulation
• Not felt to be a good candidate for long termanticoagulation

Conclusion
• Following United States FDA approval of the
Watchman™ left atrial appendage occlusion device, implant success is high at 96%,
comparable to that observed in the pre-approval clinical trials. The most common
device sizes used are 27mm and 24mm. At
implant, most patients had no evidence of residual jet upon device release.
