About This Installation Request Form

Document PurposeThe Installation Request Form document is used as part of the Atea Coordinated Implementation Service. The form is intended to gather pertinent information from the customer or partner that will be needed by the Atea Installation resource to complete the installation of the component(s).

Intended AudienceThe intended audience for this document is the Customer Project Manager and Video/Network Administrator at the customer site.

ScopeThis document will cover the Technical Requirements needed to implement a Cisco TelePresence™ video endpoint(s). Also the installation and configuration of a standalone router for external endpoints if needed.

Assumptions and CaveatsPartner, Customer, and/or End User Point of Contact completing the Installation Request Form will have reviewed the relevant documentation (Getting Started Guide, Administrator Guide, etc.) for the component(s). All IP address information needs to be added to this document before the install or installation may be delayed.

Installation Request FormThe following sheet must be completed and e-mailed back before installation date(s) can be determined. Please allow for a 15-business day advanced notification on all Installation Date requests. Customer must verify that all customer requirements are completed before the installation date, as return trips are not authorized.

Only product(s) outlined within the Service Description and Service Order will be installed by the technician onsite. NOTE: Atea will not be responsible for mounting wall brackets to walls for Profile line and Edge line (camera and codec wall mounts) Systems. This is the sole responsibility of the reseller and/or customer. In such cases, Atea will be responsible only for mounting on or to an already wall-mounted bracket. Extra equipment that is supplied by the end user or reseller will be the responsibility of the reseller.***We ask that you book the room where the equipment will be installed for at least 8 hours to allow enough time for installation/training and any issues that may arise.***
