The HL7 greenCDA Implementation Guide has been published by the HL7 Structured Documents Work Group. The HL7 Clinical Document Architecture (CDA®) is at the core of the requirements for Meaningful Use of Electronic Health Records. It supports continuity of care and re-use of clinical data for public health reporting, quality monitoring, patient safety and clinical trials. greenCDA maintains the utility of CDA while making it easier to implement. It is a simplified XML for CDA templates.

“Any developer with basic XML knowledge and a tool that can process simple XML sche-mas can create green instances. We flattened the hierarchy, focused on variable data ver-sus fixed structural markup, and removed complexities like xsi:type. The result is simple and intuitive,” said Rick Geimer, Lantana Group CTO and co-editor of the greenCDA Implementation Guide.

greenCDA features include: • XML schema validation • Simple business names • Tagged data elements in extensible library • Rapid path to Meaningful Use compliance • Modular XML with business names generate JAVA, .NET • Single style sheet display, as for all CDA • Extensible to physician documentation requirements and quality

The enthusiastic response to the development of greenCDA is driving rapid experimenta-tion and has raised the question of how greenCDA fits into the larger ecosystem of clini-cal information systems. This trial use and experimentation will help us understand how going green affects ease of use for data capture; management and analysis; when it might be an appropriate wire format for CDA; if there are significant limits on expressivity; and where the cost and benefits may lie.

The CDC is planning a pilot project with vendors interested in using greenCDA to enable use of their systems for submitting Central Line Insertion Practices (CLIP) data to the National Healthcare Safety Network (NHSN).

“Use of greenCDA and supporting transformation tools show great promise as an approach for reducing the effort required to implement fully normative CDA,” said Daniel A. Pollock, MD, Surveillance Branch, Division of Healthcare Quality Promo-tion, Centers for Disease Control and Prevention.

Respiratorydiseases,especiallychronicobstructivepulmonary

disease(COPD),lungcancerandtuberculosis,aremaincausesofmortalitythatwillcontinuetoincreaseinthecomingdecades.Aspirometeristhemedicaldevicemandatedtomeasurethepulmo-naryvolumeandcapacity,identifyingpossiblealterations.Commonly,alldeviceshaveapropri-etarydataformatoutput.Thisisasetbackfortheirintegrationindifferentenvironmentsbecausewhendataisstoredonasharedrepository,itisnotin-teroperablesinceallofthedatadoesnotsharethesameformatnordoesitcontainstructureddata.

Drivenby“Oficinad’EstàndardsiInteroperabilitatdeTICSalut”and“PladeDigitalitzaciódelaImatgeMèdicadelDepartamentdeSalutdelaGeneralitatdeCatalunya,”astandardhasbeencreatedbasedontheHL7ClinicalDocumentArchitecture,Release2(CDA®R2).Thegoalofthestandardistonormal-izeacompletedataset,includingbothdatareceivedfromspirometersaswellasthosethatcomefromthetestcitationprovidedbytheelectronicclinicalhistoryfromahospitalormedicalcenter.

Consequently,thisstandardcreatesaspirometryreportthatcontainsnotonlytheinformationre-

latedwiththespirometrytest,butalsoallthedatafromthetestrequest,patientidentification,andspirometer.Thissetofdatacompiledfromdiffer-entsourcesrequiresapplyingaCDAR2structure,orientedtoeasetheintegrationbetweenmedicaldeviceandthehealthinformationsystem(HIS),andahigherinteroperabilityamonghospitalinfor-mationsystems.

Thedatamodel1hasbeendevelopedbyamultidis-ciplinaryscientificteam,consistingofpulmonolo-gists,health-techexpertsandspirometermanufac-turers,thusprovidingdifferentperspectivesaboutthismodel.Themodelisthusenrichedbythediversityandvastknowledgeoftheteam.

Twoversionsofthisdatamodel1exist.Thefirstversionismoredetailedandisclearlyorientedtoasubsequentexecutionofadataminingsystem.Thesecondversionismorebasicandtakesintoaccountthatnotallthecentersorhospitalscanprovidetheinformationrequiredbythedetailedversion.

Afterthedatamodelwasdeveloped,asetofnor-mativeandtechnologicalarticfactswasgeneratedtofacilitatethestandardimplementation:

This standard creates a spirometry report that contains not only the information related with the spirometry test, but also all the data from the test request, patient identification, and spirometer.

• CDAR2SpirometryImplementationGuide2:ThisguidecontainsthenormstofollowtoimplementCDAR2correctly,includingmanda-toryfieldsandtheircontent.Twoversionsofthisimplementationguidehavebeencreated—oneforeachversionofthedatamodel.

• CDAR2XMLFormattedTemplates:AsetofCDAR2spirometrytemplateshasbeencre-ated.Templatesexistforbothversions,basicanddetailed.

• XSLStyleSheet:ThisisafileneededtovisualizespirometryCDAR2,whichfollowsastandardstylesheetforCDA-HL7presentation.

UsingspirometryCDAR2allowsfortheresultingreportstobesharedthroughdifferenthospitalhealthinformationsystems,andexecutesdataminingservices,thatareveryimportantformedicalresearchprocesses.Itisalsoimportanttonotethatthedoctorcanviewthespirometrydigitallyfromhisworksta-tionandwatchthetestshistoryforeachpatient.

ThefirstimplementationoftheCDAR2spirometrystandardwasthroughanopen-sourceintegrationframeworkcalledEI2Med,basedonMirthConnect,inwhichmanytoolshavebeendevelopedtoeasegenerationandintegrationbetweenstandardfilesandHIS.ManufacturersandspirometrymodelshavebeenintegratedwiththeintegrationframeworkEI2Med.

PublichospitalsinCataloniaarecurrentlycollabo-ratingonpilotprojectstovalidatethenormaliza-tionandintegrationtechnologyofthespirometrytests.ThereareplanstostarttheimplementationinallhealthfacilitiesinCatalonia.

IntroductionTheimplementationoftheCDAstandardandthevalidationofCDA-conformantXMLinstancesisbasedontwotypesofspecifications:

1. TheCDAclassmodel,arefine-mentoftheHL7ReferenceInformationModel(RIM).TheclassmodelisexpressedinMIF(ModelInterchangeFormat),HL7’smetamodelformat.

2. Context-specificconstraints(templates)ofthegenericCDAmodel,asdefinedinaCDAimplementationguideforspecificdocumenttypeandonespecificcontext.Atthispointintimetemplatesaremostlyde-finedintextualform.AsingleCDAimplementationguidemaydefinehundredsoftemplates.

AnHL7MIFdefinitionoftheCDAclassmodelisprovidedwiththeHL7Version3standard.TheCDAMIFfilecanbetransformedintoless“rich”expressionssuchasUMLandXMLschema.PartsoftherequirementsasexpressedbytheMIFarelostduringthetransformationprocess.

CDA implementation using XML techniquesThestandardrequiresthatallCDAinstancesvalidateagainstapublishedCDAXMLschema.ThisisthemainreasonwhyalotofCDAimplemen-tationsarebasedontheCDAXMLschema.ThewideavailabilityofXMLtoolsisadefiniteadvantage;however,therearedisadvantagesaswell.TheXMLschemalanguageisnotrichenoughbyfartoexpressalloftherequirementsthatpresentintheoriginalCDAclassmodel.ACDAdocumentinstancethatvalidatesagainsttheXMLschemaisnotguar-anteedtobeavalidCDAinstance–tobeavalidCDAinstanceonehastocreateXMLthatconformstotherequirementsthatareexpressedintheCDAclassmodel.

Classgeneratorsarecommonlyusednexttootherwell-knownXMLtechniquessuchasXpathandDOM/SAX.JAXBisanexampleofaclassgenerator:atoolwhichtrans-formsXMLschematocorrespond-ingJavaclasses.

Model driven CDA implementationInordertofulfillallrequirementsasexpressedbytheCDAclassmodel,thestartingpointforallCDAimple-mentationswouldhavetobetheCDAMIF.MIF,however,hasthedisadvantagethatitisanHL7spe-cificformatthatisonlysupportedbyalimitednumberoftools.BecauseCDAisessentiallyaninformationmodelwithoutanybehavioralas-

pectsassociatedwithit,onehastheoptionofcreatingaverysolidmap-pingfromCDAMIFtoUML,whichinturnallowsfortheuseofUMLbasedtools.

TheCDAMIF(ortheUMLequiva-lentthereof)canbeusedbyclassgeneratorstocreateasetofclasses(ine.g.JavaorC#).Thereareafewfreelyavailableclassgeneratorsthatonecouldconsiderwhenimple-mentingCDA:

NineteenHL7Affiliaterepresentativesandmorethan80guestsattendedthefirstInternationalCouncilmeetingtobeheldinAustralia.Theagendawasquitepacked;themorningsessionsweredevotedtoregularbusiness,reports,informationitems,andimmediatedecisionpoints.Theafternoonsessionwasdedicatedtothe“HL7aroundtheworld”sessionandfollowedfascinatingdevelopmentsin30+countriesacrossfourcontinents.

Post WGM SurveyThirty-nineworkgroupandBoard-appointedcommit-teesattendedthemostrecentworkinggroupmeetinginAustralia;29ofthosegroupscompletedthePIC-spon-soredpostWGMsurvey.WhilePIChadhopedtoim-provetheresponseratefromBoard-appointedcommit-teesforthissurvey,theresponseratedecreasedacrossallgroups.

AllofthegroupsthatrespondedtothesurveyindicatedthattheyhadrepresentationattheMondayeveningco-chairandsteeringdivisionmeetings.Thismaybeanimprovementoverthelastmeetings,whereallbutthreegroupswererepresented.However,giventhatonly29ofthe39groupscompletedthesurvey,itisdifficulttoquantify.Approximatelyonequarterofthegroupsthatcompletedthesurveyindicatedthattheyfailedtoachievequorum.

Thestatedobjectivesportionofthesurveyisalwaysinteresting.Consistentwiththelastfewsurveys,sta-tusupdatesandinformationsharingrelatedtoexistingprojectstoppedthelistofobjectiveswith95%ofre-spondentsidentifyingthisobjective.Workandprogressreportingonexistingprojectswasaclosesecondwith90%andjointmeetingsandengagementwithotherworkgroupswereidentifiedby80%ofrespondents,followedbynetworkingat60%ofrespondents.Newprojectinitiationandengagementwithlocal/regionalprojectswereidentifiedasobjectivesby50%ofrespon-dents,andballotresolutionwasidentifiedasanobjec-tivebyonly20%ofrespondents.

Ninetypercentofrespondentsindicatedthattheyac-complishedtheirworkgroupmeetingobjectivesandbusiness.The10%thatdidnotaccomplishtheirob-jectivesidentifiedinsufficientquorum,missingkeymembersandvenuefacilitiesasthetopthreeobstacles.Technicalsupportproblemsalsopresentedasignificantbarriertoachievinggoalsandobjectives.Participationbykeymembers,pre-meetingpreparednessandsuf-ficientquorumwerethetoprankingreasonscitedbyworkgroupsasenablingtheachievementofobjectivesandgoals.

Thirteenoftherespondingworkgroupsindicatedthattheyhadattendancefromlocalprofessionals.Twelveoftherespondingworkgroupsindicatedthattheywouldrecommendusingtheconferencefacilityagain.ThosenotrecommendingthefacilitycitedproblemswithInter-netconnectivityandthecostandlackofmanagementsupportforinternationalmeetings.Severalrespondentsnotedthatscatteringthemeetingsbetweenmultiplevenueswasnotideal.Similarly,nothavingcommunalbreakfast,lunchesandbreaksinterferedwithnetwork-ingopportunities.

First-Time Attendee SurveyPICalsosponsorstheFirst-TimeAttendeeprogramateachoftheworkinggroupmeetings.ThesemeetingstypicallyoccuronSundayeveningorMondaymorning,but,givenmeetingroomconstraintsinAustralia,therewasasinglefirst-timeattendeemeetingduringMondaylunch.Approximately60first-timeattendeesparticipat-edintheSydneymeetingandthirty-sixofthemprovid-edvaluablefeedbackonourprogram.Mostnotablewasthatattendeesfeeltheywouldbenefitfromadescriptionofthevariousribboncolorsandtheirassociatedroles(i.e.,co-chairs,mentors,Boardmembers,etc.).Like-wise,manyofthefirst-timeattendeesrespondedthatmoreinformationonthetypesofworkgroups(i.e.,perhapsoverviewsofthegroupsbysteeringdivisions)wouldbebeneficial.Finally,itisnoteworthythatmostofthefirst-timeattendeescitededucationasthereasonfortheirattendance,followedbynetworking.

TheJointInitiativeonSDOGlobalHealthInformaticsStandardizationisacollaborativeinitiativetohelptheendusersofstandardsbyaddressingissuesofgapsandoverlapsacrosskeyglobalstandards:onetopic,onestandard.CDISC,CEN/TC251,GS1,HL7,IHTSDOandISO/TC215aremembersoftheJointInitiativeCouncil(JIC).In2010,theJICwaschairedbyKeesMolenaar,chairofCEN/TC2511;asofJanuary2011BronKislerfromCDISCisnowchairingtheJIC.

In2010,theJointInitiativeCouncilexpandedtosixmemberorganizations,addedanumberofworkitems,andworkedhardtobecomemoretransparentandsup-portiveofthebroaderhealthstandardscommunity.WelaunchedtheJointInitiative’swebsite2–hostedbyHL7–thatprovidesaccesstoallavailableJICdocuments:charter,policyandprocedures,workitemproposals,presentationslidesandmeetingminutes.ThewebsitealsoincludestheJICworkitemregistry,wherealljointworkitemscanbefound.TheJICbeganaprojectonau-tomaticidentificationanddatacapturestandardpatientIDandcaregiverIDaswellasaStandardsKnowledgeManagementTool(SKMT).TheJICalsodecidedtoadopttheISOworkitemBusinessrequirementsforasyntaxtoexchangestructureddoseinformationformedicinalproductsasaJointInitiativeworkitem.Otherkeyongo-ingJICprojectsinclude:theBRIDGmodel,ClinicalTrialsRegistration(CTR),IdentificationofMedicinalProducts(IDMP)andIndividualCaseSafetyReport(ICSR).

The2010successesareforemostsuccessesinleadershipcollaboration.TheJIChasachievedclosecollaboration

andcooperationbetweentheleadersoftheparticipatingSDOs;monthlyteleconferencesandhave2-3face-to-facemeetingsannu-ally.WestillhavemuchtogainincrossSDOprocedureslikesimul-taneousballotingandinfurthersupportingprojectleadstogettheirjointworkdone.Lookingforwardin2011,wewillcontinueworkinghardtoprogressintheseareas.

In2010,theJICalsostartedataskforcetoinvestigatehowwecanhelpemerginganddevelopingcountriesbyimprovingaccesstomeetingsandSDOmaterials.Incol-laborationwithSDOgloballeadership,thedonorcom-munity,andotherkeyglobalstakeholders,theJICwillcontinuetopushthisimportantworkforwardin2011.Weplantoexplorefurthertheusabilityofstandards,andeducationalopportunities,aswellasprojectsparticularlyrelevanttoemerginganddevelopingcountriessuchastuberculosisandHIV/AIDS.

SAIFdescribesaframeworkwithinwhichanorganizationcandevelopinteroperabilityspecifications.SAIFwasdesignedtobegeneralenoughtobeadaptedbymanydifferentorgani-zationstomeettheirinteroperabilityneeds.HL7isobviouslyonesuchor-ganization,andtheSAIFArchitectureProgram’sprimarygoalistoflesh

outSAIFspecificallyforusewithintheHL7organization.Weactuallyhaveatermfortakingastandardanddefininghowitshouldbeusedforaparticularusecase—it’scalleddevelopinganimplementationguide.InadditiontoballotingtheSAIFstan-dard,oneofthetopgoalsoftheSAIFArchitectureProgramistodevelopHL7’sSAIFImplementationGuide.TheSAIFImplementationGuidewillultimatelydescribehowSAIFisused

withintheHL7organization.Developingthisimplemen-tationguideisgoingtobeasecondproject,thistimesponsoredbytheTechnicalSteeringCommittee.WhytheTSC?ThereasonisthattheTSCistheonegroupinHL7thatspansallthegroupsnec-essarytodevelopanddeployalltheaspectsofSAIFwithin

HL7.ManyexistingworkgroupswillhaveinputintodevelopingtheSAIFImplementationGuide.

On the surface, the SAIF Archi-tecture Program’s purpose is to roll out, within the HL7 organi-zation, the framework standards interoperability described by the SAIF standard.

HL7’sSAIFImplementationGuide.Infact,therewillbeanumberofprojectsinvolvedindeployingSAIFwithinHL7.WewillbepilotingtheuseoftheSAIFImplementationGuidewithatleastonestandardsde-velopmentprojectandwewillneedtomanagechangestotoolingandprocessesforpublishingSAIFbasedstandardsaswellasotherprojectsthatwillbeidentifiedasweproceeddownthepathofimplementingSAIFatHL7.Thatbringsustotheword“Program”intheSAIFArchitectureProgram.Overthepastfewyears,HL7hasbeenimplementingaprojectmanagementapproachtothedevel-opmentofstandards.Thecomplexityofdevelopingstandardshascontin-uedtoevolvewithinHL7,reflectingthecomplexityoftheinteroperabilityspaceourstandardsaddress.RollingoutSAIFacrosstheHL7organizationisgoingtorequiremultipleprojects,andthoseprojectsneedtobecoor-dinated.ThatistheprimaryreasonfortheSAIFArchitectureProgram.Itwillbeusingprogrammanagementtechniquestomanagetheprojectswithintheprogram.ThisactuallymakesexplicitsomeprocesseswealreadyhavewithinHL7.Weef-fectivelyalreadyhavea“program”calledVersion2Publishingthatoverseesproductionofthevariousincrementalversionsof2.x(2.5.,2.5.1,2.6,2.7…).TherearecertainlyotherexamplesofimplicitprogramsalreadyatworkwithinHL7.OneofthegoalsofSAIFistomakeex-plicitthingswhichwerepreviouslyimplicit.Explicitidentificationof“programs”isoneeffectofmovingtoaSAIF-basedapproachtodevelopingstandards.Whatthismeansisthat

projectsassociatedwiththeSAIFPro-gramwillhaveaccountabilitybacktotheProgram,notjustaccountabilitytothesponsoringworkgroups.

NowyouareprobablywonderingwhatthismeansforthestandardsdevelopmentworkyouarecurrentlyperformingthroughHL7.ForthemajorityofexistingHL7standardsprojects,thereislittleornoimmedi-ateimpact.UnlessIhavealreadytalkedtoyourworkgroupaboutyourspecificproject,thenitisverylikelythereisnoimmediateimpactonyourproject.Inthelongterm,therewillcertainlybeanimpactonhowallHL7standardsaredeveloped,butourplanforrollingoutSAIFtothebroaderHL7organizationshouldmakethisaspainlessaspossible.

Currently,weenvisionthecreationofa“brand”called“HL7SAIFArchitect-ed”standard.Thefirststandardsun-derthisbrandarethelimitednumberofstandardsthatarepilotingtheSAIFImplementationGuideundertheSAIFArchitectureProgram.TheSAIFIm-plementationGuidewilldescribetheprocessesandartifactsnecessarytodevelopastandardcarryingthenewbrandname.Oncethepilotingstageiscompleted,wewilltransitiontothenextstagewherestandardsdevelop-mentprojectscanpetitiontojointheSAIFArchitectedBrand.TojointhebrandmeanstheprojectwillneedtodevelopthestandardaccordingtotheruleslaidoutintheSAIFImplementa-tionGuide.BecomingSAIFbrandedinthissecondphasewillbeoptional.Inthelongterm,wemayrequireallnewstandardstobedevelopedundertheSAIFbrand,followingtherules

andprocessesdescribedintheSAIFImplementationGuide.Itismyhopethatinthelongrun,theadvantagesofdevelopingastandardundertheSAIFbrandwillfaroutweighanydisad-vantages.WemayidentifyprocesseswithintheSAIFbrandwhichbringmajorbenefitswhilehavingminimalornocostforimplementinginthebroaderHL7organization.TheTSCwilllookatmovingthesesortsofbenefitsoutsideoftheSAIFbrandandintothebroaderHL7organizationmorequicklythandescribedabove.

Inconclusion,theSAIFArchitectureProgramissomethingeveryonepar-ticipatinginHL7shouldkeeptheireyeson.Theshorttermimpactonwhatyouaredoingtodayisprob-ablyminimal,butinthelongtermitwillhavesignificantimpactonhowHL7developsstandards.ThegoalisforSAIFtoprovideHL7awayofdevelopingimprovedinteroperabilitystandardsinaquickerfashion.

HL7 urges all meeting attendees to secure their hotel reservations at the HL7 Working Group Meeting Host Hotel. In order to secure the required meeting space, HL7 has a contractual obligation to fill our sleeping room block. If you make reservations at a different hotel, HL7 risks falling short on our obligation and will incur additional costs in the form of penalties.
