SUPPLEMENTAL MEDICAL INSURANCEYou can take advantage of group rates when you enroll in Critical Illness, Accident, and Hospital Indemnity Insurance through our affiliation with Aflac, a leading provider of supplemental medical insurance. You don’t need to elect Hertz medical coverage to enroll in Critical Illness or Accident Insurance.

• Critical Illness Insurance: This plan pays benefits if you or a covered family member is diagnosed with a covered critical illness.

• Accident Insurance: This plan pays benefits if you or a covered family member suffer an injury as the result of a covered accident.

• Hospital Indemnity Insurance: This plan provides financial assistance to enhance your current medical coverage to pay for many out-of-pocket expenses that major medical insurance does not cover.

GROUP LEGAL INSURANCEYou can also enroll in the Group Legal Plan through Hyatt Legal, which offers full attorney representation from network attorneys or fee reimbursement, up to a set dollar amount, with non-network attorneys for fully covered legal services.

HOW TO ENROLLEnroll in these supplemental coverage options online during your enrollment right after you make your Medical Plan election at www.hertz.com/benefitsplus.

• Current employees: When you enroll during Open Enrollment, your coverage becomes effective July 1.

• New hires: If you enroll, coverage takes effect on the first day of the month following 60 days of employment.

life change, such as getting married or divorced, or having a baby.

Accident Insurance pays a cash benefit directly to you (unless

otherwise assigned) when you or a covered family member is

injured in a covered accident—on or off the job. The benefit pays

specified amounts for medical treatment, lodging for family

members, and more (you can find a full list on BenefitsPlus).

There are no health questions or physical exams required to

purchase coverage.

THE BASICS

EligibilityYou can cover yourself, your spouse/domestic partner, and/or your dependent child(ren).

Coverage Options• Low Plan

• High Plan: Pays more for injuries and for the completion of a health screening.

Health Screening BenefitBoth plans pay you each year for any one covered health screening, such as an annual physical exam, mammogram, Pap smear, immunizations, PSA tests, ultrasounds, eye examinations, and blood screenings. The Low Plan pays $30 and the High Plan pays $60.

Your CostsYou purchase Group Accident Insurance with after-tax paycheck contributions. You can find premiums on BenefitsPlus. Your premium will not increase as you age, but could increase if you change your coverage at a later time.

HOW IT WORKS

During your benefits enrollment, you choose coverage in the Accident Insurance High Plan.

A few months later, you injure your leg in an accident covered by the plan and are taken to the hospital in an ambulance.

The emergency room doctor diagnoses a fracture and treats you.

You leave the hospital on crutches.

The Accident Insurance High Plan pays you (unless otherwise assigned) $4,265*

Critical Illness Insurance pays a lump-sum benefit directly to you

(unless otherwise assigned) if you are diagnosed with a covered

critical illness, such as cancer, heart attack, or stroke (a full list

of covered illnesses can be found on BenefitsPlus).

THE BASICS

EligibilityYou can cover yourself, your spouse/domestic partner, and/or dependent child(ren).

Coverage OptionsThe coverage available to your spouse/domestic partner and children depends on which option you select for yourself.

If you choose $15,000 in coverage (the Low Plan), the coverage limit for your spouse/domestic partner is $7,500. Each eligible dependent child can receive $7,500 in coverage at no additional cost.

If you choose $30,000 in coverage (the High Plan), the coverage limit for your spouse/domestic partner is $15,000. Each eligible dependent child can receive $15,000 in coverage at no additional cost.

Health Screening BenefitThis plan pays a maximum of $50 each year for any one covered health screening, such as a mammogram, colonoscopy, or Pap smear.

Your CostsYou purchase Group Critical Illness Insurance with after-tax paycheck contributions. You can find premiums on BenefitsPlus. Premiums are based on your tobacco usage and age when you enroll. Your premiums will increase as you get older.

HOW IT WORKS

During your benefits enrollment, you choose the $30,000 High Plan option for Critical Illness Insurance.

Several weeks later, you experience chest pains and numbness in your left arm.

You go straight to a hospital emergency room.

A physician determines you have suffered a heart attack.

Based on your coverage level, Critical Illness Insurance pays a First Occurrence Benefit of $30,000*

The Critical Illness Insurance Plan includes an additional occurrence and re-occurrence benefit that could pay a full benefit if you receive a diagnosis of another covered critical illness or have a reoccurrence of a covered critical illness with a 12-month separation between occurrences. Hospital Indemnity Insurance provides financial assistance

to enhance your current coverage if you incur major medical

expenses. It pays a cash benefit directly to you (unless

otherwise assigned). This coverage can be used for such items

as transportation and meals for family members, help with child

care, or time away from work.

THE BASICS

EligibilityYou can cover yourself, your spouse/domestic partner, and/or your dependent child(ren). You will not need to prove your good health to get coverage.

Coverage Options• Low Plan • High Plan

Plan BenefitsThese benefits are for an insured that sustains a covered accidental injury or covered sickness:

• Hospital Confinement: Payable for each day an insured is confined to a hospital as an inpatient, up to a maximum of 31 days per confinement.

• Hospital Admission: Payable when an insured is admitted to a hospital and confined as an inpatient.

• Hospital Intensive Care: Payable for each day an insured is confined in a hospital ICU, up to a maximum of 10 days per confinement.

• Intermediate Intensive Care Step-down Unit: Payable for each day an insured is confined in an Intermediate Intensive Care Step-down Unit, up to a maximum of 10 days per confinement.

HOW IT WORKS

During your benefits enrollment, you elect the High Plan of the Hospital Indemnity program.

A few months later, you have a very high fever that isn’t breaking and you go to an emergency room for a diagnosis.

You are admitted to the hospital.

You have fully recovered and are released after two days.

Based on your coverage, the Hospital Indemnity Plan pays you: $1,400*

* Amount payable was generated based on the High Plan benefit amounts for Hospital Admission ($1,000), and Hospital Confinement ($200 per day).

The Group Legal Plan, offered through Hyatt Legal Plans,

provides you and your eligible family members with fully covered

legal services from a national network of attorneys.

THE BASICSCovered services include:

• Consultations on the telephone or in person

• Court appearances

• Document review and preparation

• Financial matters including debt collection defense, tax audits, identity management services, and personal bankruptcies

• Traffic ticket defense (no DUI)

• Family law including adoptions, prenuptial agreements, divorce, domestic violence protection, guardianship, and name change

• Real estate matters including boundary and title work, tenant issues, and home equity loan, sale, purchase, and refinance

• Estate planning including wills, power of attorney documents, living wills, and trusts

HOW IT WORKSYou can enroll for coverage under the Group Legal Plan when you first become eligible or during Open Enrollment. Your premiums are deducted from your pay on an after-tax basis. When you need legal help, you can choose from:

• Plan attorneys: Select one of more than 14,000 experienced attorneys from the Hyatt Legal Plans nationwide network. You won’t pay for covered services and there are no limits on usage.

• Non-plan attorneys: You’ll be reimbursed for covered services according to a set fee schedule.
