DEPARTMENT INFORMATION

The E-App is optimized for use on computer-based web browsers, iPad and Android tablets. It is NOT optimized for use on smartphones.

A valid email address is required for electronic signatures. If the client does not have an email address, the producer’s email address may be used.

Investors Heritage will not receive the application until all required signatures have been captured and the application is successfully submitted by the producer.

Applications are kept and available to view and print on the Firelight platform for 120 days.

Transfers:

a. Some companies require their own paperwork be completed for a transfer. If this is the case, the IH transfer form is not required. On page 1 of the application, choose “Agent or Applicant to originate” under the 1035 Exchange/Rollover/Transfer option.

b. Some companies may require “wet” or “medallion singatures”. If this is the case choose, “IH to originate” on page 1 of the application and “Carrier Initiated” on the Funding Page that follows the application. On the signature page of the transfer form, check the box that asks if a wet signature is required. Checking this box will disable the transfer form from the signing ceremony. Transfers requiring wet or medallion signatures must be mailed to our New Business address. IH will not be able to initiate the transfer until the original transfer form is received.

You will be taken to the E-Application site hosted by Firelight.

Click Application under Start New. 4

5 Click Select Jurisdiction down arrow and choose State of Sale from the list.

The state appropriate application and other required forms will display below. You can choose to add the Annuity Distribution Request to your package if needed. The Annuity Distribution Request Form should be used if you wish to set up reoccurring interest payments beginning 31 days from the issue date or reoccurring RMD payments beginning 6 months from the issue date. Please note reoccurring RMD payments are not currently available for Inherited IRAs.

NAVIGATING THE SYSTEM

The e-application is designed to be easy to use and to ensure in good order application submission.

The application view is the same as the paper format of the application.

Complete fields just as you would for a paper application.

Required fields are highlighted in red. Required fields will change according to choices made on the application. (If you choose “yes” to “Will this policy have a Joint Owner”, the Joint Owner fields become required.)

To assist you, some fields will auto-populate with information provided in previous fields. For example, if you choose Joint Owner, the Joint Owner’s Mailing Address will auto-fill with the information provided for the Owner/Annuitant. This can be changed if they have different addresses.

The system will auto format numbers as required - Social Security Numbers, DOB’s, Phone Numbers and dollar amounts should all be entered without any dashes or other symbols.

Applications can be saved by clicking Save in the top menu. 1

Click Open, just below Data Entry progress bar, to view a list of all forms to be completed. Any form that is missing required data will be listed in red.

6 Additional documents can be uploaded and submitted with the application package (ex. copy of government issued ID). Selet Other Actions from top menu. Then choose Documents.

Additional Document Upload. The platform provides sever Document Type choices: Illustration, Medical Exam Part 2, Annuity Intelligence Report and Other. Choose OTHER for ALL Heritage Builder uploads. Click Choose File, locate and select the file from your computer. Click Upload. All uploads must be in PDF format and no larger than 20 MB.

To prevent NIGO submissions, signatures are not allowed until all forms are 100% complete.

When all required fields are complete, the Data Entry Requirements message will appear. Choose CONTINUE to proceed to the signature fields.

You must verify the identity of the applicant and enter the required information. 4

5Each document in the list must be reviewed with the client. Click a document, review with client and then, with their consent, mark “I have reviewed and agree with the terms expressed within this document” box at the bottom of the list. Continue for each document.

6 Complete the Signer’s Full Name and City in the appropriate fields. The name will auto-populate in the signature field.

If the name needs to be changed, the signer must use the mouse to “write” their signature. Click I consent. Then repeat process for all other required signers.

After collecting all required signatures, click Continue in top right corner. 1

From the top menu, choose Other Actions. Choose Display/Print PDF from drop-down menu.1
