Reducing Hospital Re-admissions by Improving Home and Community Support

Background:Tan Tock Seng Hospital (TTSH) has paved the way with several initiatives to prevent re-admissions. One of the most recent is the "Virtual Hospital" programme which is a new model of care for patients at high risk of re-admissions.

It provides medical and social support for patients who are fi t to resume daily activities, but require close monitoring for chronic conditions after they are discharged. These patients are usually elderly and do not have strong social support at home. Very often, they end up at the Emergency Department for treatment that, in most cases, can be managed at home.

TTSH started the initiative in August 2012 and enrolled 80 patients into the programme. As a result of the programme, three in 10 of them do not need to be re-admitted to hospital.

Dr. Benjamin Ng is currently the Director of Clinical Standards and Improvement at TTSH. He is responsible for all clinical quality improvement and patient safety matters at the hospital. Dr. Ng is an advocate of service excellence in the provision of care to patients. In recognition of his efforts, he was awarded the Raffl es Medical Group’s (RMG) 1st and 2nd service star awards in October 2006 and October 2008.

Dr Ng is an occupational medicine and aviation medicine physician by training. He is the Associate Programme Director (APD) of the national Preventive Medicine Residency of the National University Health System (NUHS) for TTSH. He is also the APD of the national Occupational Medicine Senior Residency of NUHS. He is a visiting lecturer for the Master of Public Health programme of the Saw Swee Hock School of Public Health, National University of Singapore.

WORKSHOP B (1.00pm - 3.30pm)

Improving Processes to Shorten Treatment Time for High Acuity Patients (With a Special Focus on Stroke Care)

Background:Clot-dissolving “thrombolysis” treatment for stroke is time critical. An enhanced “code stroke” process was initiated at the Royal Melbourne Hospital to reduce treatment delays and improve patient outcomes. Key elements of similar systems including the Helsinki stroke protocol were analyzed to identify those suitable for translation to the Australian context.

Ambulance pre-notifi cation was enhanced and a “direct-to-CT scan” process introduced to bypass delays in the emergency department. The system lead to major reductions in treatment delay and is being expanded to other Australian centers.

Dr. Bruce Campbell Neurologist and Research Fellow, Royal Melbourne Hospital (RMH), Australia

Dr. Bruce Campbell is a stroke neurologist in the Department of Neurology, RMH and an NHMRC post-doctoral research fellow at the University of Melbourne.

Most notably, Dr. Campbell was the senior author of the Neurology paper documenting dramatic improvements in stroke treatment times through a re-design of pre-hospital and emergency department care delivery at RMH. This successful investigation has set an internationally competitive benchmark for the treatment of stroke patients.

Dr. Campbell was also awarded the prestigious Leonard Cox Award by the Australian and New Zealand Association of Neurologists (ANZAN) in May 2013.

Improving Bedside Care through Nurse Management and Empowerment (With a Special Focus on Elderly Care)

Background:Hong Kong has an ageing population and its hospitals are also impacted directly with more elderly patients needingtreatment for old-age diseases.

According to the Census and Statistics Department (C&SD) of Hong Kong, the ageing trend in the country will rise steadily over the next 50 years:

• Proportion of the population aged 65 and over is projected to rise from 13% in 2011 to 30% in 2041• Pace of population ageing projected to accelerate from 19% in 2021 to 26% in 2031• Proportion of the population aged under 15 projected to decrease from 12% in 2011 to 9% in 2041 due to low birth rates

Workshop Focus:• Discussing ways to treat elderly patients physically, cognitively and socially in hospitals• Sharing strategies on ward environment and bed management for the elderly patients• Looking at least restraint care as a multi- disciplinary approach and sharing strategies to reduce restraints on elderly patients in order to speed up their recovery• Sharing how the hospital as a whole can help nurses to perform better care for the elderly

Ms Cecilia Chan Nim-chi is the General Manager (Nursing) of the Ruttonjee & Tang Shiu Kin Hospitals in Hong Kong. She received her nursing education in the Nethersole School of Nursing in Hong Kong. She qualifi ed as a Nurse Educator at the Hong Kong Polytechnic University and holds a Master degree in Health Administration from the University of New South Wales.

She holds honorary appointment as Assistant Professor at the School of Nursing in the University of Hong Kong and Adjunct Associate Professor at the Chinese University of Hong Kong.

She has previous working experience as Nurse Educator in the Nethersole School of Nursing and as Department Operations Manager of the Department of Medicine and Geriatrics in the United Christian Hospital.

Ensuring Effi cient Patient Throughput in the Emergency Department

Background:Changi General Hospital’s Accident and Emergency Department is the busiest in Singapore. Its A&E department treats 400 - 500 patients a day. By making effi cient patient throughput in the ED a strategic goal for the organization, CGH has managed to treat A&E patients faster and successfully reduced the number of A&E patients warded.

As early as 2011, CGH built a 9-bed short stay unit and expanded its observation ward from 12 – 20 beds to better manage patient fl ow. It also built a new fever facility and expanded its waiting area in the A&E department.

Amongst the initiatives, • They implemented a 4-hour inpatient discipline clerking rule for patients to be treated within 4 hours. • They re-engineered their inpatient discharge process, increasing the % of patients discharged within 4 hours from 40% to 80%.• They created a patient-friendly Emergency Department: Service ambassador, ED journey map, ED admissions video, ED “lifeline” and get well cards

He has more than eight years in quality improvement experience, through supporting and facilitating lean, routine to research (R2R) methodology, utilization management and quality-related training in statistical process control charts (SPCC), and health economic and decision model analysis. He was also assigned as Asian Productivity Organization (APO) Technical Expert Service (TES) on Lean management in Hospitals and Healthcare Systems, in Oct 2011.

He is a course coordinator Health economic analysis for the Master of Science (M.Sc.) in Clinical Epidemiology Thai Cert. He received his training in medicine and Diplomat Thai-Board of Respiratory Medicine (2000) at Siriraj Hospital, Mahidol University, Bangkok Thailand and Research and clinical Fellow in Respirology at Toronto General Hospital, Ontario Canada (2001-2005).
