Heart

The Heart Plan is our renewed effort to assist our employees to live healthier lives.

It is open to all employees, not just the ones who enroll in our medical plans.

We asked for volunteers from all our locations to be leaders for the healthy living cause.

These folks have organized themselves into area office groups and have nominated Reps and Assistant Reps to help you know where our plan’s tools are located, how to access them and to help you stay motivated and rewarded for healthy behaviors.

Annual Physical * $20 / per month (paid quarterly)* All covered dependents must also meet qualification criteriain order to earn the financial incentives

HEART Plan Financial Incentives

Tobacco Free Incentive

Effective June 1, 2020, Flintco, LLC instituted an incentive for those employees who do not use tobacco products or who are undergoing a cessation program during the plan year. To earn the incentive, employees must not use tobacco products, or employees who do must enroll in a tobacco cessation program through Blue Cross Blue Shield of Oklahoma or any other accredited program by Sep 1, 2020. If you’re unsure what constitutes an accredited program, please contact your Area HEART Plan Rep or Jill Lingle at 918.710.2164 – [email protected]. Enrollment in the HEART Plan for the Tobacco Free Incentive will be processed on the Employee Navigator site.

I understand that I am required to acknowledge that:I and/or my dependents (including covered spouse/domestic partner and/or children) do not use tobacco products. I also certify that I and/or my dependents have not used any tobacco products in the last three months including, but not limited to, pipes, cigarettes, cigars, chewing tobacco, snuff, or any other type of smoking, vaping, or smokeless tobacco (i.e., one usage of any tobacco product in the last six months is tobacco use). I certify that, if this information changes in the next 3 months, I will notify Human Resources. By indicating and attesting to my non-tobacco user status, I am requesting the monthly incentive of $40 paid at the end of each quarter.

HEART Plan Financial Incentives

Annual Physical Incentive

Effective June 1, 2020, Flintco, LLC instituted an incentive for those employees who have an annual physical that includes measurements of height, weight, BMI, Cholesterol, Glucose and Blood Pressure.In addition, all covered dependents must also have a similar annual wellness check in order for the employee to qualify for the incentive.

Enrollment in the HEART Plan for the Annual Physical incentive will be processed on the Employee Navigator site.

I understand that I am encouraged to establish / resume / maintain regular contact w/my primary care physician

Wellness examinations from your personal physician must be completed between 6/1/2019 and 5/31/2021 to be eligible for the incentive of $20 per month (paid at the end of each quarter after this form has been received by HR) for the 2020/2021 plan year. The incentive will apply to the portion of the plan year after the annual physical has been completed. Questions can be directed to your Area HEART Plan Rep or Human Resources. By indicating and attesting that I have had my annual physical, I am requesting the monthly incentive of $20 paid at the end of each quarter.

Softball League• According to a survey conducted in OKC, a majority of our personnel indicated that they

would be willing to dedicate time once a week to play softball in a local league.Local Races• The activity that generated most interest would be monitoring local races that come to town

and setting up teams for those events. • These would include various 5k’s, spartan race teams, and relays.General Health and Wellness• There was a huge interest in gym memberships and tracking activities via iWatches, fitbits,

and other activity monitoring technology.• Ideas on incentivizing activities relative to the capabilities of personnel are also being thrown
