Annual Report Fiscal Year 2013Welcome to the LCHD/CHC fiscal year 2013 annual report. Several transitions in leadership took place this year. The Department now has a new Executive Director, Board of Health President, and Governing Council Chair. Congratulations are extended to outgoing Board of Health President Michael Waxman, outgoing Governing Council Chair Eula Crawford and former Executive Director Irene Pierce who all brought the Health Department to new levels of achievement under their leadership.

Our 2013 accomplishments primarily fall under three themes: collaborating with local partners, meeting residents’ needs and improving program efficiency. This year’s annual report is structured in accordance with these categories. One of our key accomplishments this year was the completion of our 2013-2016 Strategic Plan. This plan focuses on what our organization will do towards addressing priority issues that were articulated for Lake County through a health improvement planning process. The process placed an emphasis on a community-driven, community-owned approach in which the community takes responsibility for its own health. Over the next few years we will be working with multiple community partners toward the goal to achieve the highest level of health for all in Lake County. The Affordable Care Act made it possible for the Health Department to make significant progress in increasing access to care for numerous Lake County residents. We received more than $1 million in grants to spearhead a countywide initiative, called Enroll Lake County!, which brought together 27 community-based organizations, faith and civic groups, library networks, schools, hospitals and primary care providers. Within the first few months of the program, navigators from these organizations educated and enrolled thousands of residents into healthcare coverage. The initiative will continue through 2014.

We made significant progress toward helping residents with severe mental illness. Our Williams Consent Decree program in particular has made it possible for patients to live more independently in the community and be engaged in skills training and other activities with the ongoing support of mental health professionals. While you may not always see what we are doing, we are constantly working behind the scenes to make Lake County healthier. Our communicable disease program staff investigated 37 illness outbreaks this year, including outbreaks of norovirus, influenza, pertussis, salmonella, cyclospora, chicken pox and scabies. As a preventive measure, staff provided disease prevention information to the facilities where the occurrences took place.

We are proud to be among the top 6 percent of 2,532 local health departments defined by size of population served, financing and workforce. We are especially grateful to the Lake County Board for its continued leadership and support.

Collaborating With Local PartnersHelping Thousands Receive Health InsuranceThousands of uninsured residents in Lake County received help with signing up for health insurance coverage through the Affordable Care Act as the result of several grants

agencies. Certified navigators with these agencies provided outreach, education and enrollment assistance to residents throughout the county. The initiative will continue through 2014.

Reaching High-Risk Clients FasterThe Health Department’s Behavioral Health Services is partnering with Rosalind Franklin University of Medicine and Science to improve services to individuals involved in the criminal justice system. Through the partnership, two Rosalind Franklin University psychiatric medical residents are providing psychiatric services to the Department’s

traditional clinic population, as well as to individuals who are part of Lake County Probation and the Lake County Jail. This enables seriously mentally ill clients, who are being discharged from jail, to see a psychiatrist faster, with the goal to help stabilize mental health symptoms that often lead people to re-offend and return to jail.

Preparing for Public Health DisastersThis year, the Emergency Management program focused on developing public/private partnerships to expand the number of private sites that could be used for dispensing medications in the event of an emergency. Two local colleges and two large businesses agreed to participate. In its continuing efforts to plan and prepare for public health disasters, the program held exercises to test its mass vaccination plan, mass medical distribution plan, and demobilization plan. Additional exercises focused on building collaboration between the health department, regional hospitals and other partners.

Investigating Infectious Disease OutbreaksDuring 2013, the Communicable Disease Program investigated 37 different outbreaks of illness affecting Lake County residents. Norovirus, influenza, pertussis (whooping cough), salmonella, cyclospora, scabies and chickenpox were among the outbreaks investigated that primarily occurred in schools, day-care centers and long-term care facilities. To help share information regarding communicable disease control, the program formed an Infectious Disease Group consisting of all Lake County infection control preventionists. The program regularly shares communicable disease updates, state regulation changes and other information with the group.

Encouraging Regular Testing of Well WaterTownships and municipalities with large populations that rely on private wells for drinking water are partnering with the Health Department as a way to offer a convenience to their residents. The health department provided water sample kits to these communities so that well water owners no longer need to drive the distance to the Environmental Laboratory in Libertyville to pick them up. When flooding occurred last spring,

the accessibility of the water sampling kits came in handy for people with submerged wells. Staff analyzed more than 279 water samples in which 48 tested positive for contamination. Participating communities include Antioch, Fremont, Grant, and Lake Villa townships as well as the villages of Deer Park, Fox Lake, Lindenhurst, Long Grove and Wadsworth.

Meeting Residents’ Needs

Receiving Recognition for Leadership in Maternal and Child HealthThe Health Department received the annual Jonas Salk Health Leadership Award for Public and Community Health from the March of Dimes Illinois Chapter. The award recognizes exemplary leadership in maternal and infant health. Maternal/Child Health (MCH) services are a key part of LCHD/CHC’s operations and have been since the early days of the organization. Several of the Maternal Child Health Programs offered, including Family Case Management, WIC, Prenatal Care and Title X Family Planning, have been in place since the 1970s. Together, these programs have served tens of

thousands of Lake County women and their families with high quality services.

Teaching Residents to Fight the BiteThe Communicable Disease, Ecological Services and Communications programs joined together to launch a grant-funded public education campaign called “Fight the Bite.” The campaign’s purpose was to raise local awareness about ticks carrying Lyme disease and mosquitoes carrying West Nile Virus. The campaign, which reached people countywide through ads on billboards, buses and gas station pump toppers, was particularly prompted by the recent increase in Lyme disease cases in the county.

Helping Mentally Ill Patients Live More Independently Due to high demand, a drop-in center established to support individuals with mental illness went from offering weekday hours to being open seven days a week. The center is part of the Williams Consent Decree program that is helping individuals living in state-funded facilities to move into their own apartments and live more independently under the supervision of mental health professionals. Approximately 40 to 50 clients use the

center on a daily basis to socialize, learn new skills and touch base with their counselors. In 2013, 19 enrolled in College of Lake County classes, four were employed in the community, and numerous were involved in volunteer activities.

Helping Childcare Providers Learn From Each OtherHome childcare providers often spend their days surrounded by children in their homes with minimal adult interaction or contact. The

Childcare Nurse Consultant, who works with more than 800 childcare providers in Lake County, organized a monthly meeting where these providers can share their knowledge and challenges and learn from each other, strengthening the capacity of our childcare community.

Using Stronger Rules for Aggressive DogsThe Lake County Board and Board of Health approved revisions to Article X, the Animal Care and Control ordinance. The most significant change was to grant the authority to investigate and impound dogs that exhibit animal-aggressive behavior. An animal-aggressive dog is a dog that, without justification, causes serious physical injury or death to another owned animal. Animal Care and Control conducted four investigations in 2013. Three of the four dogs investigated were determined to be animal-aggressive.

Receiving Local Recognition for InnovationThe Lake County Underage Drinking and Drug Prevention Task Force collected 8,200 pounds of prescription drugs from 13 disposal boxes in Lake County during 2013. This is among the reasons the Lake County Municipal League recognized the Task Force as the most innovative program in the county and awarded it the Cutting Edge Award.

Advocating for More Smoke-free Ordinances Advocacy efforts by the REALITY Illinois youth group resulted in the Lake County Forest Preserve District enacting a smoke-free playground ordinance at nine forest preserve locations. Participants of this youth group are from a variety of Lake County high schools. They work with the Health Department’s Tobacco Free Lake County program to educate and advocate for tobacco-free choices.

Addressing Harmful Algal BloomsAlgal blooms, the rapid increase of algae in an aquatic system, plague several lakes in Lake County. They can have harmful effects on aquatic ecosystems and on people. Ecological Services conducted a pilot program with the Illinois Environmental Protection Agency that involved testing the water of several lakes for toxins from potentially harmful algal blooms. The project helped to proactively identify potentially harmful conditions and

staff worked with lake users to reduce exposures.

Engaging Mentally Ill Patients Counselors of the Community Integrated Living Arrangement are keeping their clients active and engaged. Their task is to improve the quality of life of mentally ill individuals and help them to be fully contributing members of society who pay rent, buy groceries, and seek employment. The counselors create daily calendars of activities for their clients, including card games, board games, walking clubs, movies, sporting events, and opportunities for socialization. At the same time they provide counseling services, medication compliance monitoring, case management and life skills training to help them become as independent and self-sufficient as possible.

Responding to Children Affected by TraumaIn response to a growing need for mental health services for children and adolescents, the Health Department recently launched a Trauma Treatment Program for Lake County residents between the ages of 2 and 18. The program teaches skills to children and their families that help them cope with the trauma they experienced and the symptoms that often start following the experience of trauma. Response to the program has been quite strong. In 2013, the program performed 229 trauma screenings and engaged 127 clients and their families in trauma therapy. The program also provided trainings on trauma to 758 people and formed

collaborations with 17 local organizations.

Receiving Local Recognition for Increased Vaccination RatesThe Immunizations Program Coordinator received the Immunizations Advocate Award from the Chicago Area Immunization Campaign, a coalition of more than 100 Chicago area public and private sector organizations. Nominated by her peers, she was selected based on her

commitment to increasing vaccination rates in Lake County and the Chicago area. The Immunizations Program provides routine and seasonal immunizations to adults and children. In 2013 the program administered more than 25,000 vaccinations of which 24,000 were given to children 18 years of age or younger.

Improving Program EfficiencySwitching to Electronic Dental Records The Dental Program made the switch to electronic dental records during three phases in 2013. Electronic dental records help improve care, quality and patient safety by enhancing both the quantity and quality of information available to providers for decision making. During patient visits, providers have access to patient prescription histories linked with their medication management and their digital X-rays. The Dental Program’s switch to electronic records follows Primary Care’s switch, which took place in 2012. Electronic health records are expected to be fully implemented throughout the Health Department in 2014.

Becoming a Patient-Centered Medical HomeThe Midlakes Health Center in Round Lake Beach became certified as a Patient-Centered Medical Home by the Joint Commission. Patient-centered medical homes improve health outcomes, contain costs and improve patient experiences in several ways. They provide team-based care to meet patient needs and engage patients and families in a collaborative process designed to manage medical, social and mental health conditions more effectively. The Health Department is now working toward transforming all of its health centers into patient centered medical homes.

Addressing Animal Cruelty and NeglectFor the first time, Animal Care and Control staff provided training for police officers to help them identify signs of animal cruelty and neglect. This training also helped them recognize links between animal and human abuse. Round Lake Beach and Vernon Hills police departments sent a total of 49 officers to attend these workshops, which also provided information about local and state laws specific to the humane care and treatment of animals.

Controlling Compost and Landfill Odors The combined efforts of Park City and Waukegan, and the enforcement actions of the Solid Waste program temporarily eliminated significant odors from a compost facility in Waukegan near Park City’s border. The group is now developing a long-term solution to the problem. The Solid Waste program regulates 10 compost facilities along

with two active landfills, 10 landscape waste transfer stations and 22 closed landfills. The program has been working with managers of active landfills to prevent odors as well as to address odor problems as soon as they are reported.

Using Skype to Assist TB PatientsThe Illinois Department of Public Health recommends that health care providers use interventions such as direct observe therapy (DOT) to assure successful completion of Tuberculosis (TB) treatment. Staff at the TB Program observes pulmonary TB patients taking their medications on a daily basis to ensure they are appropriately completing their treatment and that the disease is not transmissible to others. This can be taxing on both staff and the patients. This year, program staff began using Skype to observe the medication routine of one patient. This gave more flexibility to the patient and more time for staff to focus on clinic patients. The program plans to offer the Skype approach to more patients in the future.

Increasing Outreach to HIV-Positive IndividualsSexually Transmitted Infection (STI) Program staff strengthened their outreach to HIV-positive individuals residing in Lake County. Staff worked to identify individuals living with HIV who have never been in HIV medical care or who have been out of care for more than 12 months to link them with needed services. Linkage to care and early initiation of HIV antiretroviral medication not only supports the health of HIV-positive individuals, but also lowers the amount of virus in the individual’s body. This model of care has proven to reduce the transmission of HIV and therefore plays an important role in preventing new HIV infections.

Improving the Regulation of Onsite Wastewater Treatment Systems For the first time in more than 16 years, the Lake County Board of Health and County Board approved a revision to the ordinance that regulates the design, installation and maintenance of septic systems in Lake County. The revision clarifies a number of on-site wastewater treatment system requirements, provides a clear hierarchy of the strategies to repair failing systems when compliance is impossible, and expands and advances the types of systems and technologies that can be used.

Receiving National Recognition for Program IntegrationThe Be Well - Lake County program, a diabetes prevention and treatment program that the Health Department coordinates in partnership with NorthShore University Health System, received national recognition. Duke University and the Centers for Disease Control and Prevention selected the program for inclusion in Public Health and Primary Care Together: A Practical Playbook, an interactive web-based resource that provides examples of programs throughout the nation that have successfully integrated primary care and public health to improve population health.

Enhancing Food InspectionsThe Food Program has been involved in a two-year project with the Northern Illinois Public Health Consortium to increase the consistency of regulations and policies in the current climate of constantly changing food processes. Participants are helping to enhance and standardize inspection programs in the region. They are developing consistent regulations and policies not only for restaurants, but for increasingly common venues such as outdoor food events and farmers’ markets.

Tuberculosis Program

Tuberculosis (TB) is not a thing of the past. The Health Department’s Tuberculosis Clinic treats an average of 11

active TB cases each year. Because the disease is so contagious, staff members need to observe their patients taking their medication every day to ensure they are appropriately completing their treatment. This can be taxing on both the staff and the patients who need to be home at designated times. But a creative solution has worked well with one patient, and will be used in the future with more.

The patient, whom we will call Jane, recently immigrated to the United States. She was adjusting to life in America, when she suddenly became ill with a cough, fever, headache and chills. After receiving numerous tests and spending three weeks in a hospital, she was diagnosed with TB.

TB is a contagious and often severe airborne disease caused by a bacterial infection. Patients can be cured through a daily dose of antibiotics taken for a

six- to nine-month period. For her treatment, Jane was referred to the Health Department’s TB clinic, which is responsible for the prevention and control of TB in Lake County.

As they do with all of their pulmonary TB patients, TB clinic staff began visiting Jane on a daily basis. But after a month, they started a new approach. Instead of visiting her in person to observe her taking her medication, they began to Skype, a free voice and video internet service. This gave more flexibility to Jane and allowed staff to allocate more time to clinic patients. Jane was even able to Skype using her phone when she briefly went out of the country for a family emergency.

“Health Department staff has been very caring,” said Jane. “They have helped me in many ways.”

Jane is now more than half way through her treatment and is no longer contagious. She hopes to become a waitress or store clerk when she is completely cured.

William Paolella has a favorite destination in his life. On a daily basis he walks to the Recovery Center in Waukegan, where he can see his friends and engage in social activities.

He has such a sunny disposition, it is hard to imagine that in a dark period of his life, Paolella attempted suicide several times and underwent psychiatric hospitalization. Like many clients of the Williams Consent Decree program, he lived for numerous years in a mental health institution. His daily life was structured; he followed a strict schedule from taking showers to eating meals to taking medications. The confinement and lack of privacy caused him to continually dream of escape.

After a decade of living in this controlled environment, Paolella learned about the Williams Consent Decree program at the health department. The program provides individuals living in state-funded mental health institutions a chance to live independently with professional support. Clients receive daily case management to help reintegrate them into the community and improve their independent living skills.

With the help of program staff, Paolella moved into an apartment and began visiting the program’s Recovery Center on a daily basis. Consumer run, the Center is designed to provide a nonclinical environment for individuals to socialize, receive support from their peers and access computers.

“This program has really helped me become more independent again,” said Paolella. “At the Center I can spend time with my friends, check in every day with a counselor, and learn new skills. I have something to look forward to every day.”

Paolella says that the program has helped him regain his positive outlook on life. He now volunteers twice a week at the Captain James Lovell Federal Health Care Center, where he focuses on helping others.

Carmella Albano’s story of how she came to enroll in the Affordable Care Act’s health insurance Marketplace is not unusual.

The Lindenhurst resident spent the first part of her work life as an accountant and then became a math teacher. She worked for 12 years in Chicago Public Schools until her school was closed. Her last full-time job was as a substitute teacher for a year.

Statistics

The continuation of her employer health insurance (COBRA) ended in March 2013. Because of a pre-existing condition, Albano could not get coverage from a private health insurance provider so she used her savings to obtain health insurance through the Illinois Comprehensive Health Insurance Plan (Illinois CHIP).

When the online health insurance marketplace became available in October, she submitted an application on her own. In December, when she still had not heard anything about her coverage, she contacted the Lake County Health Department and scheduled an appointment with Enroll Lake County! navigator Linda Santos.

After meeting with Linda, Albano realized she should have called the Health Department sooner. She had to pay a 70 percent increase in ICHIP from December to January – which ended up being $1,100 for one month of coverage.

As Albano said, with the navigator’s help, “I found a policy that works for me. It’s not a Cadillac policy, but it’s a Chevy which is okay,” she said, adding that the policy is much better than the ICHIP policy she had. The navigator, she said, was very caring about her situation.

She is now paying $200 a month for her health insurance through the Affordable Care Act, and she said people should stop listening to the negative comments made in the media about the health insurance marketplace. Instead, Albano said people should call their local health department if they have questions and sit down with a navigator.

As Albano said, “They walk you through the application. They show you the options and, in the end, you still have a choice.”
