In particular for those from marginalised groups or seldom heard communities within the borough. National evidence suggests that public bodies and services need to do more to take protected characteristics within communities into account when developing services. People from BME communities report numerous issues with access to health services. Barriers include dissatisfaction with mainstream services which they perceive as lacking in understanding and consideration. This situation can result in poorer health compared to other groups, with unnecessary visits to Accident and Emergency, higher rates of hospital admission, and the likelihood of more complex, intrusive interventions. In December 2015 NHS England outlined a new approach to help ensure that health andcare services are built around the needs of local populations To do this every health andcare system in England has had to produce a multi‐year Sustainability and TransformationPlan (STP) showing how local services will evolve and become sustainable over the next fiveyears ndash ultimately delivering the Five Year Forward View vision of better health betterpatient care and sustainability of the NHS

The STP process is important because it requires health and social care organisations to plantogether to make sure services and resources are coordinated to deliver the best possiblecare now and in years to come To deliver these plans local health and care systems cametogether in January 2016 to form 44 STP lsquofootprintsrsquo

In South East London the STP footprint comprises the Clinical Commissioning Groups andLocal Authorities in the boroughs of Bexley Bromley Greenwich Lambeth Lewisham andSouthwark and the providers located within it ie hospitals community health services andmental health trusts The final STP will influence and shape commissioning intentions goingforward until March 2021
