Dear Friends,

At United Way of Delaware (UWD), we believe:

all Delawareans deserve access to quality healthcare and the resources necessary to live independently.

And:

For a community to have access to quality healthcare, there must be a sustainable, statewide public-private coalition to reinforce this mission.

Healthy Delawareans Today & Tomorrow, a coalition convened by United Way of Delaware and supported by AstraZeneca, is helping to make a difference in the health of our community.

This group of government agencies, nonprofit organizations and healthcare institutions works to increase awareness of and access to free and reduced-cost healthcare services for those in need. since its inception in 2007, Healthy Delawareans Today & Tomorrow has connected more than 38,000 Delawareans to healthcare resources, including:

• Freeorlow-costhealthcareservices • Prescriptionmedicationassistance • Patientnavigationservices • Transportationtoandfrommedicalappointments We hope that this guide helps you and your loved ones find healthcare providers near you that provide the services listed above and much more.

if you need additional guidance or have questions, please call the Delaware Helpline by dialing 2-1-1 and speak with a referral specialist.

on behalf of UWD and our partners, we wish you and your family good health!

sincerely,

Michelle A. Taylor
