Setting the scene - our challenge• A long term condition (LTC) is a condition that
cannot, at present, be cured but is controlled by medication and/or other treatment/therapies
• In total around 70% of the total health and care spend in England (£7 out of every £10) is attributed to caring for people with LTCs
• The cost and prevalence continues to grow
