1.0 INTRODUCTION
CHA was retained by the City of Troy, N.Y. to complete a hazardous building material survey of the
former Leonard Hospital located at 74 New Turnpike Road in Troy, New York. The building, which
has been vacant since 2001-2002, has been heavily vandalized and is in deteriorating condition. Due
to the current building conditions, this building-wide survey has been completed prior to the
development of any renovation/demolition plans.
This survey report consists of five sections, including this Introduction. Section 2.0 presents the
Scope of Work, which outlines the parameters by which this project was performed and the
limitations that were encountered. Section 3.0 – Hazardous Materials Survey, details the findings of
the building inspection and suspect materials identified. Section 4.0 presents a brief overview of the
inspection results while Section 5.0 presents CHA’s conclusions and recommendations regarding the
status of the building that was inspected.
2.0 SCOPE OF WORK
The purpose of this survey is to inspect all accessible interior and exterior spaces throughout the
building including basements, penthouse levels, and roof levels to identify asbestos-containing
materials, lead-based paints, and PCB-containing fluorescent light ballasts and associated mercury-
containing light tubes prior to any renovations which may disturb them. In addition to the hazardous
material survey scope items mentioned above, CHA also inventoried various equipment and
materials for possible salvage.
2.1 ASBESTOS-CONTAINING MATERIALS SURVEY
CHA provided a New York State Department of Labor-certified asbestos building inspector
responsible for the identification and assessment of the condition, location and quantity of suspect
asbestos-containing materials (ACMs) associated with the subject building. The ACM survey was
completed per the United States Environmental Protection Agency’s (USEPA) National Emissions
Standard for Hazardous Air Pollutants (NESHAPS) Regulation 61.145 which states that “…the
owner or operator of a demolition or renovation activity, prior to the commencement of such activity
shall thoroughly inspect the affected facility for the presence of asbestos…”. The survey was also
completed per the requirements outlined in Subpart 56-5 of New York Code Rule 56.
The inspector collected bulk samples of identified suspect asbestos-containing materials following

the protocols referenced in 40 CFR Part 763.86 Subpart E (AHERA). All bulk samples were
analyzed at an accredited laboratory licensed by the New York State Department of Health
(NYSDOH) Environmental Laboratory Program (ELAP) as well as the National Voluntary
Laboratory Accreditation Program (NVLAP) for the analysis of asbestos bulk samples.
2.2 LEAD PAINT SURVEY
A lead paint survey was conducted to identify Lead-Based Paint (LBP) surfaces and/or sub layers
throughout the building. Paint chip samples were collected at random locations along the interior \
of the building and the samples were analyzed for percent lead content using Atomic Absorption
Spectroscopy (AAS). Analysis was performed using ASTM Method D3335 for lead in paint.
It should be noted that the LBP survey is intended to assess the need for the safe and proper
management of painted surfaces during the renovations throughout the building. The survey and the
resultant sample data are not meant to assess current or future building occupant exposure or to
represent a risk assessment.
2.3 PCB LIGHT BALLAST/FLUORESCENT LIGHT TUBE INVENTORY
A visual survey was completed to determine whether fluorescent light ballasts contain PCBs and to
inventory the fluorescent light tubes present within the subject building. The determination of PCBs
in light ballasts was made by visually checking a representative number of ballasts. If the ballasts
are labeled as containing “No PCBs”, then they were considered to contain no PCBs, however, if
they were not labeled “No PCBs” they were assumed to contain PCBs.
2.4 MERCURY CONTAINING ITEM INVENTORY
A visual survey for mercury-containing items such as thermostat, switches and controls was
performed. CHA performed no analytical testing of any items to determine mercury content.
2.5 RECYCLABLE EQUIPMENT / MATERIAL INVENTORY
A visual survey for potentially salvageable / recyclable materials was completed throughout the
building in the event that the building is demolished and such materials and equipment could be
reclaimed.

2.6 LIMITATIONS
CHA inspected all accessible interior and exterior portions of the former Leonard Hospital located at
74 New Turnpike Road in Troy, New York. Specific areas throughout the ground and first floor have
previously sustained heavy fire damage and many building materials have been concealed by the
collapsed walls and ceilings throughout those spaces. In those areas, CHA assumed underlying
suspect materials based on materials observed in adjacent areas. Many partition walls have been
damaged, exposing the fiberglass insulation and non-insulated pipes running within. Although a
number of these chases and wall cavities were inspected during this survey and found to be free of
suspect/confirmed ACM, due to the presence of vermiculite insulation within a sheetrock wall on the
first floor and loose vermiculite debris on the fourth floor and penthouse level, additional currently
intact wall cavities may contain vermiculite. All wall cavities should be inspected prior to any
renovation project that may impact them.
The figures provided in this report were drafted from a partial set of schematic design drawings
located during the survey fieldwork. Room names and numbers were assigned to rooms by CHA
based on the prior room/space usage as observed in the field. The figures are not to scale.
3.0 HAZARDOUS MATERIALS SURVEY
CHA’s background research and field observations during the site inspection with regard to suspect
hazardous building materials associated with the former Leonard Hospital are discussed below.
3.1 RECORD REVIEW
The City provided CHA with a 2006 Appraisal of Real Property prepared by Alvey & DiMura, Inc.
of Latham, New York which included a general discussion of the overall building conditions (in
2006). The appraisal also stated that the building was constructed in 1972, vacated in 2001-2002,
and maintenance activities ceased in 2005.
CHA was not provided any record drawings, AutoCAD base drawings for the building, or
previously-completed asbestos survey reports. The figures provided in this report were drafted from
a partial set of schematic design drawings located in the building during the survey. The figures are

3.2 ASBESTOS-CONTAINING MATERIALS SURVEY
CHA performed an asbestos-containing material survey of all accessible interior and exterior
portions of the former Leonard Hospital including the ground floor, penthouse, and roof levels. The
objective of this survey is to identify and sample suspect asbestos-containing materials prior to
building renovations that will disturb them. The following paragraphs detail the observations made
and samples collected during the asbestos inspection. See Figures 1 through 7 for individual
building floor plans and bulk sample locations.
3.2.1 Exterior of Building
The former Leonard Hospital is a four-story brick and concrete structure with metal frame windows
and doors that are sealed along their perimeter with suspect asbestos caulk. The exterior brick wall
is cracked in many places and the cracks have been filled with a gray caulk that also considered
suspect for asbestos. The window, door, and brick wall caulks were sampled for asbestos laboratory
analysis. Six main roof levels extend over the building (see Figure 7 for each roof level and the bulk
samples collected). Roofs #1 and #2 have rubber roof systems extending over the entire field areas
and are flashed up the parapet walls. Multiple layers of built-up roofing exist below the rubber roofs
and are applied over a hard concrete deck. The rubber roof seam caulk, parapet wall flashing caulk,
paper on foam insulation below the rubber membrane, and the various built-up roofing layers are
suspect for asbestos and were sampled for laboratory analysis in accordance with Section 2.1 of this
report.
The remaining 5 roofs are all consistent; multiple layers of built-up roofing and a layer of granular
insulation on hard concrete deck. The built-up roofs are flashed vertically up each parapet wall and
each flashing is coated with suspect gray sealant. The coping stones atop the parapet walls have
suspect caulk sealing each horizontal stone joint. Black pitch pockets (for electrical wiring) were
observed on roof levels with air-handling units or other mechanical equipment and are suspect for
asbestos. All suspect materials observed on the remaining roof levels were quantified per roof and
sampled for asbestos laboratory analysis.
3.2.2 Interior of the Building
The interior of the former hospital has a ground floor that consists of mechanical spaces,
cafeteria/dietary and kitchen, storage rooms, offices, loading dock, and morgue. The floors
throughout are suspect vinyl floor tile with limited areas of bare concrete and clay tile. The walls are
a combination of sheetrock, plaster, ceramic tile, and vinyl cove base lining the perimeter of each

wall, while the ceilings are 2’ x 2’ and 2’ x 4’ drop ceiling tiles hung from the concrete deck with
limited areas of sheetrock and ceiling plaster. The vinyl floor tile and mastic, clay floor tiles and
grout, cove base and mastic, sheetrock and joint compound, wall and ceiling plasters, and ceiling tile
are suspect for asbestos. All suspect materials were quantified per room and sampled for asbestos
laboratory analysis.
The pipes running throughout the ground floor are covered with fiberglass insulation which has
suspect black mastic which adheres the insulation to the outer insulation paper. CHA observed
suspect mudded fitting insulation on pipe elbows and terminating ends of pipes. The metal air ducts
and plenums have suspect red sealant along the seams and tie-in locations. All suspect thermal
system insulation and duct sealants were sampled in accordance with Section 2.1 of this report.
The first floor consists of the main hospital entrance and reception, emergency room, operating
rooms, and offices. The finishes throughout the first floor are similar to those observed on the
ground floor. In addition to the previously-identified suspect materials, CHA also observed assumed
asbestos-containing transite fume hoods throughout the emergency room laboratories. These fume
hoods are quantified on Table A-2 of this report. Granular vermiculite insulation was observed
within one wall on the first floor of the hospital directly adjacent to the front entrance. Related
vermiculite debris was also observed within the rooms on either side of the wall. New York State
does not currently have an approved analytical method for detection of asbestos in loose fill
vermiculite insulation and as such, the vermiculite must be assumed to contain asbestos.
Spray-on fireproofing was observed applied to the structural steel and adjacent horizontal and
vertical surfaces throughout the building. The building has sustained water damage due to active
roof leaks, damage to ceilings and pipes hung from the concrete deck due to ongoing vandalism, and
general building decay. As such, suspect spray-on fireproofing debris was observed on the floors
beneath areas of ceiling damage throughout the building. The spray-on fireproofing was quantified
and sampled for asbestos laboratory analysis. See Section 4.0 and 5.0 for further discussion of the
spray-on fireproofing and related debris.
Three skylights exist (one at the top of each stairwell) the walls of which are sheetrock finishes
which appear visually different than the sheetrock finishes throughout the remainder of the building.
As such, CHA quantified and sampled the skylight sheetrock, sheetrock tape, and joint compound
separately from the remainder of the building.

3.3 LEAD PAINT SURVEY
CHA collected four paint chip samples from the interior of Leonard Hospital. The samples were
collected from the interior walls throughout the building. All samples were analyzed for lead
content by weight. Paint chip samples containing lead at concentrations greater than 0.5% are
identified by the United States Environmental Protection Agency (EPA) and Housing and Urban
Development (HUD) as a lead based paint, whereas any measurable amount of lead in paint is
enough to trigger the requirements of the OSHA Lead in Construction Standard. A summary of the
results is provided as Table B-1, located in Appendix B and the laboratory analytical reports are
included as Appendix D of this report.
3.4 PCB LIGHT BALLAST/FLUORESCENT LIGHT TUBE INVENTORY
CHA observed fluorescent light tubes associated with the light fixtures throughout the former
Leonard Hospital. Fluorescent light tubes contain trace amounts of mercury and are typically
handled as universal waste. In addition to the intact light tubes, CHA also observed broken
fluorescent light tube debris on the floors throughout the basement, first, second, third, and fourth
floors. In many cases, the light tube debris is limited to the corridors however debris was also
observed in offices, and patient rooms in heavily vandalized areas. The broken light tubes on the
floors throughout the building should be removed and disposed of as hazardous waste as part of the
overall renovation / demolition project. See Section 4.0 for an estimated quantity of light tubes per
floor.
In addition to the fluorescent light tubes, CHA also inspected a representative number of ballasts
associated with the fixtures throughout the building. The patient rooms generally have two wall-
mounted light fixtures, each with its own ballast. The ballasts within these fixtures are Mark III
Energy Savers made by Advance Transformer Company and are labeled as having “no-PCBs”. The
remaining light fixtures throughout the corridors, offices, and various medical suites have Watt
Reducer Rapid Start ballasts made by Universal Manufacturing Corp. and are labeled as having “no
PCBs”.
3.5 MERCURY CONTAINING ITEM INVENTORY
Many of the thermostat controls have been dismantled and the mercury tubes removed however
CHA observed approximately 22 wall-mounted mercury-containing thermostat switches throughout
the building.

3.6 CHEMICALS/HAZARDOUS MATERIALS CONTAINERS/DRUMS
There were numerous drums and other containers located on the subject site and throughout the
former Leonard Hospital at the time of the site inspection. These drums and containers are included
in the list below. It should be noted that CHA did not observe any pesticides or batteries throughout
the subject building during the survey fieldwork.

3.7 RECYCLABLE EQUIPMENT / MATERIAL INVENTORY
CHA visually surveyed the building for equipment and material that may be salvaged and recycled
prior to a building demolition. The following is a bulleted list of all potentially salvageable
equipment and materials observed during the survey fieldwork.

Recycling of the equipment and material listed above will likely be impacted by the presence of
confirmed asbestos-containing materials and asbestos debris located throughout the building. See
the tables attached to this report for ACMs that may impede building material recycling efforts.
4.0 RESULTS
CHA identified 118 suspect asbestos-containing materials during the asbestos survey within the
hospital and a total of 239 individual bulk samples were collected. Of the 118 suspect asbestos-
containing materials, 29 asbestos-containing materials were confirmed through laboratory analysis or
by assumption, as part of this survey. All confirmed and assumed ACMs are summarized in Table
4-1 below.

in Table A-1 located in Appendix A. Table A-1 lists each bulk sample, the location from which it
was collected, and also provides the analytical result for each sample. Table A-1 is not intended to
outline all locations within the building where ACMs exist. Table A-2, the Room by Room
Inventory, summarizes all suspect, assumed, and confirmed asbestos materials that were observed.
For each suspect material in each room, the asbestos content is provided. If the asbestos content is
greater than 1.0% asbestos by weight the material is considered an asbestos-containing material. An
estimated quantity is provided in Table A-2 for each confirmed or assumed asbestos-containing
material in each room or area.
Upon analysis, none of the four paint chip samples were found to have lead at levels at or above the
detection limit of 0.010% by weight and are therefore not lead-based paints. A summary of the
results is provided as Table B-1, located in Appendix B and the laboratory analytical reports are
included as Appendix D of this report.
5.0 CONCLUSIONS AND RECOMMENDATIONS
The conclusions presented in this report are based on information gathered in accordance with the
Scope of Work defined in this section of the report.
Based upon CHA’s review of the analytical results, historical data and observations made during the
inspection, the following conclusions and recommendations have been developed.

5.1 CONCLUSIONS
29 asbestos-containing materials were identified and confirmed in the former Leonard
Hospital by laboratory analysis.
Friable asbestos-containing spray-on fireproofing debris was observed above ceilings, within
chases, and on the floors throughout the building. The debris is likely a result of air
movement throughout the building due broken exterior windows, active roof leaks, and
ongoing vandalism. In addition to the spray-on fireproofing debris, vermiculite debris was
also observed on the first floor, fourth floor, and penthouse and is considered asbestos debris.
See the second bulleted recommendation below for direction regarding the debris observed.
Wall paint throughout the building was sampled and analyzed for lead content. None of the
samples were found to contain lead at or above the detection limit of 0.010% by weight.
Approximately 725 fluorescent light tubes are present within the former Leonard Hospital
and are assumed to contain minimal amounts of mercury. In addition to the intact light
tubes, large areas of broken tubes were observed throughout the building in corridors and
adjacent rooms where fixtures have been vandalized.
A representative number of fluorescent light ballasts were inspected as part of this survey
and were found to be labeled as having “no PCBs”.
While the mercury tubes have been removed from the majority of the thermostats throughout
the building, approximately 22 mercury-containing wall-mounted thermostats were
observed.
5.2 RECOMMENDATIONS
Based on the site inspection and analysis of samples, CHA has developed the following
recommendations:
Any confirmed or assumed asbestos-containing materials located in the subject building that
will be impacted by renovations must be removed prior to any work that would disturb them.
All asbestos abatement work must be performed by a licensed asbestos abatement contractor
in accordance with all local, state, and federal regulations.

Friable asbestos-containing spray-on fireproofing debris was observed above ceilings, within
chases, and on the floors throughout the building. Vermiculite wall insulation debris was also
observed on the first floors, fourth floor, and penthouse. NYSDOL requires that a site-
specific variance be submitted for the handling of any debris that is greater than 10 square
feet. The debris identified is greater than 10 square feet and therefore a site-specific variance
must be submitted for approval prior to proceeding with the immediate cleanup and
abatement activities. In the interim, the affected area(s) should be cordoned off with barrier
tape and only certified personnel with the proper PPE should enter the building until the
building has been cleaned per the approved variance.
The recycling or reclaiming of any building equipment and/or material will require
decontamination and cleaning in accordance with NYS Code Rule 56 and the provisions
outlined in an approved site specific variance.
While limited wall cavity and chase inspection was performed, not all walls and chases were
inspected as part of this survey. A thorough inspection of all walls and chases impacted by
upcoming renovations or demolition should be performed prior to any work that may impact
potentially additional suspect materials concealed within.
The fluorescent light tubes and light tube debris which were observed throughout the former
Leonard Hospital should be managed as hazardous waste/universal waste in accordance with
applicable state and federal regulations prior to renovation activities that could impact them.
Although all fluorescent light ballasts inspected were labeled as having “no PCBs”, if during
any subsequent building renovations or demolition, ballasts are encountered that are either
not labeled or the existing labeling contains no language with reference to PCB content, they
should be treated as being PCB-containing.
The mercury-containing thermostat gauges observed throughout the building should be
disposed of as hazardous/universal waste in accordance with applicable state and federal
regulations prior to any renovation activity that may impact it.
Many drums and containers containing various chemicals were observed throughout the site.
It is recommended that the owner hire a vendor to overpack the containerized materials and

dispose of them per appropriate legal means in accordance with all federal, state, and local
regulations.
This inspection report was completed by CHA to provide the client with general information
regarding hazardous materials observed throughout the building and shall not solely serve as
an asbestos abatement design document for obtaining bids from abatement contractors. It is
recommended that further inspection as discussed be completed and specific abatement
design documents be prepared relative to the scope of any renovation project in order to
specify procedures and protocols that must be observed in order to ensure that abatement
activities are completed in a manner consistent with local, state and federal regulations and
requirements.
