Block 8 Special Instructions – Dan16-0138 There were no energized underground 13.8KV Electrical Utilitiesdistribution lines or equipment identified within the boundaries of this excavation permit. 

If during the excavation activities, mechanical equipment may come within 10 feet of known energizedunderground and overhead lines or within 20 feet of power poles or utility equipment, notify the ElectricalSystem Dispatcher at 373-2321. 

This approval applies only to electrical power systems for which EU has operational control. You must obtainsign off and approval from facility owner (14b) for electrical systems not under the operational control ofEU. [BIER] No outriggers to be placed over PW and RW lines, located south of the Canton Ave and WTP PC LoopRoad. These lines are located approximately 42' and 36' from the center line of the road (Ref. drawings forwater lines & road: H-2-829681 (Sh 6 &7), H-2-829682 (Sh 1), H-2-829691 (SH 4), H-2-829692 (Sh 3 & 4)[HAQ]There are both raw water and sanitary water lines running along the north edge of this excavation site (seeH-2-830460 sheet 11 and H-2-830461 sheet 11). Excavations within 5 feed of these lines is hand dig only withapproval by MSA Water & Sewer Utilities (W&SU). Also, heavy vehicle crossings of those lines off normallytraveled roadways requires W&SU approval.[STEVENS] Approved Site Evaluation: 2E-08-11. [HARMON] To avoiddamage to the wells or prevent routine well maintenance and sampling activities, maintain a minimum of 20foot buffer of active wells and access roads. There are several In-Use groundwater wells within the workarea footprint. If any intended excavation work will be conducted within 20 feet of existing groundwaterwells, please notify the CHPRC Well Coordinator, Kelly Whitley, at 373-4929 prior to initiatingwork.[CAPELLE]

400 MHz antenna collected in two directions across each survey area with a 50nS window at 12 scans per foot. Depth of investigation is 2 feet.

Passive radio frequency and 60Hz search conducted around perimeter of each survey area. Active radio frequency conducted to exposed utilities.

North-south profiles collected over area shown on Figures 6 & 7. Profile spacing was approximately 10 feet

Results

Utility mark-outs and Mapping Site features and marked utility line were mapped in the field with a Trimble

Pro6H GPS.

The north eastern portion of the requested survey area could not be surveyed with the EM-31 due to dense sage.

As-built drawings show vitrified clay pipe in the survey areas. Electromagnetic induction will not detect VCP, GPR may detect VCP in certain situations.
