EVALUATION CONTRACT This is an agreement between Kelly Carr (hereinafter referred to as the Evaluator) and Joel Slater (hereinafter referred to as Principal). GENERAL INFORMATION Title of Project: Evaluation of Performance Matters Scope of Work: The evaluator shall provide all material and personnel to accomplish the deliverables described in this statement of work, except as may otherwise are specified. The scope of this task order and associated deliverables include (a) performing literature review using university-based search tools, (b) analysis of conceptual/theoretical basis of instructional tactics, methods, and activities, (c) evaluation of alternative approaches to teacher training, (d) preparation of agreed-upon evaluation questions that will drive and guide the evaluation, (e) creation and administration of a database for data collection in this evaluation, (f) data collection using methods and techniques that ensure valid and reliable data that relate directly with the agreed-upon evaluation questions, and (g) preparation of reports and briefing documents and revising as required. WORK STEPS Work steps shall include the following: (a) analyze school district regulations and other state and county requirements and list evaluation requirements, (b) conduct literary database search relative to the type of program being evaluated and tools and techniques used elsewhere in similar project, (c) develop and obtain approval of evaluation question that will drive data collection and findings, (d) prepare an evaluation work flow and schedule, (e) identify evaluation methods, means, processes, and techniques and relate each with evaluation questions, (f) identify and document data collection methods selected, (g) define data collection sites, sample, and resources required, (h)

EVALUATION CONTRACT

GENERAL INFORMATION

FIELD VISITSAll fieldwork and on-site observations shall be coordinated with the

office of the superintendent and/or the principal of Austin Middle School.

PROJECT MANAGEMENTThroughout the performance period of the delivery order executed by

this contract, the contractor shall keep the principal and professors fully informed as to issues and progress.

Background: This contract is a follow-up to a quick-response, short-term effort to investigate the usage of Performance Matters and the training for teachers at Austin Middle School in the Paulding County School District.

Performance Period: The work shall begin within 5 days of award, unless otherwise specified. Work at the site shall not take place on school holidays or weekends unless directed by the contracting officer.

Type of Contract: Time and materials. With fixed-price limit.

CONTRACT AWARD MEETINGThe evaluator shall not commence performance on the tasks in this

contract until the contracting officer has conducted a kickoff meeting or has advised the contractor that a kickoff meeting is waived.

GENERAL REQUIREMENTS1. The evaluator shall confirm work assignments by telephone, e-mail,

or in person with the principal. A brief outline of work approach shall be reflected in the technical proposal.

2. All written deliverables shall be phrased in acceptable terminology of the field. Words shall be defined in layperson language. Statistical and other technical terminology shall be defined in a glossary of terms and referenced for validity and usefulness.

3. Unless otherwise specified, where a written deliverable is required in draft form, the school district office shall complete their review of the draft deliverable within 10 calendar days for the date of receipt. If there is no response from the district within 10 calendar days, it shall be automatically deemed “approved.” The evaluator shall have 10 calendar days to deliver the final deliverable from date of receipt of the government’s comments.

4. This contract shall not require access to student files.

5. All deliverables, except where specified otherwise, shall be provided in one electronic copy via e-mail. All deliverables shall be delivered via software compatible with the school district office.

SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLESDescription of Tasks and Associated Deliverables

The evaluator shall provide the specific deliverables described below:

evaluation questions. The school district shall review the draft review and provide written comments to the evaluator no later than 10 calendar days after receipt of the draft review. The evaluator shall then submit the revised review no later than 20 calendar days after receipt of comments. If there is no response from the district within 10 calendar days, it shall be automatically deemed “approved.” The evaluator shall update the review as required by the guidance from the principal or designated representative. The deliverable shall be in electronic form transmitted via e-mail, using software compatible with software available at the school district office.

Task 2: The evaluator shall provide detailed analysis of selected topics and issues identified in the evaluation report number, and type of topics shall be in accordance with guidance from the principal. Work shall proceed on a time-and-material basis not to exceed specific financial limitations of the purchase order. The evaluator shall present significant and timely findings using an appropriate contractor-developed format in paper and electronic form.

If for any reason, any deliverable cannot be delivered within the scheduled time frame, the contractor is required to explain why in writing to the principal, including a firm commitment of when the work shall be completed. This notice to the principal shall cite the reasons for the delay and the impact on the overall project. The principal shall then review the facts and issue a response in accordance with applicable regulations within 10 days.

CHANGES TO STATEMENT OF WORK

Any changes to this statement of work shall be authorized and approved only through written correspondence from the project manager. A copy of each change shall be kept in a project folder along with all other products of the project.

REPORTING REQUIREMENTS

1. The evaluator is required to provide the principal with progress reports of the evaluation after each survey is conducted.

2. The progress reports shall cover all responses to the survey questions during each session. The report shall identify any problems that arose during the survey session and a statement to explain how the problem could be resolved.

TRAVEL AND SITE VISISTS

Travel and on-site visits shall be authorized by the principal.

SCHOOL RESPONSIBILITIESThe school shall provide access to technical and procedural

information. The school shall provide a copy of the required confidentiality statement at contract award or upon request by the contractor.

CONTRACTOR EXPERIENCE REQUIREMENTS

The Evaluator will perform this evaluation as an authentic learning experience in the Program Evaluation course (MEDT 8480) in the Ed.S. program at the University of West Georgia, College of Education, Department of Media and Instructional Technology. 1. The preliminary and final deliverables and all associated working papers and other material deemed relevant by the school district that have been generated by the contractor in the performance of this task order are the exclusive property of the school district and shall be submitted to the principal at the conclusion of the evaluation initiative.

2. The principal shall be the sole authorized official to release verbally or in writing any data, the deliverables, or any other written or printed materials pertaining to this task order. No information shall be released by the contractor. Any request for information relating to this task order presented to the contractor shall be submitted to the principal for response.

3. Press releases, marketing material, or any other printed or electronic documentation related to this project shall not be publicized without the written approval of the principal.

4. The Evaluation Contract, Evaluation Plan, and Final Evaluation Report will be submitted to Dr. Linda Haynes and Dr. Danilo Baylen at the University of West Georgia as partial fulfillment for the Program Evaluation course (MEDT 8480) in the Ed.S. program at the University of West Georgia, College of Education, Department of Media and Instructional Technology. The following schedule must be met for course requirements.
