This section should be used to record feedback given to the student by
the mentor/supervisor on a regular basis.
Feedback is important to ensure the student fully understands the
assessment requirements and how well they are progressing.
Progress Review (at least 6 weeks prior to the Final Assessment)
This review must be completed by the hub placement mentor at least six weeks
before the end of the practice learning period for the year. The student must
have a minimum of four weeks opportunity to address any weaknesses identified
at this point.
Where this is not achievable the Practice Team must be informed
Student to complete the self-assessment on the Progress Review sheet
in preparation for Progress Review interview.
1. I still need/would like to develop the following areas of my practice:
2. My current Practice Learning Plan (agreed at initial interview) will facilitate
the development needs specified above

If “No” please indicate why below and update and amend your Practice
Learning Plan with your hub placement mentor.

If the student has not completed one
or more of the requirements indicated
and ‘No’ is recorded, an action plan
must be commenced so that the
student understands exactly what is
required by the end of the placement.
An action plan should be viewed as a
positive means of enabling the
student to meet the assessment
requirements
It is essential that the Practice Team is
informed if an action plan is
commenced so that additional support
can be offered to both the student and
the mentor

SAMPLE ACTION PLAN
Domain/s
Performance criteria/Key Skills
In order to make it clear to students which aspects of the assessment they are at risk of failing, the
concerns should be linked to the domains of nursing and the name of the Essential Skills Cluster the
concern/s relates to. Insert reference to PAD
E.g. Domain: Nursing Practice and Decision Making - ESC: Organisational Aspects of Care Domain: Leadership, management and Teamwork - ESC : Organisational aspects of care
Issues Identified/Concerns
Specify:
• What has the student done/not done in relation to the performance criteria?
• What knowledge deficit is there?
• What skill deficit is there
E.g. Samantha has made repeated mistakes recording observations and when questioned is unable to interpret and discuss findings and use the information obtained to inform care plans. For example Samantha was allocated a small caseload, and made errors completing fluid balance charts, and plotted blood pressures incorrectly. Samantha failed to report observations outside of normal parameters, Samantha does not appear to learn from experience and needs excessive prompts to perform tasks correctly, for example acting appropriately on findings, and charting observations. Actions Required
Set ‘SMART’ actions
Specific goals should be written clearly and concisely
Measurable success criteria should be identified
Achievable determine what is achievable
Realistic determine if the goal is realistic
Timely identify what has to be done by when and by whom
E.g. Accurately complete and record blood pressure
Access additional skills training to improve blood pressure taking skills Work under direct supervision of the mentor whilst taking and recording blood pressure and report findings Revise physiology of blood pressure control, normal values, and significance when blood pressure falls outside normal parameters and prepare to discuss learning with mentor
Complete fluid balance charts independently Fluid balance charts to be checked and countersigned by mentor over the next week.
Review date/s weekly
To be achieved 24.8.12 26.8.12
Notes of Review meetings
Record progress / review meetings
Record progress towards meeting actions.
State clearly if actions are achieved.
E.g. Samantha has accessed additional skills training in the university, and is confident in the technical aspects of taking blood pressure, and plotting the findings. Samantha’s knowledge has improved, in that she is able to recall factual information, but she has difficulty applying this to practice. Agreed that Samantha will carry out blood pressure assessments unsupervised, but must continue to discuss findings. Samantha is able to accurately chart fluid balance intake and output, but has difficulty totalling fluid balance. Discussed with the tutor, and Samantha has been referred to the

This sample action plan is included as a guide. The
Practice Team can provide additional help with the
action planning process
Special Needs Tutor for advice. Review next week. Samantha said she has been seen by the Student Enabling Centre and does not have any special needs in relation to numeracy skills. Advised to use calculator to check findings, and continue to report findings to the mentor. Samantha understands the basic principles of fluid balance, but has difficulty relating this to individual patient care. Application of theory to practice seems to be a recurring problem. Agreed that Samantha will take the lead for care planning under direct supervision to improve decision making skills and application of theory to practice. Samantha is able to accurately complete fluid balance and blood pressure, and can recognise deviations from normal. Application of knowledge to practice is improving, but is not yet at the expected standard, slow but continuing improvement is seen. Review in 4 weeks.

It is important that the action plan is
reviewed regularly, at least weekly, and
written feedback given to the student
regarding their progress. If the student
is still not working to the required level
this should be clearly discussed with
the student and documented here by

Final Assessment and Timesheet
The level of competence at the final assessment point should be made in
relation to each domain overall in terms of achievement of the LEVELS
OF PERFORMANCE, SUPERVISION & PRACTICE, see page 61.
Assessment is based on:
feedback from mentors/supervisors on pathway (spoke) placements
feedback from other staff working in the hub placement area
 feedback from service users/carers
evidence of achievement of ALL essential skill clusters
completion of the specified written evidence for EACH domain
other written evidence provided in support of achievement of the essential
skills clusters
direct observation and question and answer sessions
Mentor Information: Retrieval of Practice Hours
If the student has not completed the required 667.5 hours of placement the hub
placement mentor is still required to complete an assessment at this point in
accordance with NMC requirements. If the mentor has had insufficient time to work with
the student due to sickness/absence this should be indicated in the reasons for non-
achievement on the Final Assessment page. The student is advised to submit extenuating
circumstances before the submission date if he/she wishes the sickness/absence to be
taken into account in relation to this practice assessment.
The student will return to the hub placement area (unless specific competencies need to
be completed in a different area) to retrieve any outstanding hours during the retrieval
placement weeks commencing 9th December 2013. The student is required to work full-
time until all hours have been retrieved unless advised differently by the placement area
or the Practice Team. If less than 37.5 hours need to be retrieved in one week the
student is only required to retrieve the actual number of hours outstanding. The student
will not appear on the allocation report for the placement area, as this is retrieval of
hours not planned hours.
Student Information: Submission of Practice Assessment Document (PAD)
You MUST submit this PAD to the relevant Student Office on the date given and attach
the relevant barcode sheet from evision. If the PAD has not been completed for a specific
reason, e.g. sickness/absence you may request a 1-week extension through E:vision but
you ill be required to provide evidence in support of the application. Contact the Practice
Team for further advice if required. This will allow you time to meet with your mentor
and complete the documentation. It does not allow an extra week on placement. If
having an extension will not resolve your difficulties or if you were off sick/absent for a
prolonged period of time at the end of the placement, you are advised to apply for
Extenuating Circumstances, as any incomplete or non-submitted documents will be
recorded as a refer (fail) in practice and you will only be allowed one further attempt.
Final Assessment
Student to complete
You must complete the self-assessment on the Final Assessment sheet
in preparation for the Final Assessment interview with your Hub
Placement mentor. You should also answer the following questions:
1. Have you achieved the objectives of the Placement Learning Plan?
YES/NO*
2. Comment on your key strengths and areas for improvement/priorities for your next placement
