This section should be used to record feedback given to the student by

Feedback is important to ensure the student fully understands the

assessment requirements and how well they are progressing.

Samantha failed to report observations outside of normal parameters, Samantha does not appear to learn from experience and needs excessive prompts to perform tasks correctly, for example acting appropriately on findings, and charting observations. Final Assessment and Timesheet

The level of competence at the final assessment point should be made in

relation to each domain overall in terms of achievement of the LEVELS

OF PERFORMANCE, SUPERVISION & PRACTICE, see page 61.

Assessment is based on:

� feedback from mentors/supervisors on pathway (spoke) placements

� feedback from other staff working in the hub placement area

� feedback from service users/carers

� evidence of achievement of ALL essential skill clusters

� completion of the specified written evidence for EACH domain

� other written evidence provided in support of achievement of the essential

skills clusters

� direct observation and question and answer sessions

Mentor Information: Retrieval of Practice Hours

If the student has not completed the required 667.5 hours of placement the hub

placement mentor is still required to complete an assessment at this point in

accordance with NMC requirements. If the mentor has had insufficient time to work with

the student due to sickness/absence this should be indicated in the reasons for non-

achievement on the Final Assessment page. The student is advised to submit extenuating

circumstances before the submission date if he/she wishes the sickness/absence to be

taken into account in relation to this practice assessment.

The student will return to the hub placement area (unless specific competencies need to

be completed in a different area) to retrieve any outstanding hours during the retrieval

placement weeks commencing 9th December 2013. The student is required to work full-

time until all hours have been retrieved unless advised differently by the placement area

or the Practice Team. If less than 37.5 hours need to be retrieved in one week the

student is only required to retrieve the actual number of hours outstanding. The student

will not appear on the allocation report for the placement area, as this is retrieval of

hours not planned hours.

Student Information: Submission of Practice Assessment Document (PAD)

You MUST submit this PAD to the relevant Student Office on the date given and attach

the relevant barcode sheet from evision. If the PAD has not been completed for a specific

reason, e.g. sickness/absence you may request a 1-week extension through E:vision but

you ill be required to provide evidence in support of the application. Contact the Practice

Team for further advice if required. This will allow you time to meet with your mentor

and complete the documentation. It does not allow an extra week on placement. If

having an extension will not resolve your difficulties or if you were off sick/absent for a

prolonged period of time at the end of the placement, you are advised to apply for

Extenuating Circumstances, as any incomplete or non-submitted documents will be

recorded as a refer (fail) in practice and you will only be allowed one further attempt.
